Functional characterization of cloned intrahepatic, hepatitis B virus nucleoprotein-specific helper T cell lines.

We have previously reported the establishment and preliminary characterization of polyclonal hepatitis B virus (HBV) nucleoprotein (HBcAg)-specific CD4+ and CD8+ T cell lines derived from the hepatic lymphomononuclear cell infiltrate of several patients with chronic active hepatitis B. The isolated subsets from these lines were specifically activated by HBcAg and displayed antigen-specific help and suppression with respect to proliferation of the alternate subset. One of these lines was recently cloned by limiting dilution, and four HBcAg-specific CD3+ CD4+ CD8-DR+ T cell lines were produced that had a 95.3% likelihood of monoclonality. Antigenic specificity was confirmed by dose-dependent, HLA class II (DR)-restricted proliferation in response to recombinant and human serum-derived HBcAg and the lack of proliferation to HBV envelope antigens (HBsAg and pre-S(2)Ag). All cloned lines were interleukin 2 dependent, produced interferon-gamma in an antigen-specific manner, and provided antigen-specific help to autologous B cells with respect to anti-HBc production. We conclude that HBcAg-specific, HLA-class II restricted helper T cells capable of inducing antigen-specific functional responses by autologous B lymphocytes and T lymphocytes are present at the site of viral antigen synthesis and hepatocellular injury in HBV infection.

Effects of FCE20700, a new PGE2 derivative, on gastric acid secretion and cytoprotective processes in man.

The pharmacodynamic effects of FCE20700, a new PGE2 derivative, have been investigated in 6 healthy volunteers given single intragastric (i.g.) and intraduodenal (i.d.) doses of 1 and 2 mg and placebo, according to a double-blind, within-subjects design. For 30-270 min following i.g. administration the effect of FCE20700 on peptone-stimulated gastric acid secretion (AS) was assessed by i.g. titration, and serum gastrin (G) levels were also determined. For the same period after i.d. dosing the effect of the compound on pentagastrin-stimulated AS and on mucoproteins and bicarbonate content in the gastric juice was measured. Blood pressure (BP), heart rate and possible side-effects were monitored. Following i.g. administration there was a moderate, dose-related, significant inhibition of AS; significant inhibition of G levels was observed only after the highest dose. After i.d. administration there was a very modest though dose-related and significant inhibition of AS; a brief maximal increase in mucoproteins and in bicarbonate levels was apparent after the 1 mg dose. After i.d. but not after i.g. administration of 2 mg there was a modest but significant decrease in BP. No side-effects of clinical relevance were reported. The results appear to suggest a major activity of FCE20700 on cytoprotection rather than in inhibiting gastric acid secretion. The observed change in BP may indicate that after i.d. administration there will be some systemic effects of FCE20700.

S-100-positive T cells are largely restricted to a CD8-positive, 9.3-negative subset.

The present report concerns the demonstration of the exclusive detection among peripheral blood T-lymphocytes of the S-100 protein within the CD8-positive subpopulation which lacks the antigen recognized by the 9.3 monoclonal antibody. Highly purified human peripheral blood T-cell subsets, obtained by means of panning techniques, were first stained, by an immunofluorescence method, with purified anti-S-100 protein antibodies. The vast majority of S-100 protein- (and, specifically, its beta subunit) positive cells were detected in the CD8-positive, 9.3-negative subset. This subset had previously been shown to comprise all the alloantigen-specific and histamine-inducible suppressor T-cells. Other T-subsets, even those showing either CD8-positivity (but 9.3-positivity) or 9.3-negativity (but CD8-negativity), were, as a rule, S-100 negative. Immunoelectronmicroscopy confirmed that the S-100-positive cells, showing peroxidase activity within the cytoplasm, were found exclusively within the CD8-positive, 9.3-negative subset. This finding of S-100 protein in cells of a specific T8 suppressor subset extends the range of the known distribution of this protein and may have important implications concerning its role in the modulation of immune responses.

Serum free thyroid hormones in different degrees of hypothyroidism and in euthyroid autoimmune thyroiditis.

