Static pressure-volume curves and effect of positive end-expiratory pressure on gas exchange in adult respiratory distress syndrome.

Fifteen patients with adult respiratory distress syndrome (ARDS) were studied: 11 in the early stage of ARDS (group 1); 4 in the late stage (group 2). The inspiratory and expiratory static pressure-volume (P-V) curves of the respiratory system were compared to the pulmonary shunt (Qsp/Qt) when PEEP was increased; cardiac output was kept constant. In group 1 patients, we found that a concavity on the P-V curves was associated with an abrupt decrease in Qsp/Qt when PEEP was increased; the concavity on the expiratory curve was correlated with the change in Qsp/Qt but not the concavity on the inspiratory curve. In group 2 patients, the P-V curves were found rectilinear and Qsp/Qt was not abruptly decreased when PEEP was increased. Expiratory P-V curve can be used to determine: first, whether a patient should be ventilated with PEEP; second, the PEEP level which can be set on the respirator. In group 1 patients, when PEEP was set to a value corresponding to the inflexion point, i.e., the point of departure from the exponential shape (mean value 14.6 +/- 2.8 cm H2O), Qsp/Qt compared to zero PEEP was abruptly decreased to 87.6 +/- 6%; further increase in PEEP had little advantage.

Activation of virilizing adrenal rest tissues by excessive ACTH production. An unusual presentation of Nelson's syndrome.

Nelson's syndrome occurs in 10 to 35 percent of patients treated for Cushing's disease by bilateral adrenalectomy and features an ACTH-producing pituitary tumor and hyperpigmentation. Cortisol-producing testicular tumors activated by markedly elevated ACTH levels have been described in male patients with Nelson's syndrome. We now describe a female patient with Nelson's syndrome who presented with virilization. Abdominal exploration revealed adrenal rest tumors in the paraovarian tissues and adrenal beds. Iodocholesterol scanning gave negative results. This case illustrates the need for follow-up study of patients with bilateral adrenalectomy for Cushing's disease and describes some of the unusual features these patients may present.

Growth, testicular development and sperm output in the dog from birth to post pubertal period.

This study was performed in Fox terrier dogs. Their mean body weight at birth is 315 g; it reaches 12 kg at 1 year of age. The body growth pattern has a sigmoid appearance with a rapid growth phase between 9 weeks and 24 weeks of age. With frequent radiographic examinations, 39 ossification centers were individualized on the long bones; they had all appeared by 25 weeks of age. The fusion of these centers is achieved between 36 and 44 weeks of age. This determination of the bone age has not proved to be a good factor to date the morphologic events of puberty. The testicular width measure is a reliable criteria which is well correlated to the other morphologic and spermatic criteria (volume of ejaculate, spermatozoa concentration). The testicular growth is rapid (width: 0,6 mm/week) between 22 and 36 weeks of age. The first spermatozoa are observed at the end of this period (32 to 34 weeks of age). During the post pubertal period, from 34 weeks to 45 weeks of age, volume of ejaculate, motility, spermatozoa concentration increase and percentage of dead spermatozoa and abnormal forms decrease.

[Pharmacist drug dispensing in clinical trials].

Dispensing experimental drugs in part of a pharmacist's task in the hospital since the 20.12.1988 law. A 13 month critical evaluation dispensing drugs for clinical trials is presented. Drug management, distribution and control in 61 new studies included 1,165 patients need a full time pharmacist. This new organization is well accepted by sponsors and investigators. The hospital pharmacist's contributions to clinical trials are: protocol application in hospital practice, nurses information, efficient and safe storage and distribution to patients. The pharmacist facilitates good clinical practices and quality assurance. The problem is that in many french hospitals the pharmaceutical department is chiefly a wholesale dealer for pharmaceutical products. Our opinion is that the pharmacists should deliver drugs to improve patient compliance.
