The ethnic minorities of southern Italy and Sicily: relationships through surnames.

Surnames of grandparents were collected from children in the primary schools of the Albanian-Italian, Croat-Italian and Greek-Italian villages of southern Italy and Sicily. The coefficients of relationships by isonymy show almost no relationship with ethnicity. Ethnolinguistic minorities of southern Italy and Sicily are geographically subdivided into two main clusters: the first cluster comprises the Albanian, Croat and Greek communities of the Adriatic area; and the second cluster comprises the Albanian and Greek communities of the Ionian, Thirrenian and Sicilian areas.

Migration through surnames in Campobasso Province, Italy.

Data on grandparental surnames were obtained from school-children in 22 communes from Campobasso Province, Italy (Molise Region). The distribution of surnames was shown to be almost exactly linear by a log2-log2 transformation, which justified the fitting of the data to Fisher's logarithmic distribution. The values for v were higher among women. When v was standardized to minimize bias due to sample size, the value was one-third the estimate of migration from exogamy data. The higher values of v for females indicate that there is greater mobility of female marriage partners than males.

The effect of anthelmintic treatment on helminth infection and anaemia.

A 24-week randomized double blind intervention trial was conducted on adult female tea pluckers from an estate in Bangladesh to investigate the impact of iron supplementation and anthelmintic treatment on changes in ferritin and haemoglobin levels as well as on prevalence and intensity of helminth infections. A total of 553 women were randomly assigned to 1 of 4 intervention groups: group 1 received iron supplementation on a weekly basis, group 2 received anthelmintic treatment at the beginning and half way through the trial, group 3 received both iron supplementation as group 1 and anthelmintic treatment as group 2, and group 4 was a control group and received placebos for both iron supplementation and anthelmintic treatment. Prevalence and intensity of helminth infections (egg counts/g stool) of Ascaris lumbricoides, Trichuris trichiura and hookworms significantly fell in the 2 groups receiving anthelmintic treatment and there were some reductions in the 2 groups not receiving anthelminthic treatment. Haemoglobin and haematocrit concentrations increased significantly in the iron supplemented groups with smaller increases in the anthelmintic only group. All women showed a decrease in serum ferritin levels post-trial with greater losses in the 2 dewormed groups. Significant negative associations were found between hookworm egg counts and ferritin levels and Trichuris trichiura egg counts and haemoglobin concentration.

The effect of weekly iron supplementation on anaemia and on iron deficiency among female tea pluckers in Bangladesh.

AIM: To investigate the effect of weekly iron supplementation on anaemia and iron deficiency among adult, female tea pluckers. METHOD: A randomized double-blind intervention trial was conducted in a tea estate in Bangladesh where a total of 280 women received either weekly iron supplementation (200 mg ferrous fumarate and 200 mg folic acid) for 24 weeks or a matching placebo. Capillary blood samples were drawn at baseline and post-trial to determine haemoglobin, haematocrit and ferritin concentration. Mean corpuscular haemoglobin concentration (MCHC) was calculated using the haemoglobin and haematocrit values. RESULTS: The mean haemoglobin concentration in the supplemented group increased by 5.52 g L-1 over the study period, on average, while ferritin values decreased by 0.33 microgram L-1. The control group showed a decrease in both mean haemoglobin (-0.24 g L-1) and ferritin (-5.32 micrograms L-1). Those individuals in the supplemented group with the lowest pretrial haemoglobin and ferritin values experienced the greatest improvements post-trial, whereas nonanaemic individuals showed a decrease in both haemoglobin and ferritin concentrations. A total of 62.2% of women in the supplemented group reported feeling better and more energetic compared to 51.1% in the placebo group; 14.4% of the supplemented group and 22.7% of the control group complained about side-effects. CONCLUSION: Weekly iron supplementation was logistically simpler and cheaper than daily supplementation but would have to be continued on a longer term basis in order to combat both anaemia and iron deficiency.

Intestinal helminth infections, anaemia and labour productivity of female tea pluckers in Bangladesh.

We conducted a randomized clinical intervention trial over 24 weeks on a tea estate in north-east Bangladesh to investigate the effect of iron supplementation and anthelmintic treatment on the labour productivity of adult female tea pluckers. A total of 553 full-time tea pluckers, not pregnant and not breastfeeding, were randomly assigned to one of the four intervention groups: group 1 received iron supplementation on a weekly basis, group 2 received anthelmintic treatment at the beginning and half-way through the trial (week 12), group 3 received both iron supplementation as group 1 and anthelmintic treatment as group 2, and group 4 was a control group and received placebos. No significant difference in labour productivity was found between the four intervention groups over the trial period. However, there was a negative association for all three worms (Ascaris lumbricoides, Trichuris trichiura and hookworms) between the intensity of helminth infections (eggs/g faeces) and all measures of labour productivity. Lower haemoglobin values and anaemia (< 120 g/l Hb) were both associated with lower labour productivity and more days sick and absent. Taller women with greater arm circumference were able to pluck more green leaves, earn higher wages and were absent less often.

