Peripheral leukocytes in psoriasis.

The white blood cell (WBC) count and differential count, including the morphology and acid alpha-naphthyl acetate esterase (ANAE) activity of lymphocytes, in psoriatic patients of different types, stages, and extensiveness of skin lesions were analyzed. The number of total WBC and polymorphonuclear leukocytes was markedly increased in psoriasis erythrodermica and psoriasis pustulosa. Lymphocytopenia, especially T-lymphocytopenia, was noticed in all types of psoriasis. Antineoplastic drugs and immunosuppressants intensified the degree of T-lymphocytopenia. This might be the cause of recurrence and more recalcitrant course of the disease. Therefore, it is considered that these drugs should not be the first choice in the treatment of psoriasis.

The dependency of bioactive follicle-stimulating hormone secretion on gonadotropin-releasing hormone in hypogonadal and cycling women.

An antagonist analog of GnRH, (Ac-delta 3-Pro1,p-F-D-Phe2,D-Trp3,6)GnRH (4F-antagonist), was administered to normal women and women with hypergonadotropic hypogonadism. Serum FSH levels were determined by both the granulosa cell aromatase bioassay and RIA. The constant infusion of 4F-antagonist (30 micrograms/kg.h) to the four hypogonadal women resulted in a more pronounced decline in bioactive FSH (62%) than in immunoreactive FSH levels (30%), and the FSH bioactive to immunoreactive ratio decreased significantly (P less than 0.05). Infusion of 4F-antagonist in normal women in the midfollicular phase revealed a similar pattern of suppression of bioactive (64%) and immunoreactive FSH (29%). When 4F-antagonist was administered sc at a dose of 80 micrograms/kg twice daily for 3 days to normal women in the midfollicular phase of their cycles, the bioactive FSH response was biphasic, with the maximal decrease on the second day, followed by return to basal levels on the third day. Correspondingly, there was a precipitous decline in serum estradiol (apparent demise of the dominant follicle), followed by a progressive rise in estradiol levels. Thus, in contrast to immunoreactive FSH levels, bioactive FSH more clearly reflects the biological action of FSH on the follicle in response to GnRH antagonist administration in women. The disparity in the quantitative decline between serum bioactive and immunoreactive FSH levels after presumed blockade of the GnRH receptor may reflect the microheterogeneity of the FSH molecule and suggests that alterations in the biological activity of secreted FSH may be GnRH dependent.

Dependence of mean levels of circulating luteinizing hormone upon pulsatile amplitude and frequency.

We investigated the relationship between mean serum LH levels and the amplitude and frequency of LH pulses in 23 women. Serum LH levels were determined every 10 min for 8-12 h in 6 normal cycling women during the follicular phase of the menstrual cycle, in 9 women with polycystic ovary syndrome, and in 8 postmenopausal women. A striking linear correlation (r = 0.98; P less than 0.00001) was found between the mean LH value during the study period (range, 11.1-137.7 IU/L) and the product of the mean LH pulse amplitude and frequency. The interdependence of these variables was further explored using a mathematical model which identifies two main determinants of the slope of the regression line relating the mean LH value to the product of mean pulse amplitude and frequency: 1) the set of constants that specify the biexponential disappearance pattern of LH from serum and 2) the fraction of LH associated with a given pulse which is released from the anterior pituitary after the peak peripheral LH level is reached. Our data, viewed in the light of this model, suggest that 1) the rate at which LH is cleared from the peripheral circulation (expressed as percentage removed per unit time) is independent of both LH concentrations and the sex steroid milieu, and 2) most circulating LH is released in discrete pulses and little, if any, is released in a nonpulsatile or "tonic" mode.

[Nail fold microcirculation in patients with chest malignancy].

Nail fold microcirculation was observed in 90 patients suffering from chest malignancies. 33 were patients with esophageal cancer, 41 cancer of gastric cardia, 4 esophageal and cardiac cancer, 11 lung cancer and 1 malignant neurogenic tumor in the posterior mediastinum. The results indicate that, in cancer patients, marked microcirculatory disturbances are present in the form of marked exudation, cloudy visual field, abnormal and dilated capillary loops, granular and slow blood flow. There is no obvious difference in the various kinds of malignancies. Nor is there any difference between the operable and inoperable cases. Indication of operation is not predicted by this method. The probable cause and clinical significance of microcirculatory disturbances are discussed.

Early detection of bacteremia in an outpatient clinic.

A total of 570 patients, aged less than 24 months, who were seen in the emergency room at Texas Children's Hospital for evaluation of acute febrile illness had a WBC count, differential count, and a blood culture obtained. Bacteremia occurred in 7.7% (n = 44) (22 Haemophilus influenzae; 17 Streptococcus pneumoniae; three Escherichia coli; one group B Streptococcus; one Staphylococcus aureus). The sensitivity, specificity, and positive predictive value of morphologic changes (vacuolization and toxic granulation) of polymorphonuclear neutrophils (segmented neutrophils plus band cells), WBC count greater than or equal to 15,000/microL and band cells greater than or equal to 500/microL were examined. Additionally, abnormalities in three of four tests (WBC count greater than or equal to 15,000/microL, band cells greater than or equal to 500/microL, segmented neutrophils greater than or equal to 10,000/microL, and/or total polymorphonuclear neutrophils greater than or equal to 10,500/microL) were evaluated. Morphologic changes of polymorphonuclear neutrophils were noted frequently in patients with culture-proven bacteremia (63% and 51% positive predictive value for vacuolization and toxic granulation, respectively) and were more predictive of bacterial infection than traditional tests. If both vacuolization and toxic granulation were present, the positive predictive value increased to 76%. Examination of the peripheral blood smear may provide important adjunctive information for the presence of bacteremia prior to bacteriologic confirmation.

Morphological and functional comparisons of Peyer's patches in different parts of the swine small intestine.

Fifteen conventional 8-week-old pigs were used to compare the morphology and function of Peyer's patches (PP) in different parts of the small intestine with special emphasis on the dome epithelium (DE). The comparisons were done by morphological observation through light and electron microscopy, and by the ability of the DE complex to phagocytize horseradish peroxidase (HRP). Dome epithelium of the PP in the jejunum was more superficially located in the mucosa in comparison with the ileum. The DE's of the ileum were much smaller, with an area of 3.7 micron2/DE, than that of the jejunum (18.4 micron2/DE). The number of DE areas/5 cm2 in the ileum was more than in the jejunum. However, the total surface area of DE/5 cm2 of PP, was larger in the jejunum (180.5 micron2) than in the ileum (55.6 micron2). Brown discoloration of diaminobenzidine-hydrogenperoxide (DAB+H2O2)-treated PP specimens, after HRP inoculation, intensified with post-inoculation time from 20 s to 5 min. The brown pigment first appeared on the surface of microvilli and infiltrated into the dome. No morphological differences were observed between the jejunum and the ileum in 1 micron thick Epon-embedded specimens. Intramucosally, brown pigment was almost always found in DE areas. The pigmented areas were more numerous in the jejunum but the color intensity showed no obvious difference. By transmission electron microscopy, the electron dense materials (which were interpreted as the products of HRP and DAB+H2O2) were found between the microvilli of membraneous (M) cells, in the intercellular spaces of the DE, and in a form similar to intracytoplasmic vesicles in the cytoplasm of M-cell and DE complex lymphocytes. Our results confirmed that DE of PP had much stronger phagocytic activity than did the ordinary villous epithelium. This evidence indicates that the DE complex of PP in the swine intestine is immunologically important.
