[A case of so-called carcinosarcoma of the lung].

A 65-year-old male was admitted to our institute because of bloody sputum. A tumor in right S6 was detected by X-ray, CT and MRI. Bronchoscopic study showed that the right lower bronchus was occluded by the tumor, in which non-epithelial malignant cells were detected. Therefore right bilobectomy was performed. This tumor was a pedunculated endobronchial type measuring 6 x 4 x 3cm. Histologically, the tumor presented carcinomatous (squamous cell carcinoma and adenocarcinoma) and sarcomatous elements. Immunohistologically, many malignant cells were positively stained by vimentin and muscle-actin, which suggested differentiation from muscle components.

Construction of representative immunoglobulin variable region cDNA libraries from human peripheral blood lymphocytes without in vitro stimulation.

We have developed a sensitive and specific method for preparation of representative IgM and IgG cDNA libraries of peripheral blood lymphocytes without in vitro stimulation of B cells and without primer-based bias. The procedure involves preparation of double-stranded cDNA from initial C mu and C gamma constant region primers. Linkers are attached to the cDNA and nonselective polymerase chain reaction (PCR) amplification is performed with the linkers as primers. This product is ligated to M13 RF DNA, and selective PCR amplification is achieved with a nested constant region primer and a vector primer. The product is cloned into M13 phage. More than 85% of the white plaques are positive with a JH probe. Diversity of the library is confirmed by hybridization with VH gene probes and by DNA sequencing. A sample of 7-30 ml of blood from a normal human adult can yield both mu and gamma cDNA libraries. Analysis of the libraries yields a picture of Ig variable gene usage at the time of sampling.

Empirical estimation of the gel to liquid-crystalline phase transition temperatures for fully hydrated saturated phosphatidylcholines.

Phospholipids are a major component of biological membranes. In excess water, phospholipids may self-assemble into fully hydrated lamellae which, upon heating, may undergo the gel to liquid-crystalline phase transition at the characteristic temperature, Tm. Our present knowledge about the Tm values for various phospholipids is far from complete, although it is necessary to know the Tm value for preparing liposomes. In this study, we have derived empirically a general expression of Tm = 154.2 + 2.0(delta C) - 142.8(delta C/CL) - 1512.5(1/CL) in which two apparent structural parameters, delta C and CL, of a phosphatidylcholine molecule and their ratio, delta C/CL, are applied to estimate the Tm value of the phosphatidylcholine bilayer in excess water. The parameter delta C is the effective chain-length difference, in C-C bond lengths, between the two acyl chains for the phosphatidylcholine molecule in the gel-state bilayer, and CL is the effective length of the longer of the two acyl chains, also in C-C bond lengths. A figure containing 163 calculated Tm values is presented, and this information will be useful as a guide for designing experiments.

Plasma renin activity in renal transplant patients with hypertension.

Chronic renal allograft rejection (CR) is the major cause of graft loss after 1 year following transplantation. Hypertension (HTN) and proteinuria (Up) are the hallmarks of this immunologic allograft response. Over 1 year, 23 patients posttransplantation and with HTN (diastolic greater than 95 mm Hg) were examined. Serial serum creatinine (Scr), Up, urine creatinine (Ucr), and resting and enalapril challenged (EC) plasma renin activity (PRA) were all determined. Renal artery stenosis was excluded by noninvasive imaging. Of the 23 patients, eight eventually lost their renal allografts, while 15 maintained satisfactory renal function (Scr less than 2.5). Results of this study show that: 1) worsening renal function (RF) increases EC reactivity; 2) the presence of proteinuria is not itself related to increase in EC activity; 3) renin before and after EC was higher (P = .06) in the allograft loss group. In this study, patients with high PRA represent the highest EC responses. Therefore, they display not macrovascular lesions, but microvascular intrarenal disease; this defect is most likely secondary to chronic immunologic insult.

Folding of red blood cells in capillaries and narrow pores.

