Topical disodium cromoglycate in the management of pyoderma gangrenosum.

A 52-year-old woman who had had ulcerative colitis for more than ten years experienced a third relapse of pyoderma gangrenosum of her lower extremities. She was treated with topical disodium cromoglycate. The dosage of salicylazosulfapyridine and steroids she was taking, and by which the ulcerative colitis, but not the pyoderma gangrenosum could be controlled, was not increased. Because pyoderma gangrenosum is often very difficult to treat and may require an aggressive approach using drugs that can cause serious side effects, we believe that a topical therapy is worth trying.

The role of concurrent determinations of pleural fluid and tissue carcinoembryonic antigen in the distinction of malignant mesothelioma from metastatic pleural malignancies.

A combined determination of pleural fluid and tissue carcinoembryonic antigen (PF-CEA and T-CEA) by radioimmunoassay and immunoperoxidase staining technique respectively was performed in patients with malignant mesotheliomas (12), metastatic pleural carcinomas (17) and benign pleural diseases (seven). All PF-CEA-positive (greater than 39 ng/ml) cases were T-CEA-positive metastatic carcinomas. In contrast, 4/30 PF-CEA-negative (less than 39 ng/ml) cases were T-CEA-positive metastatic carcinomas (three cases) and idiopathic pleuritis (one case). These results suggest that CEA, though present in the tumour, is not always released in measurable amounts in effusions. Hence T-CEA content should be determined in the PF-CEA-negative cases when an early and definite diagnosis of tumour type is required to enable correct management of these patients. These ancillary tests aim at enhancing the level of confidence of the routine morphological diagnosis of serous surface malignancies in living patients using minimal intervention instead of resorting to open chest surgery.

The role of transesophageal echocardiography in the diagnosis of ischemic stroke in the elderly.

Cardioembolic stroke is quite common (15% of all ischemic strokes) not only in younger patients but also in the elderly. Clinical diagnosis is often difficult. Transthoracic echocardiography (TTE) seems to be the most reliable non-invasive method of examination. Because of the close topographical relationship between heart and esophagus, transesophageal echocardiography (TEE) is particularly suitable to evaluate those cardiac structures (left atrium and appendage) where the embolus can most likely be found. Using TTE and TEE, we studied 62 patients older than 65 years of age (mean age 76 +/- 6), having been affected by ischemic stroke. TEE proved to be clearly superior to TTE in the diagnosis of cardioembolic stroke, without any major complication during the execution of this diagnostic method.

Immunohistochemical and ultrastructural investigation of neural differentiation in Ewing sarcoma/PNET of bone and soft tissues.

The authors evaluated the role of immunohistochemistry and electron microscopy in defining neural differentiation in 28 cases of Ewing sarcoma/PNET. The panel of primary antibodies used included vimentin, MIC-2, NSE, S-100 protein, leu7, neurofilaments, GFAP, and chromogranin A. Cases were considered undifferentiated when neural markers were absent, poorly differentiated if one neural marker was present, and well differentiated if two or more markers were observed. Cases were also evaluated for the presence of cytoplasmic processes, microtubules, and neurosecretory granules as ultrastructural features of neural differentiation: the tumor was classified as well differentiated if two of these features were present; and poorly differentiated if one was evident; all other cases were considered undifferentiated. According to immunohistochemistry, 10 cases (35.7%) were undifferentiated, 12 cases (42.9%) were poorly differentiated, and 6 (21.4%) were well differentiated. According to the ultrastructural analysis, 10 tumors were undifferentiated (35.7%), 14 poorly differentiated (50%), and 4 well differentiated (14.3%). The overall concordance between the two techniques was low (35.7%), and both modalities were concordant in classifying only 1 well-differentiated, 5 poorly differentiated, and 4 undifferentiated tumors. In conclusion, the authors suggest that investigations devoted to test the prognostic significance of neural differentiation in these neoplasms should employ both immunohistochemistry and electron microscopy, separately and in combination, to assess what is the most effective choice for predicting the clinical course.

"Pure" meningeal carcinomatosis as the autopsy-proven sole manifestation of an undetected cancer.

