[The value of the measurement of intra-erythrocytic lithium in the bio-clinical surveillance of lithium therapy (author's transl)].

THe correlation between red cell lithium concentration and signs of neurotoxicity have been reported in the literature by a great majority of authors. A deficiency in the lithium-sodium counter-transport mechanism may be responsible. Measurement of blood lithium levels only is not always sufficient to identify in the laboratory patients showing signs of intolerance. The three cases reported here indicate the limitations of the measurement of plasma lithium levels and the perfect correlation between neurological signs and erythrocyte levels. In vitro studies of transmembrane ion exchanges should make it possible to undertake an investigation before starting treatment in order to identify the risks of cellular toxicity.

Post-transfusion purpura as a gynecologic complication.

Post-transfusion purpura (PTP) is a recently separated category of thrombocytopenic purpura occurring mainly in women. It is an acute, severe thrombocytopenic state with clinical manifestations of hemorrhage that may be fatal. It usually occurs 5 to 8 days after transfusion, usually after administration of whole blood. The typical patient is a middle-aged, multiparous white woman who has received a transfusion and has undergone an operation, often a gynecologic procedure. Diagnosis may be suspected by normal clotting studies, bone marrow biopsy showing increased megakaryocytes, and demonstration of a potent antibody reactive against platelets by clot-retraction inhibition, complement fixation, or 51Cr-release studies. The treatment of choice is exchange transfusion or plasmapheresis, unless these procedures are medically contraindicated. Because of the lack of reports in the gynecology literature on PTP, a case report and discussion are presented.

Frozen thin-sections of rapidly forming bone: bone cell ultrastructure.

Frozen thin-sections of fresh medullary bone, which had been calcifying for 2 to 4 days, were prepared and examined in the electron microscope. The bone was obtained from male Japanese quail treated with estradiol valerate. Large numbers of 200-800 A electron dense granules, which consisted of 50-75 A subparticles, were seen within mitochondria. A population of electron-dense particles slightly smaller than ribosomes was also observed. Most granules were not present after conventional fixation, dehydration, embedding, sectioning and staining. Oother structures which were visible, also on the basis of their intrinsic electron density, were nuclei with regions which resembled condensed chromatin, ribosomes and rough endoplasmic reticulum. By the use of osmium tetroxide vapor staining, inner and outer membranes and cristae of mitochondria were delineated. Other membranous components were not detected. Since very little loss or dislocation of cell components can occur during the preparation of frozen thin-sections, the micrographs obtained may be a more accurate representation of cell ultrastructure.

[Acute postpartum anaemia. Clinical practice and interest of intravenous iron].

OBJECTIVES: Postpartum anaemia is a very frequent pathology concerning from 4% to 27% of patients. The purpose of this study was to estimate clinical practice in front of acute postpartum anaemia and to value a new treatment: intravenous iron. PATIENTS AND METHODS: Retrospective study over a period of 2 years (April 2001-March 2003) realised in the Department of Gynaecology and Obstetrics of Belfort Regional Hospital. Two hundred and seventeen patients were included (5% of deliveries in the same period) with immediate postpartum period haemoglobin <8 g/dl. Two groups were individualised according to the availability or not of an intravenous iron therapy. Clinical and biologic elements concerning deliveries were analysed. RESULTS: Average haemoglobin values, from delivery to 48 h after it, were 5.81 g/dl for blood-transfused patients, 6.88 g/dl for intravenous iron treated patients and 7.43 g/dl for oral iron treated patients. Fifteen patients were transfused during the year before the launch of intravenous iron as a possible therapy and only five patients the next year. The benefit of haemoglobin values with an intravenous iron therapy was 1.9 g/dl on 7 days and 3.1 g/dl on 14 days. DISCUSSION AND CONCLUSION: These results suggest a real efficiency of intravenous iron therapy for acute postpartum anaemia (haemoglobin values <8 g/dl) with a good tolerance. Patients with haemoglobin values <7 g/dl treated by intravenous iron tend to show that some blood transfusions would be thereby avoided even though blood transfusion remains the urgency treatment.

Changes in antimicrobial resistance among Salmonella enterica Serovar typhimurium isolates from humans and cattle in the Northwestern United States, 1982-1997.

We compared antimicrobial resistance patterns of Salmonella enterica serovar Typhimurium (ST) of isolates from humans (n = 715) and cattle (n = 378) in the Pacific Northwest from 1982 through 1997. The major changes in antimicrobial resistance can be attributed to the widespread clonal dissemination of multidrug-resistant definitive phage type 104 ST.

[N-acetylaspartic aciduria. Clinical, biological and physiopathological study].

Two cases of N-acetylaspartic aciduria in siblings are described and compared to the 18 cases already reported. The disease should be considered in childhood when a syndrome of severe encephalopathy with macrocephaly, blindness caused by optic atrophy and diffuse leucodystrophy on CT scan occurs. Urinary organic acids gas chromatography confirms the diagnosis. It is probably inherited as an autosomal recessive trait. Aspartoacylase activity deficiency has been reported and this assay could possibly be used for prenatal diagnosis. Pathogenesis is not clearly understood but N-acetylaspartic acid (NAA) seems to be essential for central nervous system myelination. Clinical and anatomic features of N-acetylaspartic aciduria are very similar to Van Bogaert-Bertrand disease (cerebral spongy degeneration or Canavan disease) but heterogeneity of this disease cannot excluded.
