Reconstruction of human skin from glycerol-preserved allodermis and cultured keratinocyte sheets.

The aim of this project was to reconstruct human skin from glycerol-preserved dermis and layers of cultured keratinocytes for use in the treatment of deep burns and ulcers. Glycerol-preserved cadaver skin from the Euro Skin Bank was treated with Dispase II or PBS, under various conditions, to find the best method of retaining the physical structure of the dermis while removing the epidermis and remnants of dead dermal cells which might provoke an allogeneic reaction in a graft recipient. Monoclonal antibodies LH39 and LH7.2, with specificity for basement membrane determinants, showed that treatment with Dispase II resulted in separation of the epidermis from the dermis with concomitant loss of all cellular elements from the dermal layer (as judged by H and E staining). However, when sheets of cultured keratinocytes were applied to the treated dermis and cultured for several days, the keratinocytes attached and regenerated a new basement membrane.

Comparative effects of calcipotriol (MC903) solution and placebo (vehicle of MC903) in the treatment of psoriasis of the scalp.

The efficacy and safety of calcipotriol solution in the treatment of scalp psoriasis was compared with placebo (vehicle solution), in a multicentre double-blind, randomized, parallel-group study of 49 adult patients. Calcipotriol solution (50 micrograms/ml), or placebo, was applied twice daily over a 4-week period. At the end of the study period 60% of patients on calcipotriol showed clearance or marked improvement of their psoriasis compared with 17% on placebo. Overall assessment of treatment response showed that calcipotriol was superior to placebo in both investigator (P < 0.001; 95% confidence interval for difference 19.0-67.6) and patient (P < 0.001; 95% confidence interval for difference 18.3-68.0) assessments. Total sign score for psoriasis (i.e. the sum of the scores for redness, thickness and scaliness) decreased by 48.9% in the calcipotriol group, and by 18.6% in the placebo group (P = 0.005). Calcipotriol was significantly superior to placebo in reducing redness, thickness, scaliness and extent of psoriasis, and in the patients' assessment in reducing scalp flaking and itching. No statistically significant changes in blood biochemistry were detected during the study, and the solution was generally well tolerated.

Sesquiterpene lactone mix is not an adequate screen for Compositae allergy.

70 patients suspected or known to have contact allergy to Compositae were patch tested with 0.1% sesquiterpene lactone mix (SL mix) and individual Compositae oleoresins. There were no apparent cases of active sensitization by, or irritancy from, the SL mix. 6 patients had allergic reactions to both SL mix and Compositae oleoresins, while 11 patients had allergic reactions to Compositae oleoresins with a negative reaction to the SL mix. In all but 2 of these, Compositae allergy was considered clinically relevant. As 0.1% SL mix picked up only 35% of cases of Compositae allergy, we do not consider it an adequate screen.

Mg+ MNS blood group phenotype: further observations.

Red cells carrying the low-frequency MNS antigen Mg reacted with the only example of anti-DANE, an antibody which had previously defined the GP. Dane (Mi.IX) phenotype. Furthermore, Mg+ cells reacted with the original anti-Mur (serum of Mrs. Murrell), but with none of 14 other anti-Mur. Therefore, Mg+ cells carry both DANE antigen and an atypical Mur antigen. Immunoblotting of membranes from Mg+ cells with anti-M, and with eluates prepared from anti-Mg and Mrs. Murrell's serum demonstrated a glycophorin A (GPA) molecule whose mobility was increased by an apparent M(r) of about 3,000 presumably due to the loss of the three O-glycans known to be absent from Mg-active GPA.

Spatial correlates of firing patterns of single cells in the subiculum of the freely moving rat.

Hippocampal lesions cause spatial learning deficits, and single hippocampal cells show location-specific firing patterns, known as place fields. This suggests the hippocampus plays a critical role in navigation by providing an ongoing indication of the animal's momentary spatial location. One question that has received little attention is how this locational signal is used by downstream brain regions to orchestrate actual navigational behavior. As a first step, we have examined the spatial firing correlates of cells in the dorsal subiculum as rats navigate in an open-field, pellet-searching task. The subiculum is one of the few major output zones for the hippocampus, and it, in turn, projects to numerous other brain areas, each thought to be involved in various learning and memory functions. Most subicular cells showed a robust locational signal. The patterns observed were different from those in the hippocampus, however, in that cells tended to fire throughout much of the environment, but showed graded, location-related rate modulation, such that there were some localized regions of high firing and other regions with relatively low firing. There were slight quantitative differences between the proximal (adjacent to the hippocampus) and distal (farther from the hippocampus) subicular regions, with distal cells showing slightly higher average firing rates, spatial signaling, and firing field size. This was of interest since these two regions have different efferent connections. Examination of spike trains allowed classification of cells into bursting, nonbursting, and theta (putative interneuron) categories, and this is similar to subicular cell types identified in vitro. Interestingly, the bursting and nonbursting types did not differ detectably in spatial firing properties, suggesting that differences in intrinsic membrane properties do not necessitate differences in coding of environmental inputs. The results suggest that the subiculum transmits a robust, highly distributed spatial signal to each of its projection areas, and that this signal is transmitted in both a bursting and nonbursting mode.

