Twenty-four patients aged from 3 to 30 years and suffering from advanced cystic fibrosis received several courses of intensive antibiotic therapy combined with prolonged respiratory physiotherapy with postural drainage. Antibiotics included carbenicillin 500 mg/kg/day and aminoglycosides in high dosage (6 mg/kg/day). These treatments were administered to prevent aggravation of chronic bronchitis and to suppress acute exacerbations. Tolerance was satisfactory, and all patients felt better, at least during the first courses. Improvement was also observed in the main respiratory function parameters. In a total of 95 courses only two side-effects (severe dizziness and giant urticaria) were recorded. Intensive courses of combined antibiotic therapy associated with physiotherapy constitute a new way of treating severe, progressive cystic fibrosis. The patients are made comfortable, and their survival time is increased by several years.
The breathing pattern was investigated in 62 children aged 4 to 16 years together with the mouth pressure generated 0.1 s after an occlusion at the endexpiratory level (P0.1). A group of 20 adults (18 to 32 years old) were tested in a similar way. The values of P0.1 obtained during room air breathing at rest in the children decreased with age as a power function P0.1 = 8.51 A-0.62 (where A is age in years), r = -0.74. Adult values of P0.1 were achieved at about 13 years. After O2 inhalation P0.1 decreased in children, but the changes in P0.1 were independent of age. Respiratory frequency decreased with age while inspiratory time (TI) increased, both reaching adult values at about 13 years. The ratio of TI over the total respiratory cycle duration did not change significantly with age in the entire population. The absolute value of tidal volume (VT) increased with age, but not when normalized for body weight (BW). The mean inspiratory flow (VT/TI) normalized for BW decreased with age in children as did P0.1. There was a significant relationship between P0.1 and VT/TI normalized for body weight (r = 0.63).
The apnea following lung inflation (Hering-Breuer expiratory promoting reflex) is a vagally mediated reflex which is initiated by the activation of pulmonary stretch receptors (PSR) and terminated by the interaction of several factors, which include adaptation of PSR, chemical stimuli, level of anaesthesia, and body temperature. Since PSR activity is determined by the changes in airway tension, the interpretation of the strength of vagal reflexes on the basis of changes in lung volume rather than transpulmonary pressure can be misleading when the mechanical properties of the respiratory system are not constant. In this study we compared the reflex apnea resulting from lung inflation of young and adult mammals, the respiratory system it can be considered weaker or stronger in the young depending upon the normalizing parameter used. However, when considered on the basis of the relative changes in transpulmonary pressure, which better reflects the activation of PSR, the reflex is weaker in young rats and rabbits than in their adult counterparts an similar in dogs. The analysis of the underlying mechanisms suggests a weaker vagal contribution in the young animal, but a satisfactory conclusion requires a better knowledge of the factors which, in the younger animals, result in the termination of the apnea.
The respiratory function of 19 children aged from 2 months to 9 years old with a cardiac lesion giving a left-to-right shunt was studied and analysed with respect to the hemodynamic disturbance. Respiratory function tests (RFTs) included measurement of functional residual capacity (FRC), the partial pressure in oxygen (Pa 02) on arterialised blood taken from the ear lobe, dynamic pulmonary compliance (CI dyn.) and total pulmonary resistance (RI). FRC and RI were not significantly modified. On the other hand, CI. dyn. was low (-31.2 +/- 22.9%, p less than 0,001) as were the CI. dyn. CFR ratio (-25.5 +/- 20.9%, p less than 0.001) and the Pa 02 (-7.6 +/- 12.7% p less than 0.02). There was a significant correlation between the reduction of CI. dyn. and mean pulmonary arterial pressure (PAP) and the increase in the pulmonary blood flow. In addition, a significant correlation was also found between the reduction in Pa 02 and the PAP and increase in pulmonary blood flow. These results show that cardiac lesions with left-to-right shunts give rise to diffuse changes in respiratory function and are discussed in the light of the abnormalities in pulmonary development previously described in pulmonary biopsy of patients with left-to-right shunts. The value of RFTs in the surveillance of patients with left-to-right shunts is underlined.

