[Congenital lipoid hyperplasia of the adrenal gland and male pseudohermaphroditism: clinical aspects of two siblings].

Two siblings of consanguinous parents, one male and one female, presented with symptoms of adrenal insufficiency related to respiratory infection at the age of two and three months, respectively. Besides a reduction of the synthesis of gluco- and mineralocorticoids, the sexual hormones were found to be reduced as well. Therefore, the boy showed a female sexual phenotype (male pseudohermaphroditism). Additionally, minor malformations including epicanthal folds, anti-mongoloid palpebral fissures, low-set ears were noticed, which have not been reported in children with the suspected diagnosis previously. The female sibling had typical Addison's crisis twice during the following years. Endocrinological tests yielded evidence for Cholesterol-20,22-desmolase deficiency.

A capillary method to measure water transmission through polyurethane membranes.

A capillary method has been developed to measure the rate of water transmission through polyurethane membranes prepared for use as ventricles in artificial hearts. The system consisted primarily of a leak-proof sample chamber containing the water, a glass capillary flow meter, and a receiver compartment with continuous dry air ventilation. The capillary flow meter monitored the volume of water loss in the sample chamber. The rate of water transmission through the test membrane was found to be proportional to the water loss in the sample chamber, and dependent on the membrane thickness. For thicknesses from 0.09 mm to 0.34 mm, water vapor transmission rates ranged from 7.53 x 10(-8) to 2.76 x 10(-8) mol/s cm2, respectively. Although the concentration of water vapor in the receiver compartment did affect the rate of water vapor transmission through the membrane, within the pressure range 50-200 mmHg, there was very little effect. These findings suggest that water transmission through a polyurethane membrane is dominated by a diffusion process rather than by bulk convection.

Resistance and tolerance of the myocardium to ischemia.

Since 1967, the authors have abandoned coronary perfusion in valve, particularly aortic, surgery. Some of the difficulties encountered during defibrillation at the period where coronary perfusion was always used have dramatically decreased. Extra-corporeal-circulation is now performed under moderate hypothermia--28 to 30 degrees C--which gives excellent myocardial protection for aortic cross clamping time of 30 to 60 minutes. In coronary artery surgery, the same technique is now used, after having wrongly believed previously that coronary perfusion was indicated. In conclusion, we feel that coronary perfusion is not indicated any more but in very exceptional cases.

The late post-operative patency rate of aorto-coronary venous grafts.

Long-term aorto-coronary venous graft patency has been examined in a consecutive series of 47 patients (average: 1.6 graft per patient) after a mean follow-up period of 5 years and 9 months (44-108 months) post surgery. Overall patency rate was 71% (52/73 grafts), with the best results for LAD grafts 85% (27/32). Seventy-seven percent of the patients experienced lasting relief from angina. Significant improvement of LV ejection fraction was observed in cases with patent grafts.
