[Implantation of ossicles (A histological study in animals) (author's transl)].

Autogenetic and allogenetic incuses as well as xenogenetic malleus were implanted into the middle-ear of rabbits after different procedures of conservation. The process of healing was studied by histological examination. The different conservation methods are leading to different antigenic qualities of the donor material. A different reorganization along the wurse of the donor vessels is resulting. Accordingly a different extent of osteoneogenesis of the intercellular substance is induced. The vascular tissue showed a revascularization in the case of autogenetic transplantation and in the case of implantation after cialit conservation of 1:5000. Whereas deep-freezing led to swelling, cialit conservation 1:500 to temporary fibrosis of the sides of vessels. Hyalinosis of the vessels was observed after lyophilization and gamma-sterilization of the xenogenetic malleus. The different effects on the healing process are discussed.

[Long term experience of using preserved cartilage in reconstructive surgery of the nose (author's transl)].

Long term experience of nasal surgery using cartilage homografts preserved in merthiolate shows the implants to have the following properties: 1. They heal without complications and are well tolerated. 2. Rate of resorption varies according to the quality of the recipient site and the stresses on it from outside. 3. Very suitable for support to replace resected or destroyed cartilage. 4. Limited use in reconstructing weight bearing areas because of unduly high resorption rate in 50% of implanted struts. 5. No clinical evidence of resorption in areas not subject to stress. 6. Resorption in areas of stress varies from a virtual absence to a rapid process.

[A simplified method of the endolaryngeal laterofixation of the vocal cord (author's transl)].

In a modification of Thornell's procedure of surgical treatment of bilateral abductor paralysis of the vocal cords, the arytenoid cartilage is preserved, but only its vocal process is removed. After preparing of a pedicled subglottic mucosaflap a partial resection of the vocal cord and of the vocal muscle is followed. An endolaryngeal packing with a soft foam-rubber stent allows an optimal adaptation of the mucosaflap. Two years follow-up studies have shown that all 14 patients had a satisfying respiration, their voice was either partially hoarse or almost as good as before the operation. All 14 cases could be decannulated, 12 of them within 12 days.

[The histological changes in cartilage (incus) in the middle ear after conservation--an animal study (author's transl)].

This study demonstrates the histological changes of incus cartilage after transplantation into the middle ear space of rabbits. Conservation was performed by deep-freezing, lyophilisation in connection with gamma-irradiation, or with Cialit in differing dilutions. Controlled deep-freezing at about -196 degrees C and uncontrolled deep-freezing at about -20 degrees C guarantee after transplantion the survival of some vital chondrocytes. The other methods of conservation cause death of all chondroxytes. Cartilage prepared by these methods demonstrates potential for new bone formation. Only the xenogenetic cartilage loses its osteogenic potential after treatment by lyophilisation in connection with gamma-irradiation. The histological value of the transplant differs according to the method of conservation. No special tendency for resorption or inflamatory reactions could be seen.

[The histological reaction of cialit preserved hyaline and elastic cartilage implanted to replace partially bony wall of the guinea-pig bulla (author's transl)].

Cialit preserved cartilage was used to fill defects of the bulla of porpoises and the histological healing was studied. Hyaline and elastic cartilage are showing an integration without reactions. The cartilage is resorbed consecutively by the recipient and substituted by bone. Whereas the hyaline cartilage shows a complete bony substitute and a small enlargement we can registrate that the elastic cartilage often shows a resorptive tendency with replacement of connective tissue and loss of the shape. The histological results are corresponding to the clinical results when using preserved cartilage in middle ear surgery for restoring the posterior wall.

Comparison of the TEMPO+ ear-level speech processor and the cis pro+ body-worn processor in adult MED-EL cochlear implant users.

A study was conducted to compare the new MED-EL TEMPO+ ear-level speech processor with the CIS PRO+ body-worn processor in the COMBI 40/COMBI 40+ implant system. Speech tests were performed in 46 experienced subjects in two test sessions approximately 4 weeks apart. Subjects were switched over from the CIS PRO+ to the TEMPO+ in the first session and used only the TEMPO+ in the time between the two sessions. Speech tests included monosyllabic word tests and sentence tests via the telephone. An adaptive noise method was used to adjust each subject's scores to approximately 50%. Additionally, subjects had to complete a questionnaire based on their 4 weeks of experience with the TEMPO+. The speech test results showed a statistically significant improvement in the monosyllabic word scores with the TEMPO+. In addition, in the second session, subjects showed a significant improvement when using the telephone with the TEMPO+, indicating some learning in this task. In the questionnaire, the vast majority of subjects found that the TEMPO+ allows equal or better speech understanding and rated the sound quality of the TEMPO+ higher. All these objective and subjective results indicate the superiority of the TEMPO+ and are mainly attributed to a new coding strategy called CIS+ and its implementation in the TEMPO+. In other words, based on the results of this study, it appears that after switching over from the CIS PRO+ to the TEMPO+, subjects are able to maintain or even improve their own speech understanding capability.

Rhinoorbital mucormycosis: surgical aspects.

The authors present a case of a young woman with a diabetic ketoacidosis and rhinoorbital mucormycosis, successfully treated by resection of necrotic areas without orbital exenteration. The favorable outcome was attributed to early diagnosis and immediate commencement of the adequate therapy, which included correction of diabetic ketoacidosis, debridement of devitalized tissue, daily irrigation of the involved areas with amphotericin B solution and application of intravenous amphotericin B.