The geometric features of red blood cells in narrow channels in vivo and in vitro were studied by electron microscopy. In rabbit myocardial capillaries about half of the red cells were folded. In polycarbonate filters with pore diameters of 2.2-4.5 microns approximately one third of the trapped red blood cells were folded. The frequency of folding did not depend on the applied pressure, which ranged from 0.1 to 8.0 cm H2O. The folding of the red blood cells in filter pores was used to estimate the bending stiffness of the membrane. An analysis based on the large deformation theory of bending of an elastic sheet was developed. Using pressures of 0.2 and 1.0 cm H2O, the bending stiffness of human red cell membranes was estimated to be approximately 2.4 - 11.6 x 10(-12) dyn-cm, which is in good agreement with other methods. A limiting radius of curvature of about 85 nm was found at higher pressures.

[Effects of high-pass filters with different frequencies on signal-averaged electrocardiograms].

The results of the root mean square voltage of the terminal 40 ms (V40), the duration of the low amplitude signal of less than 20 muV (D40) and the duration of the signal-averaged QRS complex (QRSr) after high-pass filters with 5, 10, 15, 20, 25, 40, 80, 100, 150 and 200Hz were reported. The results showed that the signal-averaged electrocardiogram (SAECG) was highly influenced by the frequencies of the filter. According to the specificity and sensitivity after different high-pass filters, 25Hz was considered as the optimal high-pass filtering frequency.

[Role of surgical treatment in thoracic empyema with bronchopleural and/or thoracic empyema].

Sixty-nine patients with thoracic empyema treated surgically were experienced from May, 1978 through December, 1990. Thirty-nine cases had bronchopleural and/or thoracic fistula. Thirty-two patients were associated with pulmonary tuberculosis, of whom fourteen had tuberculous empyema and eighteen were sequelae of pulmonary tuberculosis or tuberculous pleurisy. The remainder were postoperative, postpneumonic, and posttraumatic empyemas. Of fourteen patients who developed postoperative bronchopleural fistula, there were ten patients who had lobectomy or pneumonectomy for lung cancers. Omental pedicle flap method, in which empyema space was filled with the omentum and pedicled muscle flap, was performed on 19 patients with bronchopleural or thoracic fistula or both. Fifteen patients were cured successfully by single-stage procedure, though there was one operative death due to aspiration pneumonia, and two recurrences which were treated by muscle plombages. There was another patient who had multiple surgical procedures in the past resulting in partial recurrences, but the fistula of this patient subsequently closed without reoperation. Postoperative decrease of %VC, FEV1.0/PVC were minimal. Treatment of long standing bronchopleural fistula is a difficult problem, and our omental pedicle flap method is relatively simple and safe which can be most suitably applied to those patients in whom other procedures have failed and to those with poor pulmonary functions.

[Cloning and expression of Shiga-toxin gene from Shigella dysenteriae type I].

The chromosomal DNA of S. dysenteriae type I W30864 was isolated and digested by EcoRI. The 3-7 kb DNA fragments were recovered and ligated with vector pUC-19. After transformation, the recombinants were screened by SLT gene probe. The positive clones were obtained. The cloned EcoRI fragment containing both ST-A and ST-B subunit gene was about 4.5 kb. The cloned ST strain was also detected by Hela-S3 cell for cytotoxicity, and detected by rabbit ileal loop test for enterotoxicity. Besides, the cloned strain showed the neurotoxic activity when experimented with mouse. The production of shiga toxin in the cloned strain was 16 times of that of its parent strain S. dysenteriae W30864. The production differences between ST producing stains and SLT producing strain was also tested in our experiment.

[A clinical study of small advanced lung cancer with metastasis to mediastinal lymph node].