Genetic structure through surnames in Campobasso Province, Italy.

The population of Campobasso Province shows a level of inbreeding that is distinct from most Italian rural populations, regardless of their geographic location (Fr=0.0040; Fn=0.0102; Ft=0.0142). The genetic structure of the Italian-Greek communities of Lecce and Reggio Calabria Provinces does not appear to be affected by ethnicity. The level of inbreeding in Italian-Greeks of Reggio Calabria Province is similar to other Italians of Campobasso Province (Fr=0.0041; Fn=0.0127; Ft=0.0168). The Italian-Greeks of Lecce Province show random mating, and their inbreeding is in fact very low (Fr=0.0038; Fn=0.0024; Ft=0.0062).

Repeated measurements of energy intake, energy expenditure and energy balance in lactating Bangladeshi mothers.

OBJECTIVE: To examine changes in energetic parameters and nutritional status of chronically malnourished lactating women over a period of 13 months postpartum. DESIGN: A study of 15 lactating Bangladeshi mothers, of whom eight were tea pluckers and seven housewives, was conducted over a 13 months period from the birth of the child. Anthropometric measurements and body composition were determined on five occasions (at 3.5, 5.5, 7.5, 10 and 13 months) and energy expenditure (heart rate monitoring method), and energy intake (3-day weighing) on three occasions at 3.5, 10 and 13 months. RESULTS: The mothers, all of whom were of low BMI (range 14.9-18.1 kg/m2 at the end of the study) undertook high levels of physical activity (pluckers more so than housewives). There was evidence of weight loss over the 13 months, particularly between the fourth and sixth months postpartum, mainly due to a significant decrease in fat-free mass. Food intake and expenditure were higher in pluckers than housewives but energy balance was not significantly different, although workers were, on average, in negative balance on all three occasions. Food intake was based on cereals and fat deficient. CONCLUSIONS: A Bangladeshi sample of breast-feeding mothers studied over 13 months postpartum showed evidence of chronic malnutrition with women living on very low fat diets. All mothers, either housewives or tea pluckers were involved in energy demanding activities. There was a general tendency towards negative energy balance. Mothers lost weight, mainly fat-free mass over the study period.

The potential use of maternal size in priority setting when combating childhood malnutrition.

OBJECTIVE: To devise a strategy for assessing the nutritional status of a household and specifying the major needs in combating childhood wasting, distinguishing between inadequate food availability, poor parental care and/or the need for improved public health measures. DESIGN: An evaluation of the relationship between children's wasting, stunting, or underweight and mothers' or adult women's body mass indexes (BMIs) in the same household. A household was designated as 'malnourished' on the basis of a single child's weight/height of < -2.0 s.d. or at risk of being malnourished if the Z-score was below--1.5. Adult women's BMI was taken to signify adequate household food availability. Sibling concordance of anthropometric measures was investigated. RESULTS: A wide variety of prevalence of severe (BMI < 16.0), moderate (BMI 16.0-16.9) and marginal (17.0-18.4) malnutrition existed in the various study areas. The worst condition was recorded in India, while 18% of the women in Zimbabwe were classified as obese. Similarly wide variation in the prevalence of child wasting and stunting was observed, with the Indian children again faring worst and those in Zimbabwe the best. The within-household analysis of concordance gave higher concordance for height than for weight between siblings. Mothers' BMI was highly correlated with the BMI of all other adult women in the same household and the BMI of all the women was found to be as useful as that of the mother for relating to children's anthropometry. Households with mothers of normal body weight but wasted children were designated as in need of public health measures and improved parental care rather than of enhanced food security. The distribution of households on this combined basis of maternal BMI and child nutritional status highlighted very diverse situations in the various study areas, with higher proportions of combined maternal and child malnutrition in India and in some areas of Ethiopia, while in Zimbabwe only 1-2% presented this condition. On this basis, the principal problem in India was food security; in Zimbabwe household security was rarely apparent, so public health measures and maternal care were designated as problems. In three Ethiopian communities there was a mixture of needs. CONCLUSIONS: A relatively simple household-based approach is proposed to discriminate the most pressing needs in combating childhood malnutrition, and a policy-making tool is suggested for setting priorities in community action.

