[Fat embolism after total hip prosthesis replacement preserving the femoral stem].

Fat embolism is a known complication of traumatology, especially in long bone fractures. It may also occur in liposuction and articular surgery (0.1%). Fat embolic events are most often clinically insignificant and difficult to recognize since clinical manifestations are varied and there is no routine laboratory or radiographic diagnosis. Classically, fat embolism syndrome presents with the triad of pulmonary distress, mental status changes, and cutaneous manifestations. We report the case of a 33-year-old woman who developed acute respiratory distress 10 days after hip arthroplasty. Several aetiologies such as fibrinocruoric pulmonary embolism, pulmonary aspiration and bacterial pneumonia were discussed. Fat embolism was diagnosed, based on suggestive clinical manifestations, radiographic and laboratory findings, although fat embolism after hip arthroplasty without intramedullary pressurization is infrequent.

Randomized placebo-controlled trial of metronidazole 1% cream with sunscreen SPF 15 in treatment of rosacea.

BACKGROUND: Rosacea is a photoaggravated dermatosis responsive to treatment with topical and oral antibiotics. A formulation combining metronidazole 1% cream with sunscreen SPF 15 was developed for the treatment of rosacea. OBJECTIVE: The objective of this study was to determine the safety and efficacy of a formulation combining metronidazole 1% cream with sunscreen SPF 15 in the treatment of moderate to severe rosacea. METHODS: One hundred and twenty patients with moderate to severe rosacea were enrolled for a randomized, placebo-controlled (vehicle containing sunscreen with SPF 15), double-blind study. Study cream was applied twice daily to the entire face over a 12-week period. RESULTS: Treatment with metronidazole 1% cream with sunscreen SPF 15 resulted in significant improvement (p <0.05) in inflammatory lesion count, erythema and telangiectasiae scores, and investigator and patient global assessment scores compared with baseline and placebo. Adverse reactions related to study medication were typically mild, occurred at the site of application, and were reversible. There was no difference between the safety profiles of metronidazole 1% cream with sunscreen SPF 15 and placebo. CONCLUSIONS: The combined topical formulation of metronidazole 1% cream with sunscreen SPF 15 was an effective, well-tolerated topical agent for the treatment of moderate to severe rosacea.

Imaging the local density of states of optical corrals.

This paper reports the experimental observation, at optical frequencies, of the electromagnetic local density of states established by nanostructures corresponding to the recently introduced concept of optical corral [G. Colas des Francs et al., Phys. Rev. Lett. 86, 4950 (2001)]. The images obtained by a scanning near-field optical microscope under specific operational conditions are found in agreement with the theoretical maps of the optical local density of states. A clear functionality of detection by the scanning near-field optical microscope is thereby identified since the theoretical maps are computed without including any specific tip model.

Addressing and imaging high optical index dielectric ridges in the optical near field.

Experimental observation of light coupling between TiO(2) integrated waveguides of subwavelength cross section and pure three-dimensional evanescent light fields is reported. This near-field optical phenomenon is produced by controlling the location of the focusing of a laser beam totally reflected at the surface of the sample. The phenomenon is observed in direct space with a photon scanning tunneling microscope. Dielectric ridges several tens of micrometers long have been efficiently excited with this technique. Upon excitation, the extremities of the linear dielectric wires display intense light spots localized both inside and around the ridge. For ridge lengths up to 30 microm, the observed phenomenon has been reproduced numerically with a parallel implementation based on the three-dimensional Green dyadic method.

Failure of dogs to shed oocysts after being fed bovine fetuses naturally infected by Neospora caninum.

Neospora caninum is a protozoan that causes abortion in cattle. The dog has recently been identified as a definitive host for N. caninum. To verify if bovine fetuses can infect dogs, nine 2-4-month-old dogs were fed bovine fetuses naturally infected by N. caninum. None of the dogs excreted oocysts, seroconverted, had clinical signs or lesions compatible with N. caninum infection. Additional studies will be necessary to determine the natural mode of infection of dogs by N. caninum.

Optical analogy to electronic quantum corrals.

