[Epithelial splenic cysts].

The authors describe a case of an epithelial cyst of the spleen treated successfully by splenectomy. Diagnostic techniques and the principles adopted in treatment this rare disease are discussed.

[The ND-Yag laser in the treatment of non-resectable esophageal neoplasms: evaluation and results].

The Authors show a set of 9 cases of patients suffering from oesophagus flaky carcinoma, judged radically non-operable, and treated with endoscopic laser-therapy by N.D. Yag Laser. They illustrate the patients' main clinical data, method followed, complications observed and results obtained. What seems to emerge from the analysis of the data is: the effectiveness of laser-therapy in the control of disphagia, simplicity of the technique and its acceptability by the patient, as well as the rareness of complications. Three are the clinical parameters used for the checking of patients before and after the laser treatment: disphagia, odynophagia and thoracic ache. The intenseness of symptoms was classed from 0 up to 3. Before the treatment, six patients out of nine showed class 3 disphagia; after the treatment, two could assume fluids (class 2), two fed also on minced foods (class 1), and two fed normally; in the two cases with initial class 2 disphagia a normal feed could be adopted; in one case no initial disphagia was present. Odynophagia was initially present in one case only at class 3, in three cases at class 1, and in all the other cases it was absent (0). After the treatment, odynophagia disappeared totally, except in one class 1 case, which remained unchanged. The presence of thoracic ache before the treatment was class 1 in four cases, class 3 in one case, and absent in four cases. After the treatment, the class 3 ache turned into class 1: in one class 1 case it was not modified; in the remaining cases the ache, if present, disappeared. Some questions remain opened, concerning the effect on survival, even in relation to other palliative methods, to the duration of the palliation obtained and to the eventual associability of lasertherapy to radiotherapy.

Muzolimine and nitrendipine in the treatment of arterial hypertension.

Thirty patients with moderate to severe hypertension (diastolic blood pressure greater than or equal to 115 mmHg), after a run-in wash out period of 15 days, were treated with muzolimine at a dosage of 20 mg once daily, given at 1 p.m., for three weeks. At the end of this period of treatment the patients with diastolic blood pressure greater than or equal to 100 mmHg started a double blind randomized study of comparison of nitrendipine, a vasodilator calcium antagonist agent, and captopril, an inhibitor of converting enzyme. The dosage was 10 mg twice daily for nitrendipine and 25 mg twice daily for captopril, the duration of each treatment being four weeks. At the end of 28 days of double blind treatment, the patients with diastolic blood pressure greater than or equal to 100 mmHg were treated with a triple combination: muzolimine plus nitrendipine plus captopril at the same dosage for a further four weeks. At the end of run-in and of each period of treatment blood pressure and heart rate were recorded in supine and erect positions and after a treadmill exercise test. At these times laboratory tests, including PRA and aldosterone, were performed. After the run-in period supine blood pressure was 189.6 +/- 13.9/123.1 +/- 7.7 mmHg. At the end of muzolimine treatment, supine blood pressure was 176.5 +/- 10.8/117.8 +/- 5.6 mmHg (p less than 0.001); at this time 4 patients had their diastolic blood pressure normalized and left the study. [Hepatic cystadenocarcinoma: presentation of a clinical case and review of the literature].

The finding of a case of cystadenocarcinoma supplies the opportunity for a reviewing of the literature concerning this rare liver neoplasm. From a diagnostical standpoint, the instrumental tests result non-specific. Only the needle-biopsy of papillary formations set forth through the echography within hepatic formations of cystic type is able to supply diagnoses of cystadenocarcinoma. Yet, the procedure is not exempt from risks. Decisive may be the peroperative histologic testing on fragments of the cyst wall. In consideration of the better prognosis of cystadenocarcinoma in respect of hepatocarcinoma and cholangiocarcinoma, and the risk of relapse, the election surgical treatment should be represented by hepatectomy.

[Surgical treatment of pulmonary metastases of colorectal cancer].

