Inhibition of human placental progesterone and estrogen synthesis in early human gestation by aminoglutethimide in vivo.

The inhibitory effect of d,l-aminoglutethimide (AG) on the synthesis of progesterone and estradiol in early human pregnancy (8th-12th week of gestation) was investigated in volunteers; control group (n = 11), AG group [1000 mg AG orally at test begin (n = 6)]. Venous blood samples were taken at the beginning of the test and 0.5, 1, 2, 4, 8 and 24 h thereafter. In controls, no significant changes in serum progesterone and estradiol could be observed during 24 h. In the AG group, a decrease in progesterone and estradiol could be observed within 1 h after the test began; lowest serum steroid concentrations were reached after 4 h. Relative to the initial values taken as 100%, the greatest decrease in progesterone ranged between 37 and 83%, 62 +/- 15% (means +/- SD)(n = 6); the greatest decrease in estradiol ranged between 32 and 78%, 51 +/- 17% (means +/- SD)(n = 6). Twenty four hours after AG treatment, both steroids reached similar concentrations to those found at test begin. No clinical signs (e.g. uterine bleeding, contractions) for the abortifacient action of AG were observed. In conclusion, a single dose of AG (1000 mg given orally) cannot induce a therapeutic abortion in early pregnancy. In accordance with in vitro studies, the inhibitory effect of AG on placental progesterone formation is due to an inhibition of mitochondrial cholesterol side chain cleavage. The decrease in estradiol is thought to be related to an inhibition of placental aromatase.

[Synthesis and analgesic effect of 2-aryliminomethylquinolines].

2-Aryliminomethylquinolines were synthesized by the condensation of quinoline-2-carbaldehyde with the corresponding anilines. The compounds were evaluated for analgesic activity using the acetic acid writhing test. Some of them showed significant inhibition of the writhing syndrome. The most potent compound 3b was approximately as active as aminophenazone in the writhing and the hot-plate method.

[Indomethacin--short-term therapy vs. single low dosage radiation for prevention of periarticular ossifications after total hip endoprosthesis].

INTRODUCTION: With a general incidence of about 40% periarticular ossifications (PAO) constitute one of the more frequent complications after total hip arthroplasty. A prospective, randomized therapy--study investigates the prophylactic values of Indomethacin-Short-Term-Therapy and Single-Dose-Radiatio, respectively. MATERIAL AND METHODS: Consecutive patients for elective, cementless THA were randomized; the Indomethacin-Group (A, n = 31 at the end of the study) was admitted 100 mg Indomethacin/supp. at the day of operation and 3 x 25 mg orally from postoperative day 1 to 10. Other NSAID were not given. The Radiatio-group (B; n = 19) was irradiated by a single dose of 6 Gy within postoperative days 1 to 4; no NSAID were admitted. Follow up was at dismissal and after 6 (NU 1) and 12 (NU 2) months. Incidence and severity of PAO and the clinical objective and subjective results were registered. RESULTS: Age- and side-distribution as well as the etiologies were comparable in both groups. All patients took part in NU 1 and 90% in NU 2. There were no major differences in the incidence of postoperative PAO in both groups. In 68% (A) and 53% (B), resp., there were no PAO at dismissal and after six months, PAO of Brooker Grade I and II were seen in 30 (A) and 47% (B), resp., PAO of Grade III and IV--those having generally clinical relevance--did not occur at all. These results were confirmed after 12 months. The clinical objective result ameliorated between admission and dismissal by an average of 3.5 points and by another 2 points between dismissal and NU 2. DISCUSSION: The results show that Indomethacin-Short-Term-Therapy as well as Single-Dose-Radiatio with 6 Gy can reliably prevent the occurrence of severe PAO. Both therapeutic concepts therefore can be employed as prophylaxis in primary endoprosthetic operations. The choice between the two procedures will then mainly be determined by given logistic conditions in the clinic, specific contraindications of the patient and financial considerations.

Novel propargylamine-linked nucleosides for high throughput SNP genotyping by MALDI-TOF MS.

The synthesis of positively charged and mass tagged nucleosides containing a quaternary ammonium functionality within the penultimate position of a primer is described. Neutralization of the sugar/thiophosphate backbone by alkylation increases the detection sensitivity in the mass spectrometric analysis by a factor of at least 100. The variable introduction of these novel compounds within the extension primers enables flexible design of multiplex genotyping reactions.

18-FDG positron emission tomography of the pancreas: diagnostic benefit in the follow-up of pancreatic carcinoma.

UNLABELLED: The aim of this prospective study was to determine the role of FDG-PET in the follow up care of patients with pancreatic carcinoma in comparison with sonography, computed tomography and tumour marker levels. 19 patients were investigated, 9 patients with non-resectable tumour disease and 10 patients after resection of primary tumor. We found corresponding results in 15/19 patients, but in 2 patients additional metastases were detected. In another 5 patients of this group, PET results decisively influenced the further therapeutic procedure. In 2/19 patients only PET showed viable tumour tissue, in spite of negative findings in CT and sonography. In 2/19 patients we found false negative results with PET. One patient had increased blood glucose levels at the time of investigation, the other one was investigated shortly after locoregional chemotherapy. Compared to CA 19-9, PET investigation may also result in positive findings, in spite of non elevated CA 19-9 serum levels. CONCLUSION: PET has been shown as an effective tool also in the follow up care of patients with pancreatic cancer. In more than half of the patients PET offered important additional information to clinicians, resulting in a change of the therapeutic procedure. Especially before locoregional chemotherapy or in case of increased tumor marker levels without morphological substrate in US/CT PET with FDG seems to represent a valuable investigation.