We describe full multiple-scattering calculations of localized surface photonic states set up by lithographically designed nanostructures made of a finite number of dielectric pads deposited on a planar surface. The method is based on a numerical solution of the dyadic Dyson's equation. When the pads are arranged to form a closed circle, we find field patterns that look like the electronic charge density recently observed above quantum corrals. We propose two experimental techniques that could be used to observe these electromagnetic modes in direct space.

Genomic and functional characteristics of novel human pancreatic 2P domain K(+) channels.

We isolated three novel 2P domain K(+) channel subunits from human. The first two subunits, TALK-1 and TALK-2, are distantly related to TASK-2. Their genes form a tight cluster of 25 kb on chromosome 6p21.1-p21.2. The corresponding channels produce quasi-instantaneous and non-inactivating currents that are activated at alkaline pHs. These currents are sensitive to Ba(2+), quinine, quinidine, chloroform, halothane, and isoflurane but are not affected by TEA, 4-AP, Cs(+), arachidonic acid, hypertonic solutions, agents activating protein kinases C and A, changes of internal Ca(2+) concentrations, and by activation of G(i) and G(q) proteins. TALK-1 is exclusively expressed in the pancreas. TALK-2 is mainly expressed in the pancreas, but is also expressed at a lower level in liver, placenta, heart, and lung. We also cloned a third subunit, named hTHIK-2 which is present in many tissues with high levels again in the pancreas but which could not be functionally expressed.

Early degradation of type IX and type II collagen with the onset of experimental inflammatory arthritis.

OBJECTIVE: To determine whether following the onset of intraarticular inflammation, there is early damage to articular cartilage, specifically to types II and IX collagen, and the proteoglycan (PG) aggrecan, and whether measurement of the degradation products of these molecules in synovial fluid (SF) and serum may permit the detection of cartilage damage. METHODS: A rabbit model of rheumatoid arthritis, antigen (ovalbumin)-induced arthritis, was studied. Articular cartilage samples were analyzed by immunoassays for total type II collagen content, its denaturation and cleavage by collagenases, and for type IX collagen content. PG content was determined by colorimetric assay. In serum and SF, total PG content and collagenase-generated peptides of type II collagen were measured. RESULTS: After 6 days, both the PG content and the NC4 domain of type IX collagen were reduced in femoral and tibial cartilage, concomitant with the onset of arthritis. In only the tibial cartilage did this reduction in PG persist up to day 20. However, denatured type II collagen was increased in all cartilage samples, but only on day 20. In SF, the PG content was significantly reduced on day 20, and products of type II collagen cleavage by collagenase were significantly increased on both day 6 and day 20. CONCLUSION: This study, which is the first of its kind examining changes in both types II and IX collagen and PG content, reveals early damage to both types of collagen as well as to PG in articular cartilage samples following induction of joint inflammation. SF analyses reveal this early damage and may be of value in the study and treatment of inflammatory arthritic diseases such as rheumatoid arthritis.

[Anesthetic malignant hyperthermia and multiple organ dysfunction syndrome].

The typical explosive form of malignant hyperthermia caused by following isoflurane anaesthesia is a well-known phenomenon. Nevertheless, since dantrolene is used, its evolution toward a multiple organ failure has been rarely described. We report a case of typical explosive malignant hyperthermia caused by an isoflurane anaesthesia complicated by a cardiovascular failure, a disseminated intravascular coagulation, an acute liver failure and an acute renal failure. Afterwards, muscle weakness of the right calf was the only aftermath.

Near-field optical probing of two-dimensional photonic crystals: theory and experiment.

We report new experimental results, and their theoretical analysis, on the mechanisms that control light transfer between two integrated waveguides connected by two-dimensional matrices of dielectric pillars. The optical properties of the system are analysed from the well-established formalism of classical field susceptibilities (Green dyadic functions). We apply this scheme to investigate the optical properties of two-dimensional arrays connected to two integrated waveguides. Comparisons with current experimental work based on near-field optical probing are provided together with a spectral analysis of the phenomenon.

Direct interpretation of near-field optical images.