Lung function was studied in 29 children suffering from recurrent bronchitis (average number of bronchitis attacks per year: 4.2 +/- 0.5). Most of them (73%) were 1--7 years old. The date of exploration in relation to the last acute bronchitis was on average 6 weeks. Lung mechanics, residual functional capacity, blood gases and regional lung function were measured. Most of the children were suffering from some kind of functional disorder; alteration of lung mechanics -- increase of lung resistance (22/29) decrease of dynamic compliance (15/23); increase of functional residual capacity (6/10), non specific bronchial hypersensitivity (12/17); regional ventilatory and perfusion malfunction. These functional disorders could be the first evidence of the bronchial lesions which cause chronic obstructive bronchitis in adults.

The authors describe 7 cases of idiopathic pulmonary hemosiderosis occurring in children, and emphasize the diagnostic value of the association of pulmonary and anemic manifestations, one of which could appear to be an isolated finding and thus delay diagnosis. Complementary investigations are discussed and the relative inocuity of a lung biopsy with a Gerbeaux needle, and the greater value of bronchoscopic samples over those obtained by gastric intubation for evaluating the presence of siderophagic activity emphasized. Functional pulmonary tests, especially for blood gases and ventilatory function, were also conducted. The difficulties in classifying the various types of pulmonary hemosiderosis, especially the "idiopathic" form in children, are outlined. Immunological disorders must be systematically searched for in order to choose the most effective therapy: corticoids or immunodepressants.

Four children aged between 7 and 19 months with severe bronchopneumonia due to adenovirus type 7, proved by virology and/or serology developed severe hyponatraemia. One of them is reported in detail: it was possible to estimate plasma ADH levels and thereby prove the existence of reversible hypersecretion of the hormone. Whilst the syndrome of hyponatraemia with inappropriate secretion of ADH has not yet been reported in association with severe pneumonia in the child, it is known in adults. The limits of the syndrome and its physiopathology are discussed. It may be due either to vagal stimulation as a result of a fall in left aressure, or to central involvement. Therapeutic implications of the problem are emphasized.
In two groups of healthy children synchronized with a diurnal activity (light-on at 07.00) and a nocturnal rest(light-off at 21.00), lug resistance (R1) and dynamic lung compliance (C1 dyn) were measured at fixed clock hours (07.30, 11.30, 16.30, 22.30). The measurements were performed before and 10 minutes after the inhalation of bronchodilators (a beta-sympathetic stimulating agent (orciprenaline, dose 2 mg) and a vagolytic agent (SCH 1000, doses 80 mug and 200 mug). A circadian rhythm is detected for R1 and C1 dyn and quantified by the cosinor method. R1 and C1 dyn acrophase (peak time in the 24-h scale) are not significantly different for both groups: they are located for R1 at 05.39 and 03.31, for C1 dyn at 09.31 and 12.51. R1 and C1 dyn circadian rhythms are not detectable both after 2 mg orciprenaline and 200 mug SCH 1000 inhalation. This chronopharmacologic effect of the bronchodilators might be considered indirect evidence of a circadian rhythm in the bronchial tone. This latter may be related to circadian changes in neurovegetative activity. A dose-effect difference of the vagolytic agent on R1 at night seems to indicate a relative nocturnal prominence of the vagal tone.
In one year eighteen children were admitted to an intensive care unit with severe viral pneumonia. Four groups were identified by the appearance of the chest X-ray. Bronchiolitis was seen in eight patients, alveolitis in four, interstitial pneumonia in two and combined bronchiolitis and alveolitis in four. Three of the four children with alveolitis died and six of the twelve with bronchiolitis, alone or with alveolitis, had residual bronchial obstruction.
