Study on the genetic determinants responsible for expression and assembly of CS3 fimbriae.

Colonization factor antigens (CFAs) are important virulent factors and protective antigens of enterotoxigenic Escherichia coli (ETEC). Among the known CFAs of human origin, CS3 is a major fimbriae antigen. To study the genes associated with the expression and assembly of CS3 is a key step to delineate the regulating mechanism of expression and assembly of the fimbriae. In this paper we analyzed the restriction map of the CS3 genetic determinant and successfully cloned the CS3 subunit gene and the region encoding the auxiliary proteins. Expression results of the recombinant plasmids in minicells showed that the genetic determinant of CS3 fimbriae encodes six polypeptides with molecular weights 15, 17, 24, 27, 48, and 90 kDa. Western blotting and complementary expression analysis among the different mutants showed that the 15 kDa/17 kDa proteins compose the CS3 subunit and the 17 kDa protein is the precursor. The others may be necessary for the assembly of CS3 fimbriae. The relative locations of various genes of the CS3 genetic determinant were also determined.

[Determination of food oral-colon transit time with breath hydrogen test].

OBJECTIVE: To establish a method to determine food oral-colon transit time (OCTT). METHODS: OCTT was determined for five groups of food rich in carbohydrate in 13 healthy adults with breath hydrogen test (BHT) using lactulose as control. RESULTS: OCTT was 90.0 +/- 50.6 min, 237 +/- 64.9 min, 341.3 +/- 77.9 min and 352.5 +/- 59.5 min, respectively for lactulose syrup, sweet potato, steamed bun and corn flour, and OCTT could not identified for rice and rice with ground meat due to hydrogen production only in few study subjects or no rising in hydrogen value. OCTT inversely correlated with peak hydrogen value (r = -0.6625) and correlated with the time of peak hydrogen value and food absorption rate (r = 0.7668 and 0.8790). CONCLUSION: BHT is a simple, reliable, non-invasive and feasible method to determine food OCTT.

[Studies on incidence of malignant tumor in workers exposed to dust in a mine in 30 years].

OBJECTIVE: To investigate incidence of malignant tumor in workers exposed to dust in a mine during the past 30 years. METHODS: A retrospective cohort study was conducted in 16,711 workers exposed to dust and 7,598 non-exposed workers. RESULTS: Incidence of lung cancer in exposed workers ranked the first place, whth an SMR of 2.648, as compared with that of non-exposed workers. Incidence of lung cancer in the dust-exposed workers with a longer duration of employment was significantly higher than in those with a shorter one. Incidence of lung cancer in exposed workers with a wetoperation mode was lower than that in those with dry-operation mode. CONCLUSION: Malignant tumor, especially lung cancer, occurred more frequently in the workers exposed to dust, which could be a potential risk factor contributing to carcinogenesis.

[Repair of massive bone defect with free vascularized fibular graft].

In the study of repair of massive bone defect with free vascularized fibula graft, 13 cases were reported, in which traumatic defect in 7 cases, segmental resection of bone from tumors in 5 cases and osteomylitis in 1 cases. They all were treated successfully with vascularized fibular graft. After a follow-up of 6 months to 7 year, bone healing was observed with satisfactory and rehabilitation of functions. In one case, fatigued fracture occured twice due to early walking. It was concluded that free vascularized fibular graft was very helpful in the repair of massive bone defect, but prolonged external fixation after operation might be important to prevent fractur of grafted bone.

Study on acupuncture and moxibustion therapy for female urethral syndrome.

Among 180 patients with female urethral syndrome, 128 were treated by acupuncture and moxibustion and 52 by western medicine as controls. The short-term effective rate in the acupuncture and moxibustion group was 90.6% and the long-term effective rate, 80.4%; whereas the short-term effective rate of the control group was 26.9% (P < 0.01). The maximal uroflow rate increased by an average of 4.6 ml/s, after acupuncture and moxibustion treatment (P < 0.001) and the mean uroflow rate increased by an average of 3.1 ml/s (P < 0.001); on the contrary, no changes were found in the control group (P > 0.05). Sixty-nine cases from the acupuncture and moxibustion group and 39 from the control group were subjected before and after treatment to determinations of the maximal bladder pressure, maximal abdominal pressure, bladder-neck pressure, and maximal urethral closure pressure during urination. All these indexes were decreased remarkably in the acupuncture and moxibustion group, while no changes were observed in the control group.