Serum total and free T4 and T3, thyroxine-binding globulin (TBG) and TSH, basal and 20, 30 and 60 min after TRH (200 micrograms, iv), were evaluated in 125 hypothyroid patients (38 with severe, 23 with mild, and 64 with subclinical hypothyroidism), in 35 euthyroid subjects with autoimmune thyroiditis, and in 51 healthy controls. T4/TBG and T3/TBG ratios were also calculated. A significant decrease in all indices of thyroid function except for T3 occurred simultaneously with a significant increase in basal and TRH-stimulated TSH levels from healthy subjects to subclinical hypothyroids, from subclinical to mild and from mild to severe hypothyroids; euthyroid patients with autoimmune thyroiditis did not differ from healthy subjects. All severe hypothyroid patients had low T4 as well as free T4 (FT4), free T3 (FT3), T4/TBG and T3/TBG ratios, but among mild and subclinical hypothyroids direct determination of FT4 and FT3 proved to be a better index of thyroid function than determination of T4 and T3 even after correction for TBG levels. FT4 was the most commonly abnormal index (19 of 23 subjects with mild and 14 of 64 with subclinical disease). Regression analysis showed that FT4, T4/TBG ratio, T4, and FT3 had a significant inverse correlation with TSH in hypothyroid patients. Discriminant analysis showed that among the thyroid parameters, FT4 is the variable which discriminates best between control subjects and the 3 groups of hypothyroid patients. These data extend previous reports and in a large series of patients confirm the biological meaning and the clinical value of direct measurement of serum free thyroid hormones in hypothyroidism.(ABSTRACT TRUNCATED AT 250 WORDS)

Advantage of desogestrel containing pill in oral contraception: influence on blood lipids and LCAT activity.

In the present study the effects of a combined oral contraceptive preparation containing 0.150 mg desogestrel and 0.030 mg ethinylestradiol on lipid metabolism were investigated. In particular, we observed significant increase in HDL-cholesterol and apolipoprotein-A-I (apo A-I) and B (apo B). Triglycerides were not significantly modified. The cholesterol esterifying enzyme LCAT, assayed under "maximal" conditions against an exogenous substrate, was significantly decreased despite an increase in the physiological stimulator apo A-I. No changes were observed in the anti-atherogenic indexes apo A-I/apo B and HDL-cholesterol/LDL-cholesterol. Thus, it appears that this combined oral contraceptive has the promising ability to increase the anti-atherogenic HDL-cholesterol particle without altering the atherogenic LDL-cholesterol.

Renal cell carcinoma and transitional cell carcinomas of the pelvis and bladder in a patient affected by chronic renal failure due to abuse of phenacetin.

This is a report on a case of 2 concurrent neoplasms of different histology within the same kidney: a renal cell carcinoma and a transitional cell carcinoma of the renal pelvis in a patient affected by chronic renal failure due to abuse of phenacetin. There was also a transitional cell carcinoma of the urinary bladder.

Ultrastructural characterization of fundic endocrine cell hyperplasia associated with atrophic gastritis and hypergastrinaemia.

Clinical and experimental evidence indicates that carcinoid tumours of the stomach fundic mucosa represent another example of hormone-dependent neoplasm, gastrin being the hormone involved in tumour induction. In this context hyperplasia of fundic endocrine cells associated with chronic atrophic gastritis (CAG) and hypergastrinaemia is regarded as the most frequent preneoplastic lesion. However, the cell type involved in this hyperplasia has not been clarified. To elucidate this problem fundic endocrine cells were characterized ultrastructurally in 9 patients from which endoscopic gastric biopsies were obtained. ECL cells were the most frequent cell type in 8 cases, in 4 of which they were more numerous than all other cell types taken together. D1 cells were the most frequent type in one case while they were inconspicuous in the other cases. P cells were found with a frequency in each case intermediate between that of ECL cells and that of D1 cells. These results indicate that fundic endocrine cell hyperplasia occurring in hypergastrinaemic CAG is in most cases cytologically similar to that found in other hypergastrinemic conditions, in which the gastrin-dependent ECL cells were already found to prevail. They also explain why fundic carcinoids arising in CAG are mostly composed of ECL cells. The relation between ECL, D1 and P cells, if any, remains obscure.

Ovarian failure without gonadotropin elevation in a patient with post-traumatic isolated hypogonadotropic hypogonadism.

A woman with secondary amenorrhea following head trauma showing isolated gonadotropin deficiency of hypothalamic origin did not respond to massive doses of hMG therapy (11 250 IU daily dose). Ovarian biopsy showed the absence of follicles, despite persistently low FSH levels. In this case the occurrence of premature ovarian failure was only suspected from the lack of response to hMG and diagnosed by ovarian histology.

Morphometric characterization of NK cell subset expressing the Leu-11 antigen in comparison to Leu-7 positive, 11 negative cells.

Three subpopulations of human natural killer (NK) cells were identified by immunoelectron microscopy, using combinations of anti-Leu-7 and anti-Leu-11 monoclonal antibodies. For each subpopulation the nuclear area/cellular area ratio (An/Ac) and the perimeter/equivalent circumference ratio were evaluated employing an interactive image analyzer. Leu-11+ cells showed a larger area, a smaller An/Ac and a higher "villousity degree" in comparison to Leu-7+, 11- cells. These differences were proved to be significant using the Kolmogorov-Smirnov two sample test. Previous studies described the existence of distinct cytotoxic capability, recombinant interleukin 2-mediated activation, and ultrastructural features of Leu-11+ in comparison to Leu-7+, 11- cells. This is the first report in which morphometric differences within NK cell subsets are exactly determined.

