Mifentidine: evaluation of antiandrogen effects and kidney function studies.

Among a number of investigations aimed at evaluating the safety profile of a new antihistamine (H2), N1-[(4-imidazolyl)-phenyl]-N2-isopropyl-formamidine (mifentidine), its effects on male accessory sexual organs and on kidney function were studied in greater detail because of the known propensity of H2-antagonists to cause antiandrogenic activity and to alter renal function. Mifentidine (190 and 380 mg/kg p.o.) was compared to cimetidine (475 and 950 mg/kg p.o.) for its ability to affect growth of seminal vesicles and prostate of immature castrated rats treated either with testosterone (TP) or 5-alpha-dihydrotestosterone (DHT). Lower doses of mifentidine were investigated because, in terms of H2-antagonistic activity, it was shown to be several times (15 to 30) as potent as cimetidine. Cimetidine at the highest dose (950 mg/kg) reduced prostate growth sustained by TP, and the DHT-promoted growth of seminal vesicles and prostate. Mifentidine had no effect on hormone promoted growth of male accessory sexual organs. Renal function studies after repeated (7 days) administration of mifentidine (150 and 600 mg/kg p.o.) and cimetidine (1500 mg/kg p.o.) revealed a significant increase of the excreted fraction of filtered sodium (FeNa) and potassium (FeK) only after cimetidine, indicating altered tubular reabsorption. Creatinine and urea clearances were unchanged by either drug treatment. Thus mifentidine appears to be devoid of antiandrogenic activity and renal effects.

[A microanalytical gas liquid chromatography method with a nitrogen-phosphorus detector in the assay of various psychotropic drugs of tricyclic structure and of their N-demethylated metabolites].

Nanogram amounts of chlorimipramine, chlorpromazine and their Nor1- and Nor2-metabolites were detected in plasma by GLC with nitrogen-sensitive detection. Two extraction procedures were compared. The use of Sep Pak C18 cartridges produced a higher degree of accuracy and precision with significant time and materials saving, as compared to the use of organic solvents in a three steps extraction procedure.

Plasma exchange in myasthenia gravis.

Results obtained with P.E. therapy on 19 myasthenic patients are reported. 15 of them improved during treatment and 4 remained unchanged. We believe P.E. is an useful therapy in M.G. because it produces a rapid improvement in most patients.

Plasma exchange in radiculopolyneuropathies.

The AA. report the results obtained with Plasma Exchange (P.E.) therapy on 24 cases of polyradiculoneuritis; eleven patients had typical Guillain-Barré Syndrome (G.B.S.) ten suffered from a chronic progressive form and three were affected by a relapsing form. The acute patients were selected according to criteria established by the NNCDS Committee (1978) while guidelines laid down by Pineas & Load (1978) were followed in choosing chronic cases. Patients with G.B.S. had four sessions of P.E. at intervals of one or two days while those with chronic forms of polyradiculoneuritis had a total of 6 sessions spaced one to three days apart. P.E. produced no apparent change in chronic progressive patients but two out of three cases with the chronic relapsing form showed rapid and steady improvement resulting in complete cure within a few months. Nine of the eleven G.B.S. patients showed after only one or two sessions a clear and rapid improvement which led to a complete cure within a matter of weeks. The remaining 2 cases showed only partial improvement at first and remained stationary thereafter. The findings confirm the usefulness of P.E. in acute and chronic relapsing radiculopolyneuritis. However, potential side effects and the procedure's high cost suggest that its use should be limited to carefully selected cases, and in particular to those involving respiratory disorders.

Clozapine inhibition of metenkephalin binding to synaptosome-enriched fractions of rat whole brain and hippocampus.

Clozapine has been found to inhibit the binding of [3H]met-enkephalin to synaptosome-enriched fractions prepared from rat whole brain and hippocampus in a Tris-buffered media without added ions. The IC50 value for hippocampus is 28.5 microM. The inhibition constant for whole brain (41.0 microM) was found to be slightly larger than the corresponding value for chlorpromazine, which was measured under similar experimental conditions.

Late embolization of disc occluder of a Beall mitral prosthesis with survival.

A case of arterial embolization of a Beall mitral prosthesis disc (model 104) 5 1/2 years after implantation is presented. The patient was referred to use 3 days after the onset of acute symptoms and was immediately operated upon in pulmonary edema and cardiogenic shock. The migrated disc was removed from the abdominal aorta 26 days later, showing marked signs of wear. The postoperative course was complicated and the patient made a slow recovery. It is suggested that patients with models 103 and 104 Beall mitral prostheses be followed very closely and that electric valve substitution should be considered when first signs of prosthetic attrition are noted.

Nijmegen breakage syndrome-associated T-cell-rich B-cell lymphoma: case report.

In 1981 Weemaes et al. first described the Nijmegen breakage syndrome (NBS), a rare autosomal recessive disorder characterized by stunted growth, microcephaly, immunodeficiency, spontaneous chromosome instability, and a peculiar predisposition to cancer development. Most NBS-related malignancies are lymphomas, but their pathologic features have rarely been specified. We report here the case of a northern Italian 8-year-old child who, 2 years after the diagnosis of NBS, developed a diffuse large B-cell lymphoma (T cell-rich B-cell lymphoma variant). The histological and immunobiological features of the lymphoma population are analyzed and discussed in detail.