Production of longer acting tissue plasminogen activator of humans in milk of transgenic mice.

An exogenous gene was expressed in the mammary epithelium of transgenic mice in the hope that the encoded protein could be secreted into milk. The transgenic mice carrying the promoter and upstream regulatory sequences from the sheep BLG gene which were fused to cDNA encoding human tissue plasminogen activator (tPA) with its endogenous secretion signal sequence were generated. The hybrid genes were microinjected into mouse embryos. Two mice were identified as being transgenic by Southern blot. Milk obtained from lactating females contained biologically active tPA, and its concentration was calculated to be about 1.5 microg/ml. This result establishes the feasibility of secreting protein into the milk of transgenic animals for production of biologically active tPA proteins, and may provide a powerful method to produce such proteins on a large scale.

[Study on the relative absorption of food carbohydrate in healthy adults].

The absorption of food carbohydrate was determined with breath hydrogen test (BHT) in healthy adults. When the absorption of lactulose was assumed as 0, the absorption of food carbohydrates were as follows: 100% for rice with ground lean pork; 98% for rice; 85% for bread; 58% for corn and 30% for red potato. The absorption of food carbohydrate was positively related to the oral-colon transit time (OCTT, r = 0.8790) and the peak hydrogen time (PHT, r = 0.6745), and was negatively related to the peak hydrogen value (PHV, r = -0.6468) and to the hydrogen producing percentage (H2 > 1.8 mg/m3, r = -0.8679). The breath hydrogen test can be used in estimating the absorption of food carbohydrate in human body because this method is safe, non-invasive, reproducible and accurate.

[Leu 72 Pro mutation in the NADH-cytochrome b5 reductase gene found in a Chinese hereditary methemoglobinemia patient].

OBJECTIVE: To characterize the mutation in NADH-cytochrome b5 reductase gene in a Chinese hereditary methemoglobinemia patient, and elucidate the molecular basis of the disease. METHODS: B5R gene from a propositus was analyzed by sequencing the RT-PCR products as well as cDNA clones; and the results were further confirmed by restriction enzyme analysis of the genomic DNA fragments. RESULTS: A novel mutation was found at codon 72 of b5R gene from the propositus. CONCLUSION: Replacement of Leu with Pro at codon 72 of b5R gene is the molecular basis of the propositus; and the mutant allele located in 5' part of b5R gene mainly cause hereditary methemoglobinemia type I.

[Analysis of 3130 cases of medicolegal postmortem].

3130 cases of medicolegal postmortem in Shantou City between January 1977 and December 1996 were analyzed statistically. The results revealed that the ratio of male to female was 2.3 and most dead were young and middle age. The number of postmortem has increased in resent years. Of 3130 cases, 2829 (92.3%) were caused by mechanical injury, mechanical asphyxia, sudden death and poisoning. As to manners of the death, 1226 were related to accidents (39.2%), 776 homicide(29.8%), 658 suicide (21.0%). These results were compared with other studies reported in domestic and international literature.

[The clinical value of tumor marker CA50 and CA242 in digestive tract cancer].

OBJECTIVE: CA50 and CA242 are newly recognized serum tumor marker. This study is to show their value in the diagnosis of digestive tract tumor. METHODS: 164 patients with malignant digestive disease and 69 patients with benign disease were chosen. CA50 and CA242 level of these patients were measured by immunoradiometric assay. RESULTS: The total sensitivity of CA50 and CA242 are 70% and 63% respectively. Two tumor markers rarely elevated in the patients with benign disease (false positive rate CA50 7.3%, CA242 4.3%). High percentage of elevated CA242 level was recorded in the patients with pancreatic and biliary cancers (Sensitivity 79%). Both have low sensitivity in patients with liver carcinoma. CONCLUSIONS: CA50 and CA242 are valuable marker in the diagnosis of digestive tract tumor.

[The effects of allitridum on the proliferation of rabbit aortic smooth muscle cells in vitro].