The pulmonary metastatic locations due to colorectal cancer were considered, in not remote times, not worthy of surgical treatment. At present, according to our experience and some other Authors' one, the surgical removal of metachronal pulmonary metastases should be considered suitable to improve the period of life free from symptoms and the survival. The surgical treatment has improved the survival in the single or double pulmonary locations, whereas the monolaterality of multiple pulmonary lesions, in respect of the bilaterality, was not significantly bettered the prognosis (Hiroshi Takita, Francis Edgerton). The survival, moreover, seems to be affected by the interval free from illness and by the doubling time of the metastatic location. The surgical indication to the pulmonary metastasectomy is done on condition of the absence of any metastases in other organs. In our experience, out of 742 interventions with radical purposes for colorectal cancer, we documented 15 cases where lung appeared as the first and sole metastatic filter (many patients were lost for their non-cooperation in the follow-up). In 7 cases pulmonary resections were performed, in patients selected and free from secondary locations in other organs, with a satisfactory result. Casuistry and personal results are reported.

[Pregnancy and enterostomy].

The authors describe the difficulties and possible complications of pregnancy in colonostomized women. In the neoplastic forms, generally appearing in advanced age, the knowledge of the basic disease discourages any possible attempt at pregnancy. In the chronic inflammatory diseases (ulcerous rectocolitis and Crohn's disease), typical of young age, pregnancy, if opportunely followed and programmed, can get an absolutely favourable course. The authors report their own experiences and some other authors' ones.

[Surgical therapy of gastric carcinoma in an advanced stage (clinical cases)].

The Authors study the casuistry of the Verona Surgical Clinic concerning the interventions for gastric cancer; they particularly study 35 cases of plurivisceral demolition for neoplasm in advanced phase. After a discussion of the data supplied by the literature; the Authors, as a conclusion, suggest not to consider the overcoming of the gastric barrier in multivisceral sense as a rigid element for the exclusion of these patients from surgery, although they exclude widely demolitive interventions in seriously weakened patients, or in bearers of such spreadings as to involve tremendous operative risks, not rewarded with a satisfactory advantage.

[Benign epithelial neoplasms of Vater's ampulla. Usefulness of the "epithelial dysplasia" concept in prognosis and surgical strategy].

The surgical behaviour in front of the benign epithelial tumors (adenomas) of Vater's ampulla is object of discrepancies due to the possible malignant evolution of such neoplasms. The authors stress the basic importance of a careful examination of histological bioptic material before and/or during operation, which, through the identification of the degree of epithelial dysplasia, may lead to the choice of the tecnique to be adopted. In 2 of the 3 cases reported by the authors, showing a low degree dysplasia of the papillary tumor, a submucous papillectomy was performed; in the third case, where histology showed severe dysplasia and some other elements of risk were present (patient's age and macroscopic features of the tumors), the surgeons performed a duodenopancreatectomy. Semiseriated histological sections of the surgical specimen revealed the presence of carcinomatous areas. The authors think simple papillectomy to be the surgical procedure to follow when low grade dysplasia of ampulla is documented, and duodenopancreatectomy to be performed in high risk conditions as high grade dysplasia and macroscopic features suggestive of malignancy. In addition, the authors deal with epidemiology, clinics and instrumental diagnostics of such benign neoplasms of ampulla, on the basis of their own experience and the review of the few cases reported in literature.

[Primary seminoma of the mediastinum: diagnostic and therapeutic approach].

The Authors show three cases of mediastinal seminoma, and discuss the epidemiologic, etiopathogenetic and clinical aspects of such neoplasms; they dwell upon the criteria allowing to define as primitive an extragonadal germinal tumour, and emphasize the importance of the multidisciplinary therapeutical approach in controlling these neoplasms, characterized anyhow in the pure forms by a remarkable sensitiveness to radium; promising looks the use of the tumoral markers (beta-HCG and AFP) for a diagnostical specification, for the choice of the treatment and for the evaluation of its effects.

[Primary malignant neoplasia of the exocrine pancreas: epidemiological findings, risk factors, early diagnosis. Personal case reports, 1970-1981].

