Rapid healing of MRSA infection at the suprafascial radial donor site.

Methicillin-resistant Staphylococcus aureus (MRSA) infection at the radial suprafascial donor site resulted in significant loss of the skin graft, but no tendon exposure, in two patients. The complication was successfully managed with wound debridement, appropriate antibiotics, a negative-pressure wound dressing and early partial-thickness skin grafting. The suprafascial dissection creates a donor site that resists both skin graft loss and tendon exposure. It also allows early re-grafting with no loss of function in the event of skin graft loss from infection. These advantages have not been described previously.

Why does schizophrenia develop at late adolescence?

Schizophrenia is one of the most researched, yet still one of the least understood, of the mental disorders. One key area that remains comparatively neglected is the fact that schizophrenia typically develops at late adolescence. In common with people with psychotic disorders, around 25% of normal teenagers also report finding adolescence very distressing, and a substantial empirical literature shows that certain characteristics typical of adolescence such as conflicted family relationships, grandiosity, egocentrism, and magical ideation bear a distinct resemblance to phenomena seen in psychotic disorders. Indeed, such phenomena, as might be judged prodromal or symptomatic in first-onset schizophrenia, have been shown to be remarkably common in normal adolescents, generally in about 50% of samples. Furthermore, prodromal-like signs in normal adolescents appear to be functionally linked to psychological development. For most adolescents, such phenomena pass with successful psychological development. It is proposed that psychosis in late adolescence is a consequence of severe disruption in this normally difficult psychological maturational process in vulnerable individuals, and explanations are offered as to why and how this comes about. It is suggested that problems either in reaching psychological maturity with regard to parents or in bonding to peers or both, may lead to crucial self-construction difficulties, and that psychosis emerges out of such "blocked adolescence." This approach proposes therapeutic interventions that enable professional services to side with both parents and clients simultaneously, and is normalizing and stigma-free.

Strategies to encourage mammography use among women in senior citizens' housing facilities.

BACKGROUND: This randomized trial evaluated the impact of mammography-enhancing interventions implemented in 40 senior citizens' housing facilities in Pennsylvania and North Carolina. METHODS: After stratification of the facilities according to the socioeconomic status and racial backgrounds of their residents, they were randomly assigned to one of four groups to receive the following: 1) standard care--a Medicare mammography benefit flier; 2) education--the flier and a community education program; 3) access--the flier, mammography appointments, and transportation; 4) combined--all interventions. Telephone surveys, conducted prior to any intervention, identified mammography-eligible women, who were re-surveyed six months later to ascertain mammography use. RESULTS: Logistic regression analyses identified mammography intention, age, and the interaction between ever having had mammography and being in the combined intervention group to be significant predictors of mammography use at six months. However, no significant main effect was found for any of the interventions. CONCLUSIONS: These results suggest that the combination of community-directed mammography education and access to mammography appointments encourages mammography use primarily by women who are already predisposed to having mammography. However, individually targeted and tailored interventions may be needed to encourage mammography use among women who have never had mammography and/or express no intention of having it done.

A study of Campylobacter enteritis.

During the years 1978 and 1979, 432 separate outbreaks of gastro-enteritis were reported to this department by the employees of this company. All these outbreaks were fully screened, and it was seen that nineteen employees had gastro-enteritis caused by Campylobacter organisms. Investigation of these employees revealed significant findings with regard to the possible human source of the organism, and leads us to conclude that the organism can be spread by human-to-human contact, and must be treated as a food poisoning organism with subsequent exclusion of food handlers from work during the period of carriage of the organism. The study also demonstrated that the illness suffered by the affected employees was severe in nature and lasted approximately 1 to 10 days. It took on average between 17 and 19 days during 1979 for clearance to occur in the specimens of the affected employees. Thus, if food handlers are excluded from work when affected, this is a considerable period of absence from work, with considerable cost to industry and the community.

Study of migraine and the treatment of acute attacks in industry.

A simple study of migraine was undertaken in an industrial setting to identify facts about the condition of migraine which had previously been unrecorded in industry. The study was based on a definition of migraine encompassing headache accompanied by visual or gastro-intestinal disturbances. During the period from April 1979 to December 1979 there were 111 separate incidents reported to this department, with 281 working days lost in this period, which would give an average annual loss of 420 working days through migraine. Approximately 6% of the work-force were seen to be affected by migraine. Using a simple treatment it was possible to return the majority of the work-force to the place of work within 1 hour, with no subsequent problem caused to their health, either through migraine or accident.
