Aplasia cutis congenita in surviving co-twins: four unrelated cases.

Four unrelated patients born from twin pregnancies and showing extracranial aplasia cutis congenita are reported. All the patients lost their co-twins during the first half of the pregnancy. Two of the patients had the characteristic truncal and symmetrical type of aplasia cutis associated with fetus papyraceus and placental abnormalities. The presence of multiple hepatic hematomas in one of them gives further credit to a "vascular disruption" as the possible pathogenetic mechanism of the disorder. The two other patients were born with symmetrical aplasia cutis at the extensory aspects of both knees, which presumably represents a milder expression of the former defect. Thus the so-called aplasia cutis with fetus papyraceus shows heterogeneity in localization, extension, presence of extracutaneous abnormalities, and possible association with fetus papyraceus at birth. To include all the patients affected by this peculiar type of aplasia cutis congenita, the designation of aplasia cutis with extracranial symmetrical involvement is proposed.

[Cleansing of the skin in childhood].

The author describes the problems of skin cleansing in pediatric age on the basis of the recent knowledge of cosmetology and skin physiopathology. Principal detergents now available are summarized and their characteristics and indications are given. Skin cleansing in children is also seen in correlation with age and peculiarities of the skin itself. Modalities of bath, main types of bath (antiseptics, colloidal, emollient) and the substances used to date are discussed critically. The features which should characterize an ideal pediatric detergent are finally proposed.

[Small skin surgery in children: skin analgesia with a cream made of lidocaine and prilocaine].

The problems arising daily in the management of many skin diseases as the removal of warts, molluscum contagiosum, etc. are often complicated simply by the fear of the pain of some treatment modalities (i.e., cryosurgery, curettage, etc.). A choice between no analgesia and analgesia by the classic percutaneous infiltration with local anaesthetics is sometimes difficult. A new lidocaine/prilocaine cream provides us an effective local anaesthesia for such treatments.

[Cutis marmorata telangiectatica congenita. Description of a further case].

A new case of Cutis marmorata telangiectatica congenita is reported. This disease is characterized by a persistent vascular mottling of the skin generally on the limbs. In our case the lesions were present on almost the entire body even if one side was more affected, this one was also lipotropic. The face of the child was asymmetric because of the jaw's hypoplasia and nose curvature. The examination of the deep vessel did not show any abnormality but the possibility of future vascular defects as varicosities must be considered. The nosological entity of this disease is discussed.