The interpretation of the detection process in near-field optical microscopy is reviewed on the basis of a discussion about the possibility of establishing direct comparisons between experimental images and the solutions of Maxwell equations or the electromagnetic local density of states. On the basis of simple physical arguments, it is expected that the solutions of Maxwell equations should agree with images obtained by collecting mode near-field microscopes, while the electromagnetic local density of states should be considered to provide a practical interpretation of illumination mode near-field microscopes. We review collecting mode near-field microscope images where the conditions to obtain good agreement with the solutions of Maxwell equations have indeed been identified. In this context of collecting mode near-field microscopes, a fundamentally different functionality between dielectric and gold-coated tips has been clearly identified experimentally by checking against the solutions of Maxwell equations. It turns out that dielectric tips detect a signal proportional to the optical electric field intensity, whereas gold-coated tips detect a signal proportional to the optical magnetic field intensity. The possible implications of this surprising phenomenon are discussed.

Near-field optical addressing of single molecules in coplanar geometry: a theoretical study.

Photonic transfer through elongated optical structures of submicrometre section microfabricated at the surface of dielectric or semiconductor samples can be enhanced by an appropriate structuring of the local refraction index. We show from computerized simulations that both the light localization and the spectroscopic properties of such structures can be used to selectively excite, in coplanar geometry, individuals molecules located in the near-field.

A bioassay using the human hepatoblastoma cell line HepG2 for detecting phototoxicity of furocoumarins.

We successfully evaluated the human hepatoblastoma cell line HepG2 as a model to assess phototoxicity of coumarins. Five natural furocoumarins were tested and their phototoxic activities, obtained by measuring cell viability in the presence of UV using the MTT test, were as follows: xanthotoxin (8-MOP) >> heraclenol = trichoclin = imperatorin >> peucedanin, both in growing and confluent cell cultures. This easy-to-perform, miniaturised, quantitative and sensitive method could therefore be used as a primary screening test for phototoxicity of a large number of compounds and plant extracts.

Safety of oral nicorandil before coronary artery bypass graft surgery.

Nicorandil is a K(ATP) channel opener used to treat angina. It is cardioprotective and a vasodilator. We conducted a prospective, randomized, double-blind, placebo-controlled study to assess oral nicorandil in patients undergoing coronary artery bypass grafting (CABG) with cardiopulmonary bypass (CPB). Twenty-two patients received nicorandil (10 mg twice a day) and 23 patients received placebo. Haemodynamic data were recorded before induction of anaesthesia (T0), 5 and 20 min after starting mechanical ventilation (T1, T2), before aortic cannulation (T3), after 30 min of CPB (T4), 10 min after CPB (T5) and after 3, 8 and 18 h in the intensive care unit (T6, T7, T8). Serum proteins (creatine kinase metabolite and cardiac troponin I) were measured before and 8 and 18 h after surgery. Haemodynamic values did not differ between the two groups. There was no tachycardia during the study, no significant difference in hypotensive episodes, ST segment changes and no changes in cardiac enzymes. Myocardial infarction after surgery was similar in the two groups. Vasoactive therapy was similar in the two groups. Nicorandil can be continued safely up to premedication without deleterious haemodynamic consequences, but a myocardial protective effect of nicorandil in CABG surgery was not found.

Rare detection of Neospora caninum in placentas from seropositive dams giving birth to full-term calves.

Neospora caninum is thought to be transmitted to cattle by dogs, the only known definitive host. Although aborted fetuses seem the most likely source of infective material for dogs, placentas from seropositive dams appear also as a potential source of infective material. The objective of the study was to evaluate the presence of N. caninum organisms in placentas of full-term calves born to seropositive cows. Sixteen placentas, 11 from Neospora-seropositive cows, were examined histologically and by immunohistochemistry and polymerase chain reaction (PCR) assay for the presence of N. caninum. Mild placentitis was observed in all placentas. Neospora caninum was not identified by immunohistochemistry, but placentas from 2 seropositive dams were positive for N. caninum by PCR. These results suggest that placentas of full-term calves from seropositive cows may be a potential source of N. caninum for dogs, but the incidence of this mode of transmission is likely to be low.

The androgen-conjugating uridine diphosphoglucuronosyltransferase-2B enzymes are differentially expressed temporally and spatially in the monkey follicle throughout the menstrual cycle.

