Endothelial lesions associated with vascular clamping--surface micropathology by scanning electron microscopy.

The endothelial cell pavement in normal blood vessels is very fragile. In cardiovascular surgery, the need for secure clamping of the aorta results in trauma which may be followed by thrombosis or occlusion. Various types of vascular clamps and other related devices offered commercially were clinically investigated in canine models and their effects on the vascular endothelium were ascertained with the aid of scanning electron microscopy. On the basis of these studies, the Fogarty silicone filled clamp appeared to be the least traumatic. However, further technical developments in this area are needed in order to obtain a completely atraumatic method of temporarily closing major blood vessels.

Albuminated dacron protheses as improved blood vessel substitutes.

An improved vascular graft was achieved by treating knitted Dacron prostheses with cross-linked albumin. Eight segments, each six centimeters in length, were coated and implanted in the canine thoracic aorta for periods ranging from 24 hours to six months. The results were compared with the eight untreated grafts implanted for similar periods as control experiments. Albumin coating resulted in a faster healing : cells appeared earlier and collagen development was greater. The most beneficial effect of that treatment was the reduction in embolization, as shown by the considerable decrease in kidney damage.

[Utility of soft hydrogel contact lenses in case of epithelial defects after corneal grafts (19) (author's transl)].

The authors relate the utility of soft hydrogel contact lenses in 19 cases of epithelial defects after penetrating corneal grafts, 8 of them being primitive and 11 secondary. Out of the 19 cases fitted with lenses, 10 have required a new keratoplasty. An attempt is made to define the indications, the advantages and the drawbacks of this therapy in observing the action of the lens on the epithelium, the stroma, the visual acuity as well as the possible problems due to the wearing of the lenses in the whole of the 19 cases. They try to understand the reasons for a new graft and they study the non-regrafted cases. They evidence the benefits of the wear of soft hydrogel lenses but the limits of their uses too.

Stored blood microfiltration. Evaluation of micro-aggregate filter composed of polyurethane foam and nylon wool.

Aggregates of amorphous material which develop during storage of banked blood have been implicated as a cause of pulmonary micro-embolism in man following massive transfusion. Such pulmonary micro-embolism may be a causal factor in the development of post traumatic pulmonary insufficiency. At present several microaggregate filters for use in massive transfusion are commercially available, and on of these is the Fenwal filter. It is composed of a screen filter which removes microclots of 250 microns and higher, a layer of polyurethane foam and a layer of nylon wool. The resistance of this device is very acceptable and the filter may be used for several blood units, but its efficiency seems less than that of Dacron wool filter.

Dacron as arterial prosthetic material: nature, properties, brands, fate and perspectives.

Dacron graft prostheses are routinely implanted to overcome impairment of arterial flow to distal tissues. Many different fabric configurations have been developped and evaluated. Unfortunately, after implantation the flow surface of any graft persists as a fibrinous lining. No human specimen has a commonly ingrown extension of tissue exceeding some millimeters in length. Full wall healing and endothelialization of arterial prosthesis is a goal that must be pursued intensively because it seems that the goal is attainable.

Implantable artificial lung. Preliminary report.

The ultimate treatment of chronic respiratory insufficiency is pulmonary replacement by an artificial organ, homologous lung transplantation, or chronic paracorporeal respiratory supplementation. The woven capillary membrane oxygenator appears to be a major development toward implantable artificial organs. The four units tested are made up of screens 3.5 by 4.0 cm. of capillary tubing 0.3 mm. I.D. by 0.64 mm. O.D. assembled into rectangular blocks. Units made up by five, ten, twenty, and forty screens have been assembled and tested according to the protocol suggested by Galletti. The maximum oxygen transfer rate with blood was 48 ml. per minute per square meter. Water carbon dioxide transfer rate was 23.1 ml. per minute per square meter. The pressure drops in the liquid phase were 8.5, 15.3, 13.8, 17.6 mm. Hg at 1 L. per minute flow. These results indicate that the woven capillary membrane lung is an acceptably efficient oxygenator. The characteristics of design and performance suggest that this oxygenator can be made to be implanted into the chest or used as a paracorporeal respiratory assistance device.

Preclotting and thrombosis of a dacron prosthesis. A scanning electron microscope study.

The authors report eht study of preclotting made on a knitted Dacron prostheses before implantation into a patient. Within 24 h of implantation, clotting occurred. The patient had to be reoperated, and a second preclotting followed. Specimens were removed at different times and fixed in a Rembaum solution before being dried and studied by scanning electron microscopy.