Association of vitamin D receptor genotype with leprosy type.

Host genetic factors including major histocompatibility complex (MHC) polymorphisms influence both susceptibility to leprosy per se and also to leprosy type. Non-MHC genes may play an important role, but such genes remain undefined. The influence of two non-MHC candidate genes was assessed in a case-control study of Bengali leprosy patients from Calcutta. Recent studies have implicated variation in the vitamin D receptor (VDR) gene in susceptibility to several diseases, including osteoporosis and pulmonary tuberculosis. In this population, homozygotes for the alternate alleles of the VDR polymorphism are associated, respectively, with lepromatous and tuberculoid leprosy. The NRAMP1 (natural resistance associated macrophage protein 1) gene may influence human mycobacterial disease susceptibility based on studies with the murine homologue Nramp1. However, no significant association was found between NRAMP1 and leprosy susceptibility. This study suggests that the VDR polymorphism may influence susceptibility to some diseases by affecting the type and the strength of the host immune response.

A study of the cost effectiveness of selective health interventions for the control of intestinal parasites in rural Bangladesh.

The study examined the cost effectiveness of 4 different regimens in reducing the prevalence and intensity of infection of Ascaris lumbricoides, Trichuris trichiura, and hookworm over an 18-mo period in randomized community samples of children aged 2-8 yr living in rural Bangladesh. The household was the unit of randomization in each community. The 4 regimens were (1) only chemotherapy to all household members at the commencement of the study (i.e., at an interval of 18 mo), (2) same as group (1) and regular health education throughout the study period, (3) chemotherapy to all household members at the commencement of the study and subsequent chemotherapy to all children at intervals of 6 mo, and (4) same as group 3 with the addition of regular health education throughout the study period. Health education (through home and school visits and focus group discussions) was aimed at increasing awareness of worm transmission and the disabilities caused by intestinal helminths. Simple ways of improving personal hygiene and sanitation through hand washing, nail trimming, wearing of shoes, and use of a latrine and clean water supplies were encouraged. Because albendazole is a broad spectrum anthelmintic, the cost effectiveness of the 4 interventions were compared by the weighted percentage reduction in prevalence and the weighted percentage reduction in intensities of infection as measured by geometric mean egg loads of all 3 worms combined. The most cost-effective strategy was the single albendazole mass chemotherapy at an interval of 18 mo. The 2 regimens involving health education were the least cost effective.

The effect of dietary intervention on changes in total cholesterol, blood pressure and weight in a Cambridge study.

The study examined the impact of dietary advice on cholesterol reduction, to see whether diet was more effective when preceded by a cholesterol estimation. A total of 635 healthy individuals aged between 40 and 60 years attended a general practice cardiovascular risk screening in 1990. All subjects had their blood pressure, body mass index and smoking history recorded, and 50% were randomised to cholesterol measurement. Two years later, all individuals were invited to participate in a follow-up screening and 356 individuals took part (158 men, 198 women). All the above estimations were undertaken in 100% of attendees. The mean total cholesterol levels and weight declined in the subjects who were given dietary advice and had their cholesterol measured, while the mean cholesterol level increased slightly in subjects who did not receive dietary advice but had their cholesterol measured. The was a significant increase in diastolic blood pressure in all patients. The figures for reported smokers decreased from 22.4% to 19.4% in men and 19.4% to 8.4% in women. The results suggest that dietary advice can achieve a sustained reduction in cholesterol levels, but may require the stimulus of a prior cholesterol estimation.

Consanguinity and its relationship to differential fertility and mortality in the Kotia: a tribal population of Andhra Pradesh, India.

Data on patterns of marriage, differential fertility and mortality were collected from 211 Kotia women residing in Visakhapatnam district of Andhra Pradesh, India. Consanguineous marriages made up just over a quarter of the total, and of these, father's sister's daughter (FSD) were more common than mother's brother's daughter (MBD). The mean inbreeding coefficient for the sample (F) was 0.0172. Women in consanguineous marriages had a lower mean number of total conceptions, live births and living offspring (net fertility) than women in non-consanguineous marriages. Significant heterogeneity was found in the means of living offspring for FSD, MBD and non-consanguineous couples, but not for conceptions and live births.

Tumor necrosis factor promoter polymorphism and susceptibility to lepromatous leprosy.

