Psychodynamic factors contributing to esthetic dental failures.

Traditionally, dentistry has focused on biomedical and mechanotechnical approaches to patients' dental health care. Objective and quantifiable measurements, such as tooth form, occlusal function, color, and biological compatibility, are conventional indicators of success or failure. However, unlike most facets of dental practice, the attainment of an enhanced facial and dental appearance is influenced and measured by more abstract, psychological factors such as perception, ideal body image, self-image, and personal motivation. These complex concepts are affected by emotional and personality factors that develop during the life cycle. This article will explore a psychodynamic perspective of patients who are dissatisfied with the esthetics of their dental treatment, although the result may be functionally and esthetically acceptable.

Advantages and limitations in the use of porcelain veneer restorations.

Porcelain veneer restorations including preparations, impression materials, cast materials, refractory casts, handling of porcelain, the try-in, and the final luting are discussed. Techniques are described to increase marginal accuracy, establish predictable natural polychromatic color within the porcelain, establish good contours and surface textures, and shorten final placement time. Tooth preparation is routinely recommended, as is the combination of epoxy dies, refractory casts, and anatomic buildup techniques. Emphasis is placed on a close working relationship and communication among the dentist, dental technician, and patient.

Modern dentistry and the esthetically aware patient.

With esthetic dentistry, the profession derives excitement and satisfaction from providing service patients want, not just need. The new materials and techniques rest on a firm foundation of sound restorative and diagnostic principles. Patients who seek esthetic dentistry today are informed consumers who have specific objectives. Esthetic care, more than any other treatment, has psychological aspects, the consideration of which is crucial to treatment success.