Preclotting of knitted Dacron prosthesis. A scanning electron microscope study.

A scanning electron microscope study of preclotting on knitted Dacron prosthesis is reported. Five steps of the interaction are well identified: (1) before any blood contact (virgin Dacron), (2) during the first 3 minutes (fibrin and platelet aggregates), (3) fifth minute of contact (clotting), (4) 15 minutes of contact with heparinized blood (thin fibrin network), and (5) the following minutes (invasion of fibrin, which enmeshes blood cells).

Polyester prostheses as substitutes in the thoracic aorta of dogs. II. Evaluation of albuminated polyester grafts stored in ethanol.

In an attempt to find an alternative procedure to the preclotting of porous textile vascular prostheses, the feasibility of an albumin coating and ethanol preservation technique has been evaluated by implanting albuminated polyester prostheses as substitutes in the thoracic aorta of dogs. Nine different grafts representing woven, knitted, and velour constructions were implanted for periods ranging from 4 h to 6 months. At the sacrifice, the grafts were excized for measurement of the thrombogenicity of the flow surface, for pathological examination by light microscopy and SEM, and for quantifying the changes in the textile structure during implantation. Also the kidneys were removed and examined for infarcts caused by trapped circulating emboli. The healing characteristics of the nine different grafts proved similar and followed the same sequence of events as preclotted control grafts. The albumin coating and ethanol preservation resulted in a somewhat slower rate of healing. Yet, given sufficient time, a more completely healed graft was achieved as evidenced by the presence of endothelial-like cells throughout the length of the graft. In addition, the albumin is believed to reinforce the textile structure by reducing the tendency to stretch and dilate in vivo.

In vivo tissue distribution of fibroblast growth factor-1 after intraarterial delivery.

The clinical applicability of fibroblast growth factor-1 (FGF-1) plus heparin delivery in optimizing the healing of both autogenous vein and synthetic vascular grafts has been suggested. The authors have reported enhanced endothelial cell proliferation, concurrent increased capillarization, and minimal intimal hyperplasia using suspensions of FGF-1 and heparin impregnated onto expanded polytetrafluoroethylene grafts. The current study characterizes the tissue distribution of 125I-FGF-1 delivered by continuous intraarterial infusion. 125I-FGF-1 delivered by continuous intraarterial infusion. 125I-FGF-1 (1.1 ng) and heparin (28 U) were continuously infused into the thoracic aorta via the proximal end of the ligated left carotid artery for 24 hr in four New Zealand white rabbits using an Alzet (Alza Corp., Palo Alto, CA) osmotically activated pumping device. Rabbits were sacrificed after 24 hr, exsanguinated, and biopsies taken from the liver, kidneys, spleen, lungs, heart, thyroid gland, muscle, and fat. These samples were assayed for radioactivity and results expressed as cpm 125I/gram of both wet and dry weight of tissue. 125I-FGF-1 uptake (cpm/g dry wt.) was greatest in the thyroid (551.1 +/- 131.4). This was 2.5-5.5 x greater (p < or = 0.01) than those organs with intermediate uptake (lungs, liver, kidneys, spleen, and heart). Lowest uptake was noted in the blood, muscle, and fat. A similar distribution pattern was found in wet weight comparisons. Total organ 125I-FGF-1 content was greatest in the liver at 818.1 +/- 176.3 cpm (p < or = 0.002) and intermediate in the lungs (204.7 +/- 38.5 cpm) and kidneys (191.2 +/- 11.9 cpm). Although no FGF-1-induced toxicity has yet been reported, these results will allow for future tissue-specific toxicology studies before clinical trials.

Developmental profile of Down's syndrome infants receiving early intervention.

Thirty-two infants with Down's syndrome, who were enrolled in an early intervention programme were followed during the first 2 years of life. Progress in five developmental domains was monitored prospectively by evaluating the children at 6, 12, 18 and 24 months. The largest degree of retardation was consistently exhibited in the hearing and speech subscale. The locomotor subscale experienced the most severe decline over the 24 months and was the second most retarded domain at 2 years of age. The remaining three subscales, personal-social, hand-eye and performance, demonstrated less severe declines over time as well as actual increases in developmental quotients at specified points in time. The overall performance of female infants at 18 months was significantly better than that of male infants (P = 0.05). The degree of retardation of Down's syndrome infants receiving early intervention services differs according to chronological age, developmental domain and sex.