Genetically determined differences in immune responses to environmental agents may underlie susceptibility to many autoimmune and infectious diseases. Leprosy provides an example of a polarity in the type of immune response made to an infectious agent, and there is evidence that the major histocompatibility complex is genetically linked to leprosy type. It was found that HLA-DR2 is associated with both tuberculoid and lepromatous types of leprosy; however, a variant at position -308 of the promoter of the neighboring tumor necrosis factor (TNF) gene was increased in frequency in lepromatous (odds ratio = 3.0, P = .02) but not tuberculoid leprosy. Some studies have found higher serum levels of TNF in lepromatous than tuberculoid leprosy, and high TNF levels are found in malaria and leishmaniasis, which are also associated with this TNF allele. It is speculated that this association reflects genetic variability in cytokine production, which influences the immune response to and clinical outcome of leprosy.

Smoking and asthma among 23-year-olds.

Participants in a longitudinal cohort study (the National Child Development Study) were asked, at the age of 23, about their smoking habits and asthmatic experiences since 16 years of age. Of the total sample (n = 8860) 10.8% reported smoking cigarettes, and the percentages were very similar in the two sexes although males tended to be heavier smokers. There was an association between asthma and smoking; more than expected of those reported as having asthma at any age had smoked, and of those with asthma since 16 years of age more reported smoking than expected by chance. In addition, all who report asthma at any time since the age of 16 are overrepresented among those who report current smoking (p < 0.001). Those reporting asthma since 16 are more likely to be living with others who smoke, and their spouses or partners were more likely to be heavy smokers (30+ cigarettes per day). In addition, in more than the expected number of homes where asthmatics live, there are others who smoke (p < 0.003).

Influence of social class on the correlation of stature of adult children with that of their mothers and fathers.

Studies of parent-child correlations in stature require data which can be viewed as random samples of some general population and which are large enough to allow partition of the variable and evaluation of non-genetic and genetic influences. In a sample of 4336 individuals drawn from a cohort of all persons born in England, Scotland and Wales in 1 week in 1958, the correlation of statures of the males with their fathers, the females with their fathers, the males with their mothers and the females with their mothers were 0.36, 0.43, 0.41 and 0.47 respectively at age 16 of the offspring and 0.41, 0.41, 0.47 and 0.46 respectively at age 23. Allowance for the occupational social class of the fathers lowers the correlations, but in no case by more than 5%. Allowance for the occupational class achieved by the offspring by age 23 has little effect on the correlations.

Repetition of the same pair of surnames in marriages in Albanian Italians, Greek Italians, and the Italian population of Campobasso Province.

The isolation of a population as a result of any boundary leads to a kinship mating pattern, the extent of which can be measured by the frequency of repeated pairs of surnames in actual marriages compared with that in random pairings. The repeated pairs within surname lineages (RPw) method can be used to assess random repetitions and the endogamous or exogamous behavior of a population. The RPw method was applied to data from grandparent surnames of children living in 45 Albanian Italian and 13 Greek Italian villages of southern Italy and Sicily and in 22 Italian villages of Campobasso Province (central Italy). The total mean RPw was 0.02782 in Albanian Italians, 0.01993 in Greek Italians, and 0.03427 in the Italian-speaking population. When RPw was subdivided into its components and compared with random and marital isonymy, the low level of inbreeding shown by the two southern Italian ethnic minorities and by the Italian population of Campobasso Province could be accounted for by the subdivision of the populations.

Further note on possible changes in the geographic distribution of ABO and RH blood groups in Great Britain.

We previously reported the tendency for the clines of blood group distributions in England, Scotland, and Wales to become flatter as the result of internal migration. The data came from a one-week birth cohort followed for 16 years. A further seven years and a finer geographic analysis yield mixed results. The Rh- clines did become flatter, but the clines for ABO blood groups A and O showed the opposite tendency during the additional years.

Effect of interregional migration on geographic variability in biological and social traits in Great Britain.

Over 19% of individuals born in England, Scotland, and Wales during March 3-9, 1958, resided in 1981 in a region different from the region of their birth. This internal migration among the 11 regions increased geographic homogeneity for one genetic variable (ABO blood group). Cramer's V for mother's A, O, and B or AB blood group decreased from 0.0504 to 0.0476. Mother's Rh+/- blood group was not significantly different among regions of place of birth or subsequent place of residence of the offspring. Variability among the regions increased by migration from region of birth to region of residence 23 years later for the social class of male head of household (Cramer's V increased from 0.0815 to 0.0877) and for years of schooling completed (V increased from 0.107 to 0.129). Stature behaved more like the social variables (mean square deviation among regions increased from 371 cm2 to 481 cm2 in females and from 426 cm2 to 471 cm2 in males), but body weight tended to become more uniform among regions (mean square deviation decreased from 220 kg2 to 178 kg2 in females and from 315 kg2 to 260 kg2 in males).
