Fifteen patients (with dysglobulinemia, ranging in age from 35 to 52 years, were studied over a period of 6 years. The following parameters of immune response and immunoelectrophoresis of serum proteins were checked: total hemolytic complement (CH50), serum cryoglobulins, presence of circulating immune complexes (CIC), beta 2-microglobulins, rheumatoid factor (RF). Of the 15 patients investigated, 6 (40%) developed during the period of study severe forms of rheumatoid arthritis (RA) which required the introduction of continuous cortisone therapy. In a control group, ranging between the same age limits but without immune disturbances, no case of RA was observed. It should be noted that polyclonal hypergammaglobulinemia preceded by at least 5 years the appearance of articular inflammation. The changes of serum complement were also significant. CH50 was found constantly decreased below 40 HU/ml, particularly in the patients with febrile forms of RA; positive latex fixation test greater than 1/80 was observed at least 6 months before the appearance of articular tumefactions. Increase of beta 2-microglobulins in the serum and the presence of cryoglobulins was also observed in the 4 patients with RA. Dysglobulinemia has often been observed in the course of RA but it has seldom been presented as a preceding disturbance predicting the disease. The originality of the observations reported in this paper consists in a case of malignant RA appeared in the course of dysglobulinemia.

Forty patients with rheumatoid arthritis (RA) selected according to the ARA criteria, in whom the onset of disease had appeared after the age of 65 years, were studied comparatively with a control group of 40 RA patients in whom the disease had occurred before this age. Every patient was subjected to a complex clinical and biologic investigation. The characteristics of the two groups are presented. The onset of disease in old age was frequently acute (65% of the cases as compared with 45% in the control group). The difference between the two groups was statistically significant from this viewpoint (p less than 0.001). Presence at onset of certain general manifestations associated with loss of weight, risomelic involvement and association of Horton disease were found in 50% of the old agers as compared with 12.5% in the control group. The radiologic aspect was similar in both groups. Positive rheumatoid factor in 64% of the cases, as well as the homogeneous or speckled antinuclear factor were likewise comparable with the data in the younger group. From the evolutive point of view RA with onset in old age showed good response to therapy. Chrysotherapy was effective in 80% of the patients in the old age group and in less than 50% of the controls.

The study of the copper, zinc, iron and magnesium serum values in 35 patients with rheumatoid arthritis (RA) stage I-II, before and after therapy with indocin, gold salts and transfer factor showed a significant increase of copper to 129-340 gamma/100 ml, mean value 214.6 gamma/100 ml as compared with 65-145 gamma/100 ml, mean value 107.6 gamma/100 ml in the controls. The values of serum copper were found to evaluate with the severity of diseases returning to normal values in the patients with good response to therapy. In disagreement with some authors who found decreases of serum zinc in the course of rheumatoid arthritis, in our study the values obtained did not vary significantly as compared to normal (mean value 119 gamma/100 ml in RA as compared with 94.7 gamma/100 ml in the controls). The decrease of iron and magnesium serum concentration was also found nonsignificant in the patients with RA investigated. The results are discussed in terms of physiopathology.
A transient pseudoatrial dissociation (Deitz-Marques phenomenon) occurring immediately after right atrial pacing in a 44-year-old woman, and spontaneously together with periods of hiccups in a 50-year-old man are presented. The same phenomenon was produced experimentally in three dogs, after rapid diaphragmatic stimulation. The diaphragmatic origin of fibrillary-like waves on standard ECG is argued because of dependence on respiratory phases and because it does not influence the basic sinus rhythm.
From among the 2813 E. coli strains isolated from various specimens (urine, feces, pharyngeal exudates, organs from fowls) in the course of two years, the authors selected and carried out a complex study on the strains exhibiting atypical features, considered to give rise to errors of diagnosis. Of the 2813 strains studied, 599 (21.29%) presented different atypical traits : 45 (1.59%) morphological and cultural (very small pulvurelent colonies or extremely mucous, oily colonies), 21 (0.74%) morphological, cultural and enzymatic atypical aspects and 533 (18.96%) only enzymatic (non-indolegenous strains, strains giving a methyl red reaction, H2S positive strains, lactose-negative and adonite or inosite-positive strains).
The efficiency of immunotherapy with autovaccines was determined in 162 patients with urinary infections produced by Gramnegative bacteria. In each case quantitative urocultures were performed, with examination of the urinary sediment, and blood samples taken before and after treatment with autovaccines in order to determine the agglutination and hemagglutination titer with autovaccines in order to determine the agglutination and hemagglutination titer with regard to the incriminated germs. The latter were identified by their enzymatic and serologic characters and their sensitivity to chemotherapeutical agents. Autovaccines were prepared (2, 3 or even 4 series), repeating the tests after administration of the vaccines. Following immunotherapy, 87 patients (53.7%) presented repeated negative urocultures, 11 presented alternate positive and negative cultures, and 64 remained positive in spite of the treatment applied. There was an increase in hemagglutination titers in the patients with negative urocultures following autovaccination. To conclude it is assumed that the efficiency of immunotherapy with autovaccines depends upon their administration at the onset of infection after the patient has benefitted by the chemotherapeutical treatment. In the cases in which germs belonging to different taxonomic groups are repeatedly isolated in urocultures, it is recommended to prepare a polyvalent autovaccine to be applied in 2-3 series.