The effects of allitridum on the proliferation of cultured rabbit aortic smooth muscle cells (SMC) were studied with SMC incorporation data of 3H-thymidine(3H-TdR) and contents of superoxide dismutase(SOD), lipid peroxide(LPO), prostacyclin(PGI2) and cyclic adenosine monophosphate(cAMP) in medium. The results showed that allitridum could inhibit SMC proliferation by increasing SOD activity, decreasing LPO, raising PGI2 and cAMP(all P < 0.05-0.01), and the inhibitory action was accompanied by the dose-effect relation.

[90Y glass microspheres intraradiopraxis of advanced hepatic carcinoma].

OBJECTIVE: To evaluate intraradiopraxis with (90)Y glass microspheres for advanced hepatic carcinoma. METHOD: 28 patients were divided into two group: A treated by (90)Y and hepatic artery embolism and chemotherapy (HAEC) (n = 13), and B by (90)Y and hepatic artery embolism (HAE) (n = 15), and 15 cases (group C) only treated by HACE simultaneously were used as controls. The average age, course, and diameter of tumor were similar in all groups (P > 0.05). RESULT: The effective rate of the group A and B was 61.5% and 66.6% respectively, and that of the group C was only 33.3% (P < 0.001). The average survival time of the group A and B was 9.86 and 11.9 months respectively, and that of the group C was only 4.5 months (P < 0.001). CONCLUSION: The intraradiopraxis with (90)Y glass microsphere would be a valuable method for patients with hypo-leukocyte count, insensitive to chemotherapy or resistant to chemotherapy.

[The detection of circulating hepatocellular carcinoma cells in peripheral venous blood by reverse transcription-polymerase chain reaction and its clinical significance].

OBJECTIVE: To detect circulating hepatocellular carcinoma by demonstrating hepatocellular carcinoma cells-associated mRNA in the nuclear cell component of peripheral blood (PBL). METHOD: Peripheral blood (5 ml) samples were obtained from 93 patients with hepatocellular carcinoma (HCC) and from 37 controls (15 controls with liver cirrhosis after hepatitis B, 12 chronic hepatitis B, and 10 normal liver function). To identify HCC cells in peripheral blood, liver-specific human alpha-fetoprotein (AFP) mRNA was amplified from total RNA extracted from whole blood by reverse transcription-polymerase chain reaction. RESULT: AFP mRNA was detected in 50 blood samples from the patients with HCC (53.8%). In contrast, there were no clinical control patients whose samples showed detectable AFP mRNA in PBL. The presence of AFP mRNA in blood seemed to be correlated with the stage (by TNM classification) of HCC, the serum AFP value, and the presence of intrahepatic metastasis, portal vein thrombosis, tumor diameter and/or distant metastasis. AFP mRNA was detected in the blood of 21 patients showing metastasis at extrahepatic organs (100%) in contrast to 29 of 72 patients without metastasis (40.3%). CONCLUSION: The presence of AFP mRNA in peripheral blood may be an indicator of malignant hepatocytes, which might predict hematogenous spreading metastasis of tumor cells in patients with HCC.

[Research and development of the surface EMG detection and analysis system].

To meet the requirement of developing myoelectric controller for a man-machine systems, a surface electromyography (EMG) detection and analysis system is discussed in this paper. This system consists of two parts--data acquisition hardware and signal processing software. In the surface EMG acquisition system both wire and wireless TELEMG are available. In the surface EMG processing system various methods are integrated such as the methods in time domain, the methods for frequency analysis, and also including some newly developed methods, e.g., wavelet transform and fractal analysis. This system provides a useful platform for research and development of the myoelectric controller that is widely used in rehabilitation engineering products.

[Study on the surface EMG pattern classification with BP neural networks].

This paper presents a surface electromyography (EMG) motion pattern classifier which combines Neural Network (NN) with parametric model such as autoregressive (AR) model. This motion pattern classifier can successfully identify four types of movement of human hand, wrist flexion, wrist extension, forearm pronation and forearm supination, by using of the surface EMG detected from the flexor carpi radialis and the extensor carpi ulnaris. The result shows that it has a great potential application to the control of bionic man-machine systems such as prostheses because of its fast calculating speed, high recognition ability, and good robust.

[Field trials on the efficacy of albendazole composite against intestinal nematodiasis].