[Clinical and radiologic assessment of vertebroplasty].

INTRODUCTION: Percutaneous vertebroplasty consists of the injection of acrylic cement into weakened vertebral bodies to achieve pain relief and mechanical stability of the spine. OBJECTIVE: To evaluate the characteristics and effectiveness of the vertebroplasties performed at the Hospital Universitario Miguel Servet in Zaragoza. PATIENTS AND METHODS: This is a retrospective study of 147 vertebroplasties performed in 95 patients (60 women and 35 men; age range: 19 to 84 years). The oblique transpedicular approach, which achieves adequate cement injection with a single puncture, is currently used. A visual analogue scale (VAS) was used to evaluate pain before and after the procedure. RESULTS: The osseous lesion most often treated by vertebroplasty is fracture secondary to osteoporosis, accounting for 65% of the cases in this series, followed by hemangiomas (23%), and osteolytic metastases, traumatic fractures, lymphomas, and myelomas. Prior to vertebroplasty, the mean VAS score was 8.88 versus 2.78 after the treatment. Only 7.3% of the patients had symptomatic complications. CONCLUSION: Vertebroplasty is safe and efficacious; it is the treatment of choice for vertebral pain refractory to medication. It enables patients to return to their habitual lifestyle quickly and thus helps reduce hospital stays and costs.

Validity of mother's history regarding antimalarial drug use in Malawian children under five years old.

History obtained from parents and carers is an important, and often the only, source of information for health workers treating children for malaria, but its validity has not been well evaluated. At 2 hospitals in Malawi, we obtained malaria treatment histories from mothers of 973 ill children reported to have had fever as part of the illness. Urine samples were collected from 755 of the 973 children (78%). Of the 755, 457 (61%) were reported to have received some kind of treatment. Among those who reportedly received treatment, 79 (17%) were said to have received chloroquine and 23 (5%) a sulphonamide-containing medicine; however, when urine specimens were tested for antimalarial drugs, chloroquine was found in 182 specimens (40%) and a sulphonamide in 148 (32%). Among urine specimens collected from 291 children who were reported to have received no treatment (no report was recorded for 7 children), chloroquine was detected in 56 (19%) and a sulphonamide in 44 (15%). Although not statistically significant, mothers often reported a child as not having received an antimalarial drug if the child was younger than 12 months or had been sick for more than 3 d. The mothers' information regarding home treatment of fever in children was highly inaccurate. Malaria treatment histories, whether collected at health facilities or in surveys of knowledge, attitudes, and practices, must be interpreted with caution.

Inhibition of erythrocyte aminolevulinate dehydratase by a 56.2-kD peptide from uremic plasma.

Among the abnormalities in erythrocyte porphyrin metabolism already described in patients with chronic renal failure on hemodialysis, a decrease in blood aminolevulinate dehydratase activity has been reported, suggesting the presence in uremic plasma of an inhibitor of the enzyme. The aim of this work has been to isolate and characterize such an inhibitor. Blood samples from 105 patients with chronic uremia were collected; plasma was applied to Sephadex G-100 columns and the fraction with the highest inhibiting capacity was identified and purified by subsequent SDS-polyacrylamide gel electrophoresis, followed by electroelution and electroblotting. It was demonstrated that the factor present in plasma of uremic patients inhibited blood aminolevulinate dehydratase in a concentration-dependent manner; its inhibitory properties were abolished after heat, trypsin and TCA treatment indicating its peptidic nature. The purified inhibitor has an apparent molecular mass of 56.2 kD, it inhibits blood aminolevulinate dehydratase in a competitive way and the Ki value is 12x10(-6) M. The amino acid composition of the inhibitor has been determined and it has been found that its N-terminal amino acid is blocked. The isolated peptide may play a role in heme biosynthesis disturbances and in the pathogenesis of uremic anemia.

[Hypothalamic-infundibular histiocytosis: magnetic resonance findings].

INTRODUCTION: We report the magnetic resonance imaging (MRI) of a case of Langerhans cell histiocytosis in the pituitary. Isolated central nervous system involvement is uncommon. CASE REPORT: An eighteen-years old female patient who had an acute onset of central diabetes insipidus because of the Langerhans cell histiocytosis. The MRI evidenced a lesion in the hypothalamic-pituitary axis. The hyperintensity in the posterior pituitary lobe, which is seen in normal subjects on T1-weighted images, was absent. The pituitary stalk was thickened and enhanced homogeneously following contrast administration. During the follow-up, the infundibullar lesion extended to the hypothalamic region and other systemic manifestations appeared. Diagnosis of Langerhans cell histiocytosis was confirmed by lung biopsy. After radiotherapy and chemotherapy, MRI showed regression of the hypothalamic-pituitary lesion. CONCLUSION: The combination of these findings, although nonspecific of Langerhans cell histiocytosis, should nevertheless prompt further studies, including chest films, bone scanning and temporal bone computerized tomography in order to narrow the differential diagnosis.
