Massive air embolism during cardiopulmonary bypass: successful treatment with immediate hypothermia and circulatory support.

Two patients experienced an episode of massive air embolism during extracorporeal circulation. Several emergency measures were taken. (1) The roller pump was reversed to take out air from the aorta. (2) The circuit was disconnected and recirculated to eliminate air bubbles. (3) Perfusion was restarted and the patient cooled to 24 degrees C for 40 minutes. (4) Pentothal (thiopental), steroids, and assisted ventilation were administered. The outcome was favorable in both patients, and there were no neurological sequelae. Immediate cooling and prolonged circulation seem to be a satisfactory approach to this problem.

Becker's melanosis: an organoid hamartoma.

Herein we have reported the histologic findings in two cases of Becker's melanosis. In one patient the lesion showed hyperplasia or hair muscles and in the other patient the lesion was associated with an underlying neurofibroma. The proliferative changes seen in the epidermis and dermis in Becker's melanosis can best be explained by the concept of an organoid hamartoma with involvement of the epidermis, pilar structures, and some other dermal components. The localization of the plexiform neurofibroma within the Becker's nervus is a unique observation. It may fall within the spectrum of an organoid hamartoma or it may only represent a chance occurrence.

[Postoperative complications and therapeutic management after aneurysmectomy of the left ventricle].

51 patients underwent left ventricular aneurismectomy between 1975 and 1980. In 37 patients a Mitral Valve Replacement and/or Myocardial Revascularization were associated. Low cardiac output and ventricular arrhythmias accounted for more than 50% of the problems encountered. Hospital mortality was 11.7%. Low cardiac output and ventricular fibrillation were the only causes of death, and 60% of the times they affected patients who had a Mitral Valve Replacement + Myocardial Revascularization associated to the aneurysmectomy. Left ventricular aneurysmectomy profoundly alters the geometry of the left ventricle which tends to be in a negative oxygen balance during the first postoperative hours with ischemia and possible cardiac failure ensuing. The thorough monitoring of the filling pressures of the left ventricle and the prompt and aggressive therapy of the complications are a must and strongly contributed to lower the hospital mortality after left ventricular aneurysmectomy.

Outdoor mobility and social relationships of elderly people.

Social relationships and activities are important elements in the quality of life of older people. With advancing age, they are made more difficult through the possible loss of physical functions, through societal processes of differentiation, and through unfavourable environmental and technological conditions. Therefore, mobility becomes a fundamental prerequisite for the participation in social relations and activities. This article presents some initial findings of a project which investigates the mobility needs of the elderly and the main factors hindering their desired mobility in three European countries. It became apparent that there is a clear connection between the social situation of elderly persons and specific mobility patterns: if they are tied into a closely meshed network of family or friends, then they are away from home more frequently than when this is not the case. In addition, individual factors like the age, the health situation and the driving ability of a person are important influences. External factors which restrict the mobility can be found in the respective spatial or technical conditions, on the one hand, and in affecting social interactions in the public sphere on the other hand.

Bioprosthetic valve endocarditis: factors affecting bad outcome.

Prosthetic valve endocarditis (PVE) remains an uncommon but serious complication of cardiac valve replacement. We analysed several risk factors (active or healed, early or late endocarditis, congestive heart failure, arterial emboli etc.) in order to identify the factors which may predict bad outcome. The overall mortality rate was 46.8% (15/32 patients). There was a significantly higher mortality rate in patients with early endocarditis (80%) than in those with late endocarditis (38%) (p less than 0.01). In the group of patients who underwent reoperation, the mortality rate was higher in those with active endocarditis (70%) than in those with healed endocarditis (28.5%) (p less than 0.05). We believe that combined medical and surgical treatment is the best management for bioprosthetic valve endocarditis, with the institution of appropriate preoperative antibiotic therapy, to attempt to achieve sterilization.

A preliminary report on low-profile and pericardial bioprostheses: a new combination in particular anatomical situations.

Bovine pericardial bioprostheses (BPB), such as the Ionescu-Shiley or the Vascor, and low-profile porcine bioprostheses (Liotta, LPB) have particular features that make them useful in certain anatomical situations, such as aortic stenosis with hypoplastic annulus, pure mitral stenosis with little ventricle and double valvular stenosis with hypertrophic ventricular wall. In such situations, the use of traditional porcine bioprostheses causes problems. We report our experience with BPB and LPB in 25 cases of double valvular stenosis. Operative results were fairly good, and, with the excellent mid-term follow-up by 2-D echo and haemodynamic evaluation, the present authors are confident that these bioprostheses can be considered to be a good alternative to traditional ones and the first choice in particular anatomical situations.