UDP-glucuronosyltransferase (UGT) enzymes enhance the polarity of steroid hormones by catalyzing their conjugation with the sugar group from UDP-glucuronic acid. Previous results have shown that the monkey is a suitable animal model to study steroid glucuronidation in steroid target tissues. In humans, as in the monkey, the main androgen metabolites found in the circulation are 5alpha-androstane-3alpha,17beta-diol-glucuronide and androsterone glucuronide, and high levels of androsterone glucuronide were also measured in human follicular fluid. Ovarian androgens play a significant role as precursors for estrogens and may modulate the recruitment and growth of follicles. To analyze the expression pattern of UGT2B enzymes involved in androgen metabolism throughout the menstrual cycle, cynomolgus monkey ovaries were collected during the mid and late follicular and luteal phases. Microsomal proteins and total RNA were analyzed for UGT2B expression in the whole ovary. Western blot and specific RT-PCR analyses demonstrated no significant changes in the expression of UGT2B protein or transcripts during the menstrual cycle. Immunocytochemistry analysis showed that UGT2B proteins are expressed in the cytoplasm of thecal and granulosa cells of growing follicles. Interestingly, the thecal cells of secondary follicles and of corpus luteum were extensively stained, whereas luteal granulosa cells were not labeled. These results suggest an important regulation of cell type-specific UGT2B expression during follicular development. Previous results demonstrated similar changes in the expression of the androgen receptor. The colocalization of the androgen receptor and UGT2B enzymes in the same cell types of the ovary provide evidence for a potential role of glucuronidation as a modulator of the intracellular androgen response during follicular development.

Characterization of the inflammatory infiltrate in canine chronic hepatitis.

Canine chronic hepatitis (CCH) is a progressive inflammatory disease of unknown etiology. To characterize the inflammatory infiltrate, 16 dogs with CCH were selected and classified into three groups based on the stage of fibrosis, as evaluated with Masson's trichrome stain. The inflammatory infiltrate in each liver section was immunohistochemically characterized and evaluated using CD3, lysozyme, lamba and kappa light chain, and alpha-smooth muscle actin antibodies. Numerous breeds were affected, and middle-aged females predominated in this select group. Necroinflammatory activity progressively increased and then waned as the hepatitis progressed to cirrhosis. CD3+ lymphocytes were the most numerous lymphoid cells in dogs with CCH. Degenerate hepatocytes were occasionally surrounded by CD3+ lymphocytes. Necrosis was positively correlated with the number of CD3+ lymphocytes. The lamba and kappa light chain-positive cell infiltrate was variable but generally mild. A positive correlation between the lambda and kappa light chain-positive cells and the portal alpha-smooth muscle actin was found. The number of alpha-smooth muscle actin-positive cells (myofibroblasts) in portal triads and fibrous septa was positively correlated with the stage of fibrosis. In contrast, no correlation was found between the number of lysozyme-positive cells (Kupffer cells) and the stage of fibrosis. These results further support the idea of an immune-mediated process in CCH and suggest that periductular myofibroblasts play an important role in canine liver fibrogenesis.

[Cardiogenic shock complicating extensive infarction with ventricular septal defect. Circulatory assistance and heart transplantation].

A 47 year old man had a massive anterior myocardial infarction with cardiogenic shock with a left parasternal murmur. Coronary angiography showed occlusion of the left anterior descending artery for which angioplasty resulted in failure. There was antero-lateral-apical akinesia and a ventricular septal defect (VSD) with a left-right shunt (Qp/Qs = 1.54). Persistence and aggravation of haemodynamic instability led to intra-aortic balloon pumping with inotropic pharmacological support followed by biventricular assistance with a MEDOS device. Under transoesophageal echocardiographic monitoring, the outcome was marked over 7 days by the progressive increase in the shunt volume of the VSD, a decrease of drainage and injection flow, progressive increase in spontaneous contrast echos followed by the presence of fibrin in the cardiac chambers and canulae, the presence of thrombus in the external ventricles, blockage of the right external valve which only opened after increasing the degree of anticoagulation, and, finally, cardiac tamponade which required drainage before the patient's state improved. On the 8th day, the patient being stable with a normal neurological status, the availability of a donor heart led to the decision to transplant, which was carried out without complications. This case poses the problem of cardiac assist devices and their daily monitoring, and then that of cardiac transplantation in this indication.