AIM: To study the anthelmintic effect of albendazole composite (containing 67 mg of albendazole and 83.3 mg base of pyrantel pamoate per tablet). METHODS: A randomized controlled study was carried out to compare the efficacy of a single dose of 3 or 2 tablets of albendazole composite versus a single dose of 400 mg of albendazole or 10 mg base/kg of pyrantel pamoate for treatment of intestinal nematodiasis including 1,864 cases infected with hookworm, 1,568 cases infected with Ascaris, 1,785 cases infected with Tricuris trichiura and 373 children infected with Enterobius vermicularis. RESULTS: In adults, the egg negative conversion rate of a single dose of 3 or 2 tablets of albendazole composite reached 65.0% and 52.7% for hookworm infection (P < 0.01), 100% and 100% for Ascaris infection, and 26.5% and 19.2% for Trichuris infection (P < 0.01), respectively. There were significantly better effect against hookworm with 3 tablets of albendazole composite than that with single albendazole or pyrantel pamoate (65.0% vs. 47.6% and 38.5%, P < 0.01). The effect of 2 tablets of albendazole composite against hookworm was also higher than that of single pyrantel pamoate (P < 0.01) and equal to single albendazole but the anthelmintic effect against Trichuris infection was lower than that of single albendazole (19.2% vs. 26.5%). In 2-6-year-old children, the effect of 1.5 tablets of albendazole composite against Enterobius vermicularis infection showed an egg negative conversion rate of 100% which was higher than that of single pyrantel pamoate (100% vs. 83.0%, P < 0.01). The worm collection data showed that the worm-expelling action of albendazole composite was much more rapid than that of albendazole. There were no adverse effects of albendazole composite on blood picture, liver or renal function and ECG. The side effect of both 3 and 2 tablets of albendazole composite was mild and transcient. CONCLUSION: Albendazole composite exhibits a synergistic effect of both albendazole and pyrantel pamoate.

[Detection of TT virus DNA and sequence analysis of partial gene of TTV in sera from blood donors].

In order to investigate TTV infection in blood donors, 262 sera samples from blood donors were detected by nested--PCR method using primers from ORF1 of TTV genome DNA. The results showed that 18 of 58(31.0%) serum samples from blood donors with elevated transaminase level were positive for TTV DNA. 30 of 204 (14.7%) sera samples from blood donors with normal transaminase levels were positive for TTV DNA. There were significant difference between them (chi 2 = 38.1, P < 0.05). Five products of PCR for TTV DNA were purified and cloned to T Vector. The sequence analysis showed that there were above 97% nucleotide identity among AB008394 (from Japan), TTVCHNI (from China) and our 5 isolates from blood donors. The results suggest that there exists TTV infection among blood donors in China. TTV may be transmitted by blood transfusion. Our isolates, AB008394 and TTVCHN1 share a same genotype.

[Detection of GBV-C/HGV infection in patients with liver diseases].

The aim of this article is to study the prevalence of GBV-C/HGV in patients with liver diseases and the clinical features in patients with (GBV-C/HGV infection. The serum samples were obtained from 169 patients with liver diseases in You An Hospital). Serum GBV-C/HGV RNA was detected by reverse transcription nested polymerase chain reaction (RT-nPCR) using two primer pairs of 5' untranslated region (5' UTR) of HGV. Serum anti-HGV was detected by ELISA simultaneously. The partial GBV-C/HGV genome isolated from one patient was cloned into T vectors and sequenced by dideoxy-mediated chain termination methods. Among 169 patients with various liver diseases, the GBV-C/HGV RNA positive rate was 9.5% (16/169), including 11.1% (1/9) of AHA, 4.1% (3/73) of CHB, 16.2% (6/37) of CHC, 13.3% (2/15) of AH(nonA-E) 12.5% (1/8) of CH(nonA-E) 15.4% (2/13) of LC and 33.3% (1/3) of SH cases. Of 29 patients having a history of surgical operation and transfusion, 9(31.0%) was positive for GBV-C/HGV RNA, which was remarkably higher than those cases without surgical operation and transfusion. The sequence analysis showed that there was 89.14%-98.91% nucleotide identity between our isolate and published GBV-C/HGV isolates. The results also suggested that infection rate of GBV-C/HGV was around 9.5% in patients from Beijing region, patients with GBV-C/HGV infection might show various clinical features and GBV-C/HGV might not be the major cause of hepatitis nonA-E.