From January 1981 through December 1989, 15 patients with small advanced lung cancer were treated surgically at the Tenri Hospital. In these cases, the diameter of peripheral lung cancer did not exceed 3.0 cm (T1) and mediastinal lymph nodes were proved to be N2 postoperatively by lymph node dissection or sampling. The histological types were as follows: 8 adenocarcinoma, 4 large cell carcinoma, 1 squamous cell carcinoma, 1 small cell carcinoma, and 1 adenosquamous carcinoma. All but one patient were received postoperative chemotherapy and/or radiotherapy. The survival rate was 44.5% at 3 years, and median survival time was 36 months. The mediastinal lymph node metastasis with small peripheral lung cancer (T1N2) was ominous, and it should be said that complete mediastinal lymph node dissection and adjuvant therapy were indispensable to small advanced adenocarcinoma of lung.

[A clinical study on bilateral metachronous pneumothorax].

Three hundred and thirty-six cases of spontaneous pneumothorax were treated in our hospital from 1976 to 1988. In these 336 cases, there were 49 cases (14.6%) of bilateral metachronous pneumothorax. Bilateral metachronous pneumothorax happened significantly more frequently in the younger generation. There was no correlation between occurrence rate of contralateral pneumothorax and Brinkman index. In addition, postoperative contralateral recurrence occurred in 18.1% of surgical cases under general anesthesia. Especially nitrous oxide might have influence on postoperative contralateral recurrence.

Bone acidic glycoprotein-75 is a major synthetic product of osteoblastic cells and localized as 75- and/or 50-kDa forms in mineralized phases of bone and growth plate and in serum.

Anti-peptide and anti-protein antisera were produced which both recognize bone acidic glycoprotein-75 (Mr = 75,000) and an apparent fragment or biosynthetic intermediate (Mr = 50,000) in calcified tissues and/or serum. A fragment-precursor relationship is suggested from the fact that closely spaced doublet polypeptides of Mr = 50,000 could be produced by proteolysis of the purified protein upon long term storage. No reactivity was detected with osteopontin, bone sialoprotein, or small bone proteoglycans. Bone acidic glycoprotein-75 represents 0.5-1% of the total radiolabeled proteins synthesized by explant cultures of neonatal calvaria or growth plate, by calvarial outgrowth cultures, and by rat osteosarcoma cells. Amounts produced by explant cultures and calvarial outgrowth cultures were similar to that for osteopontin, a major product of osteoblasts. In osteosarcoma cultures, 80% of labeled antigens were associated with the cell layer fraction wherein specific immunoprecipitation pelleted Mr = 50,000 and 75,000 sized antigens. Bone acidic glycoprotein-75 (Mr = 75,000) is enriched in 4 M guanidine HCl/0.5 EDTA extracts of neonatal rat bone and growth plate tissues, whereas largely absent from heart, lung, spleen, liver, brain, and kidney. Explant cultures of these noncalcifying tissues also synthesized bone acidic glycoprotein-75 antigen, but the quantities produced were only 5% or less that obtained with calvaria. By immunohistochemistry, antigenicity is associated with the bony shaft and calcified cartilage of long bones, but is absent from associated soft tissues. These finding demonstrate that bone acidic glycoprotein-75 is antigenically distinct, predominantly localized to calcified tissues, represents a major product of normal osteoblastic cells and may undergo a characteristic fragmentation in vivo and in vitro.

Mixed-chain phospholipids and interdigitated bilayer systems.

The lipid bilayer is a fundamental structural component for all biological membranes. The function of lipid bilayers may be considered not only to serve as a permeability barrier preventing the free flows of ions and polar molecules between the cell interior and its external environment, but also to modulate the activity of bilayer-spanning proteins or glycoproteins in biological membranes. Based on the location of the terminal methyl groups of the two hydrocarbon chains in phospholipids or glycosphingolipids, two broad categories of lipid bilayers have been recognized: (1) the noninterdigitated and (2) the interdigitated bilayers. Depending on the chain-length difference between the two hydrocarbon chains, three different types of packing models for interdigitated bilayer systems have been identified. Among the three types, the mixed interdigitated bilayer is perhaps unique, since the hydrocarbon chains in the bilayer core are observed to be interdigitated in both the gel and the liquid-crystalline states. In this communication, various experimental data supporting the chain packing characteristics of the mixed interdigitated bilayer for a large number of mixed-chain phospholipid species are considered. In addition, two types of phase diagrams for binary phospholipid mixtures obtained with mixed-chain phospholipids are also presented. These studies may be of great importance in understanding the functional control of bilayer-spanning proteins in biological membranes, and for providing basic information explaining the dynamic regulation of membrane activities in general.

