[Induced abortion in a Swiss canton from 1990 to 1993: implications for epidemiological surveillance].

BACKGROUND: To describe abortion utilization in the Canton of Vaud (Switzerland) and to identify desirable changes in the data collection system to improve the epidemiologic monitoring METHODS: Retrospective survey of the 5600 abortion requests to the Canton Health Authority for resident women between 1990 and 1993. RESULTS: Nine women out of 1000 residents requested an abortion, but the abortion rate was higher among women of foreign origin compared to Swiss women (17/1000 vs. 6/1000, p < 0.0001). The proportion of terminated recognized conceptions was 127/1000, and the ratio of IVG to livebirths was 0.18. The abortion rate was lower during adolescence (6/1000) or after 40 years of age (1.8/1000), whereas the proportion of terminated conceptions was highest at these ages (527/1000 and 312/1000). Abortion utilization in Canton of Vaud is among the lowest. CONCLUSIONS: Although these indicators do reflect a satisfactory accessibility and use of family planning services, improvements could be made. A better knowledge of the causes (motives, contraceptive method, reproductive history) and of the consequences of abortion (psycho-social implications, medical complications) would allow to design more focused prevention strategies and to fit health services to current needs. Updating the 1942 Swiss legislation is also desirable.

Adolescent sexuality: the gender gap.

The aim of this paper is to determine gender differences as regards adolescent sexuality, in order to improve the adjustment of prevention programmes to boys' and girls' specific needs. Data were collected as part of the Swiss Multicentric Adolescent Survey on Health. Anonymous questionnaires were distributed in school classes among a national representative sample of about 9,300 adolescents (15 to 20 years old). About 45% of the total sample reported a previous sexual experience. Differences between boys and girls were identified by means of bivariate and multivariate analyses. A higher proportion of Swiss girls report intra-family discussions about sexuality, having had a previous sexual experience, having sexual intercourse regularly, having had only one partner, and using contraception regularly. A higher proportion of Swiss boys report positive attitudes towards condoms and using condoms regularly. Variables independently associated with the occurrence of first sexual intercourse before 15 years old also show gender differences, notably as regards health behaviours and social adjustment. The findings suggest that prevention programmes should emphasize, among boys, responsibility in contraception and the need for protection in situations of multipartnership, and among girls, a positive attitude towards condom use and an increased familiarity with condoms presented both in a perspective of contraception and prevention of STDs.

[Implementation and evaluation of a program of prevention against abuse in at risk infants].

Teams of pediatricians and psychiatrists led a joint action-research in public health institutions for infants which intended to evaluate original modes of prevention of abuse concerning infants with medico-psychosocial high risk factors. The framework of this study was a randomized controlled trial on the modes of prevention. The sample, consisting of children selected according to risk indicators appearing on their health certificate of the 8th day, was divided into two groups: the treated group who benefited from specific prevention interventions and a control group who benefited from classic surveillance. The research was longitudinal and was centered on the study of mother-infant interactions at 3 months, 1 year, 2 years and 3 years of age. The dynamics of early interactions was studied very precisely on the occasion of each examination. The first results concern the 161 children (72 in the "intervention" group, 89 in the control group) who were evaluated at 1 year of age. Although a real change in practice could be observed (the treated group profited by a mean number of 7.5 actions, while the control group on the average profited by only 1 action), the efficacy of those actions could not be demonstrated. No significant differences were observed between both groups with respect to height and weight growth and psychomotor development, their somatic status and the quality of early interactions, except with respect to a better quality of interactions in the action mode for the treated group. The difficulties inherent in such a type or research are analysed (children lost to follow-up, contamination of the control group by actions intended only to the treated group, difficulty in disposing of reliable criteria of estimation).

Films on AIDS.

An assessment of 16 French films giving information on AIDS was made by people in the health, social work, teaching and other professions. Their observations are outlined below and the qualities required in such material are discussed.

[The health of adolescents: what do parents think about it?].

Parental care of adolescent children was studied in 501 families who attended the Centre of Preventive Medicine of Vandoeuvre-lès-Nancy. The health of their adolescent children was the parents' main concern and they considered morale, hygiene and alimentation as being the most important health determinants. Twenty-eight per cent of the parents indicated that they were anxious about their adolescent children. When they contacted the Centre of Preventive Medicine, parents were mostly seeking a regular check-up and information for their offspring. A correspondence analysis showed a breakdown of the parents into several groups, according to their responses and degree of concern for the health of their adolescents.

[Partial deficiency of cell-mediated immunity in some cases of bronchiectasis (author's transl)].

Two case-reports are presented to support the hypothesis that bronchiectasis may occasionally be associated with a partial deficiency of T lymphocyte functions. This deficiency would represent another etiology of the heterogenous sino-pulmonary syndrome of which several causes have been identified or postulated: immunoglobulin deficiency, alpha-1-antitrypsin deficiency, mucociliary abnormality, phagocytosis or bactericidia abnormalities, etc.

[Adolescents and AIDS: knowledge, attitudes, behaviors, perceptions and consequences for prevention].

The vital issues at stake in the credibility and effectiveness of AIDS prevention programs for young people have led to the emergence of a trend in research focused on the study of adolescents' understanding of AIDS, their attitudes and behaviour towards it, and their beliefs about it. This research can be classified under the three main ways of approaching the problem: 1) results of inquiries about knowledge, attitudes and behaviour; 2) research centred on beliefs and their consequences on a social and individual level; 3) reports and analysis of prevention programs. The results obtained by the different authors bring to light certain characteristics among the adolescent population, notably, their understanding of the disease, their attitudes towards people who are HIV positive, their sexual behaviour, their beliefs about AIDS, and the general direction of the prevention programs which are best adapted to this particular population.