Meningeal carcinomatosis (MC) was diagnosed in a 37-year-old woman, based on the finding of neoplastic cells in the cerebrospinal fluid. The patient had no relevant antecedent except the resection of a breast "nodule" at the age of 20. No primary cancer nor other metastases were detected, even at autopsy. Histopathology, immunocytochemistry and electron microscopy confirmed pure leptomeningeal infiltration by poorly differentiated adenoepithelial cancer. This case includes several unusual features and raises the possibility of an extremely long-lasting interval between an unrecognized primary (breast?) carcinoma and MC.

Anatomopathological study of dementia syndrome linked with an abnormal cerebrospinal fluid flow. Report of literature and personal observations.

The authors have analysed the data of the literature to identify the cases of normotensive hydrocephalus that underwent surgery and then died after the operation; some of these patients died over varying periods of time after the operation and the death was due to accidents. It seems that the anatomopathological lesions are less important in cases that benefited from the operation compared with cases that did not present perceptable clinical variations. The authors report the anatomopathological data of four personal cases which, from the clinical point of view, presented the dementia symptom associated in varying degrees to other neurological symptoms such as disturbances of the gait and of the sphincters functions (Adams' triad). All four subjects presented dilatation of the cerebral ventricles without cortical atrophy. From the histological point of view, there was: exfoliation of the ependyma, subependymal gliosis, demyelination of the white periventricular matter and spongiosis; there were no lesions of the meninges, of the cerebral cortex, no vascular alterations, except for those due to age, or stenosis of the aqueduct. The cause of the ventricular dilatation that was responsible for the clinical symptoms was not clear from the histological examination; the value and the significance of the histopathological data obtained and from the data available from the review of the literature are discussed and they point out the fact that many of the lesions encountered seem to be the consequence rather than the cause of the hydrocephalus.(ABSTRACT TRUNCATED AT 250 WORDS)

Nucleolar organizer regions and Ki-67 immunostaining in ductal breast cancer: a comparative study.

53 cases of invasive ductal (NOS) carcinomas of the breast were studied by means of an immunostaining method with Ki-67 monoclonal antibody and an argyrophilic method for the demonstration of Nucleolar Organizer Regions (AgNORs). The percentage of cancer cells with nuclear Ki-67 immunoreactivity and the mean number of NORs for each tumour were statistically related. The data obtained showed a good correlation between Ki-67 index and NOR score (rS = 0.47 - P less than 0.001). The authors suggest that the AgNOR method--which is applicable to routinely processed material--might effectively substitute Ki-67 immunostaining as a marker of cell proliferation in ductal breast cancer.

Intracerebral granular cell tumor. Case report.

The authors report a case of intracerebral granular cell tumor. These neoplasms are uncommon and, particularly, the intracerebral location has been reported only in ten previous cases. The authors discuss the clinical and histological features of these lesions, the histogenesis of which is still controversial. Our data seem furthermore to support the theory that at least in most cases, granular cell tumors are derived from Schwann cells.

[The nucleolus organizer region in benign and malignant melanocyte lesions of the skin: a quantitative study].

By means of a silver staining method, Nucleolar Organizer Region-associated proteins (NORs) have been evaluated on paraffin sections of two series of benign and malignant melanocytic skin lesions (represented by 30 intradermal naevi and 30 malignant melanomas). A significant difference (P less than 0.001) was found between the numbers of Ag-NORs in the nuclei of benign lesions and those of malignant lesions. It is suggested that this method could be of value in assessing the biological potential of cutaneous melanocytic neoplasms.

Aggressive angiomyxoma of the pelvis and perineum. Report of a case with immunohistochemical and electron microscopic study.

A case of aggressive angiomyxoma (AAM) of the pelvis and perineum is reported, whose clinical and pathological findings are consistent with the literature. This is a clinicopathological entity recently described by Rosai and colleagues (1983) who stressed its locally infiltrative nature and its tendency to recur. Its differential diagnosis includes a series of benign and malignant soft tissue tumours.

[Quantitative evaluation of the nucleolus organizer regions (NORs) in malignant melanoma of the skin: is it a new prognosis index?].

Nucleolar Organizer Region-associated proteins (NORs) have been evaluated on paraffin sections of a series of cutaneous malignant melanomas, stage IA at the time of the initial diagnosis. A significant difference (P less than 0.001) was found between the numbers of AgNORs in the cases which developed metastases and those without metastases: increasing values of AgNORs representing increased risk of metastases.