Organization of the auditory area in the posterior cerebellar vermis of the cat.

We mapped the topographic distribution of auditory responses in the posterior cerebellar vermis of the cat under barbiturate anesthesia. Auditory neurons in the granule cell layer of lobules VI and VII appeared to be arranged in columns perpendicular to the surface of the cerebellar cell layers. Mapping the surface of the cerebellum, auditory responses were found as separated patches of the order of a square millimeter. Neurons on these patches responded to auditory stimuli but neurons between patches did not respond to sound. In decerebrated cats, the entire granule cell layer within the cerebellar auditory area responded to acoustic stimulation without a patchy pattern. Responses to tonal stimuli from single neurons in the granule cell layer were studied before and after the induction of barbiturate anesthesia. Some neurons showed no change in their responses to sound before and under barbiturate. But other neurons showed dramatically attenuated responses or essentially stopped responding as a result of barbiturate anesthesia. These results suggest that there may be two types of granule cells distinguishable in their auditory responses and therefore possibly in function.

Bilateral injection of fasciculin into the amygdala of rats: effects on two avoidance tasks, acetylcholinesterase activity, and cholinergic muscarinic receptors.

These experiments examined the effects of the bilateral injection of fasciculin-2 (FAS), a natural acetylcholinesterase (AChE) inhibitory peptide, into the amygdala of rats on acquisition and retention of two avoidance behaviors. Intraamygdala injection of FAS (150 ng/amygdala) produced a pronounced and long-lasting inhibition of AChE activity: 85% and 74% on day 2 and day 5, respectively. After 48 hr, FAS-treated animals showed no changes in training or test session performance in a step-down inhibitory avoidance task (training-test interval was 24 hr). In a 2-way shuttle avoidance task, intraamygdala FAS slightly reduced retention test performance without modifying training session scores. Two and five days after FAS injections into the amygdala, the density of muscarinic receptor decreased about 50% as measured by the specific bindings of 3H-quinuclidinyl benzilate and 3H-oxotremorine. No alterations were observed in the apparent dissociation constants. On the other hand, the central-type benzodiazepine receptor population of the amygdala remained unchanged, suggesting that FAS microinjection did not produce damage to neuronal components of these nuclei. In conclusion, the results presented have indicated that a clear-cut and long-lasting inhibition of AChE activity in the amygdala is not accompanied by a facilitation of learning and memory of two different avoidance tasks. Compensation of the increased cholinergic activity by a down-regulation of muscarinic receptors could account for these findings.

[T-lymphocyte subsets in peripheral blood of patients with bone tumors].

Peripheral blood T-cell subsets of 54 patients with malignant or benign bone and soft tissue tumors and 20 age-and sex-matched normal controls were studied by using monoclonal antibodies. T-cell subsets were also studied before and after operation in ten patients with osteosarcoma. The number of T cells (OKT 11) and OKT 4 cells significantly decreased in malignant bone and soft tissue tumors. The number of OKT 8 cells increased. Statistically significant decrease in the T4/T8 ratio and the number of macrophage were also found in malignant bone and soft tissue tumors. In giant cell tumor of bone, the number of T cells and OKT 4 cells were also statistically decreased. But, the number of OKT 8 cells and macrophage remained unchanged. The ratio of T 4/T 8 was nearly normal. There was no difference in T cell subsets between benign bone tumors and normal controls. In malignant bone tumors, the number of T cells and T 4/T 8 ratio in some degree may be as a predictor for prognosis.