The Authors report their own casuistry of malignant tumours of exocrine pancreas, collected in the period from 1970 up to 1981. The question was of 214 cases, namely 136 of head and 78 of body-tail or diffused. The male/female ratio was 2.3:1, the most affected decades were 6th and 7th, with a range of 27-86 years. The resecability was 27% for tumours of head and 24% for those of tail; the operative mortality was 10.8% and 17.6% respectively; the survival after 3 years was 6.8% (head) and 5.8% (body-tail), after 5 years 6% (head) and zero (body-tail). In analysing the etiopathogenetic rôle of some factors and some pathological associations, the Authors particularly point out the predisposing rôle of both tobacco smoke and diabetes, remarked in 50% and 33% of the cases observed, respectively. From a symptomatologic standpoint, the symptoms are subdivided into initial symptoms (retrospectively interpreted as first manifestation of the disease), symptoms of alarma (leading to the attending physician's), symptoms at the entrance (hospitalization), and importance is given to the exploitation of the initial symptoms for the purpose to reach a diagnosis as precocious as possible, since 40% of the tumours of head are hospitalized only after 8-16 weeks and 33% of the tumours of body-tail after 16-32 weeks from the arising of the first symptom.

[Cystic neoformations of the retroperitoneal space (4 case reports)].

Based on the study of four cases of retroperitoneal cysts, the complex histopathogenesis of these neoplasms - the clinical diagnosis of which has recorded substantial progress thanks to the use of the computed tomography - has been analyzed. The practice of exeresis, though comparatively simple and radical in most cases (as recorded in 3 out of the 4 cases reported) owing to the little adhesions of these neoplasms to the contiguous anatomical formations and also to their histological benignity, may, in some instances, present such unexpected difficulties due to the expansion in volume and the complex and delicate anatomical connections, that palliative operations have to be opted for.

[Bilemia. Considerations apropos of a case].

The Authors report a case of bilemia, a complication of hepatic biopsy. They analyse the physiopathogenetic problems and those of surgical tactics, and emphasize the importance of retrograde and/or interoperative cholangiography ii the diagnostics of such disease.

[Bilemia: diagnosis and conservative treatment. Apropos of a case complicated by hepatic abscess].

The authors report a case of bilemia of their own--with traumatic genesis--complicated by liver abscess, treated through outside biliary drainage and piloted transhepatic percutaneous drainages along the line of the echotomography and computerized axial tomography (T.A.C.). They report the treatment adopted, and emphasize the importance of the instrumental methods followed in the diagnostical and therapeutical approach of both diseases.

[Vascular problems in destructive pancreatic surgery].

Obstacles of vascular type may hinder or prevent pancreatic destruction for tumours of the pancreas or periampullary tumours. Out of 72 major pancreatic demolitions (cephalic duodenopancreatectomy or subtotal pancreatectomy from rt to lt) there were vascular difficulties on 26 occasions (36%), of which 15 were in existence prior to development of the neoplasia (anomalies of rise and course of peripancreatic vessels, stenosis/obliteration of the common hepatic artery in pancreatic head or periampullary tumours) and 11 due to tumoral growth (direct involvement of the mesenteric-portal tract, the hepatic artery and the superior mesenteric artery). The technical solutions used are discussed individually in the light of the physiopathological repercussions consequent on any temporary or permanent interruption of important hepatic and splanchnic vascularisation vessels (hepatic artery and various mesenteric a.), in relation to operating mortality and the prospects of long-term survival. In the most complex cases of direct vascular involvement by the neoplasia, the Author's trend was orientated to a position which, while accepting demolition decisions useful for the widening of the surgical radicality margins, nevertheless avoids extensive demolition solutions imposed by conditions of necessity or considerations of principle that probably do not bring significant advantages as regards long-term prognosis for these tumours.

[Cavitary lesions and carcinoma of the pancreas. Diagnostic difficulties and errors].

Among 197 pancreatic and periampullar tumours operated between 1970 and 1979, an associated cystic of pseudocystic lesion was found on 15 occasions (a rate of 16%). In 10 cases it was a question of pseudocysts located at the bodytail, while the tumour occupied a distinct site (periampullar tumour or carcinoma of the head); in 5 cases the cavitary lesion coincided with the tumour (2 endocrine tumours of the head, 2 cystoadenocarcinomas and one adenocarcinoma of the body-tail). In 6 cases (of which 5 operated) the error in diagnosis led to misappreciation of the tumour: only 3 of these cases could be treated with radical surgery. In the other 9, correct diagnosis was possible from the start and in 6 radical surgery was performed. Stress is placed on the need for prompt and early diagnosis, in order to allow adequate treatment of the tumour in line with the canons of radical action.
