Towards a family approach in a psychiatric day hospital.

In a day hosptial all staff members must be able to make rapid decisions, even when they cannot refer to their seniors. It is difficult to develop this capability in a teaching day hospital where the staff and student groups are constantly changing. It has, however, been facilitated by regular staff discussions with a visiting psychotherapist. In these staff groups, the study of role attributions and dynamics characteristic of families has diminished the interference caused by 'family' type dynamics in daily professional interaction. The paper describes the effect of such discussion in stimulating the adoption of a treatment approach which takes the family situation into account. This family approach is distinguished from family therapy, and its advantages are discussed.

Late results of valvoplasty for mitral regurgitation due to rupture of chordae of the posterior (mural) cusp.

Nine patients with rupture of chordae tendinea of the posterior cusp of the mitral valve are presented. All have been treated by plication of the posterior cusp and atrial wall. The presenting postoperative features have been described: 89 per cent are markedly improved; two patients have normal heart sounds with no murmurs; 6 patients have only a Grade 1 pansystolic apical murmur. The advantages of this procedure compared with mitral valve replacement for the same condition have been presented.

Enhancement of recall of cardiopulmonary resuscitation (CPR): Use of moving-image card to review the dynamics of CPR.

Since victims of cardiac arrest have a more favorable outcome when CPR is immediately applied, it is important to continue to motivate individuals to learn proper technique. It is equally important to ensure that individuals who have learned CPR maintain adequate recall of the procedure. With the intent of enhancing recall of the principles of CPR, a wallet-size plastic card was developed which, when tilted, creates the illusion of movements. Thus, the card enables a viewer to review the dynamic aspects of CPR and reinforces the essential steps of establishing an airway and maintaining breathing and circulation.