The incidence and size of gap junctions between the bone cells in rat calvaria.

Polyclonal antisera to synthetic peptides matching sequences on the cytoplasmic regions of connexin-43, a gap junction protein first identified in rat heart, have been used to immunolabel gap junctions in the calvarial bone, maintained intact as in vivo, of 1- to 2-week-old rats. The specimens were examined in reflection and fluorescence modes by scanning laser confocal microscopy, and the numbers of gap junctions and their sizes estimated. The mean number of connexin-43 immunolabelled junctions per osteoblast (n = 65) was 15.3 (SD +/- 4.5). The mean length of 227 junctions, selected for the sharpness of the image of the fluorescent spot, was 0.67 micron (SD +/- 0.18; range 0.37-1.29 microns) and their mean area 0.26 micron2 (SD +/- 0.145; range 0.075-0.93 micron2); these probably fell within the upper half of the total size range. Gap junctions were detected between preosteoblasts, osteoblasts, osteocytes and chondrocytes, and between these juxtaposed cell types. In addition, connexin-43 immunolabelled junctions were found between some osteoclasts and overlying mononuclear cells at active sites of resorption.

A longitudinal study of the relationships between psychometric test scores, offence history and the plasma concentrations of phenylacetic and 5-hydroxyindoleacetic acids in seven inmates of a prison for the psychiatrically disturbed.

1. The plasma concentrations of phenylacetic (PAA) and 5-hydroxyindoleacetic (5HIAA) acids in seven inmates incarcerated in the Regional Psychiatric Centre (Prairies), Correctional Service of Canada, were assessed each weekday for four weeks (i.e., 20 samples each). Psychometric assessments for hostility, anger, depression and anxiety were also performed daily. Mean differences between subjects in psychometric and biochemical measures were subjected to tests of statistical significance. 2. The subject who was clearly most aggressive by offence history/institutional behavior scored significantly highest on scales of anger and hostility and significantly lowest with respect to PAA concentration. It was concluded that PAA may be a trait marker for aggression. 3. Plasma 5HIAA concentrations were invariant between subjects. 4. The psychometric measures were intercorrelated, thus confounding the variables of interest. They also varied little, proving insensitive to subtle mood changes.

Use of a bilingual medical history questionnaire in the emergency department.

STUDY OBJECTIVE: To demonstrate that a complete and rapid clinical history can be obtained using a self-administered, bilingual questionnaire. DESIGN: A post-test experimental design was used to evaluate time spent obtaining the history and information completeness for the two groups. SETTING: Urban county emergency department. PARTICIPANTS: All patients were non-English-speaking Hispanic women who presented with gynecologic complaints. METHODS: Fifty-five patients completed a history questionnaire in Spanish on a form that displayed their responses in English on the carbon copy. In the control group, a history was obtained using a bilingual interpreter. RESULTS: Average completion time was significantly less (t = 14.38; P < .0001) for the experimental group (chi 2 = 5.67 minutes; SD, 1.89) than for controls (chi 2 = 14.58 minutes; SD, 4.19). There was no statistical difference in review criteria for documentation between groups. CONCLUSION: Using a bilingual questionnaire to obtain a clinical history results in more rapid patient evaluation without affecting the quality of information obtained.

Ictal SPECT in a 16-day-old infant.

Neonatal seizures can be difficult to classify according to partial versus generalized onset on the basis of clinical appearance or electroencephalography (EEG). Single-photon emission computed tomography has proven to be useful in adults when adjunctive tests are needed to identify the nature of seizure onset. Although its use has been extended recently to children, the lower age limit at which this technique is useful remains to be established. A case is reported in which ictal Tc-99m HMPAO SPECT of a 16-day-old infant revealed an area of focal hypermetabolism in the right temporal lobe corresponding to an area of focal atrophy revealed by MRI. The EEG of this infant demonstrated multifocal interictal epileptiform abnormalities and an ictal pattern with a generalized onset. This case indicates that ictal SPECT is a useful tool in the evaluation of even the youngest patients with seizures.

A trial of accelerated PUVA in psoriasis.

Ten psoriatic patients were treated with an accelerated PUVA regimen. Nine patients cleared in an average of 13.7 treatments (range 4-24) after an average total UVA dose of 83 J/cm2 (range 18-186). One patient had to withdraw due to pruritus and burning and six others had burning problems. The high incidence of burning may limit the routine use of this irradiation regimen.