Clonal analysis of intrahepatic T lymphocytes in chronic active hepatitis. Isolation of a T-cell line specific for hepatitis B core antigen from a patient with serological evidence of exposure to HBV.

We have evaluated whether peripheral blood and hepatic lymphocytes from a patient with chronic active hepatitis (CAH) and antibodies to HBV in serum were specifically sensitized to HBV envelope antigens (HBsAg and pre-S Ag) or to HBcAg. No proliferation to HBV antigens was demonstrated upon stimulation of peripheral blood mononuclear cells either unfractionated or enriched in CD4+ (helper/inducer) T cells. Of 15 T-cell cloned lines (7 CD8+ and 8 CD4+) obtained by limiting dilution in the presence of PHA and recombinant IL2 from liver-infiltrating lymphocytes, one, designated H2, showed specific sensitization to HBcAg, whereas none demonstrated sensitization to viral envelope antigens. The H2 line displayed the CD8+ phenotype, suppressor activity on polyclonal immunoglobulin production and IL2-dependent, HBcAg-specific proliferation. These results suggest that in patients with CAH and serological evidence of previous exposure to HBV, it is possible to obtain lymphocytes specifically sensitized to HBcAg from liver biopsy.

Medical treatment of hyperprolactinaemic disorders.

The current status of treatments designed to treat prolactinoma and other hyperprolactinaemic disorders is reviewed. The use of ergoline derivatives is described to correct prolactin levels, restore reproductive dysfunctions and reduce the size of prolactinomas. Side effects are minimal but withdrawal is almost always followed by a recurrence of the original condition. Radiation therapy is less effective and surgical resection of prolactinomas is effective, but the condition may recur. The authors recommend that dopaminergic drugs should be the primary therapies for prolactin-secreting adenomas and idiopathic hyperprolactinaemia, and surgery should be reserved for dopamine-resistant conditions.

Langerhans cells are not damaged in contact allergic reactions in humans.

Fifteen biopsies from contact allergic reactions in sensitized patients were taken at different time intervals after application of simple chemicals. Two different procedures for electron microscopy were done. No signs of damage were ever seen in Langerhans cells. The authors are therefore of the opinion that there is still no proof for a target role of Langerhans cells in contact allergic reactions in humans.

An in situ immunogold method applied to the identification of plasma membrane-associated antigens of skin-infiltrating cells.

This report concerns a pre-embedding in situ double-step immunogold technique to detect cell surface antigens of skin infiltrating cells. Reliable immunogold labeling of cell surface antigens, as well as good ultrastructural preservation of cell and tissue details, were achieved. In comparison with the commonly used method of in situ immunoperoxidase labeling, the present in situ immunogold labeling has some advantages, such as higher specificity, higher sensitivity, and the possibility of estimating the density of the labeling. However, a drawback in the present technique is the compulsory high-magnification analysis of the specimens at the electron microscope, due to the small size of the gold particles.

Immunogold labelling of epidermal Langerhans cells on tissue sections of normal human skin.

A colloidal gold staining procedure was developed for immuno-electron microscopy, which allowed the demonstration of T6 surface antigens of epidermal Langerhans cells on sections of normal human skin with consistent ultrastructural preservation of subcellular structures. All the observed Birbeck granule-bearing Langerhans cells revealed immunogold labelling along the plasma membrane. The penetration of the tissue by the gold particles was adequate, as was the sensitivity and the specificity of the reaction. A major advantage of the present method in comparison with immunoperoxidase techniques is the absence of non-specific staining of the membranes of keratinocytes facing the labelled plasma membrane of the Langerhans cells.

Characterization of two morphologically distinct Leu-7+ cell subsets with respect to Leu-15 antigen. Evaluation of Leu-15 determinant distribution on both E rosetting and non-adherent non-E rosetting cell populations.

In the present study the fine structures of Leu-7+-Leu-15+ and Leu-7+-Leu-15- cell subpopulations were characterized by using an immunogold-immunoperoxidase double labelling in electron microscopy. The densities of Leu-15 antigenic sites on both E rosetting (Er) and non-adherent/non-E rosetting (non-A/non-Er) Leu-15 positive cell surfaces were also evaluated by using an immunogold analysis in electron microscopy. A majority of Leu-7+ cells co-expressed the Leu-15 antigen and showed an ultrastructural pattern specific for mature natural killer (NK) cells, i.e. abundant cytoplasm with many organelles, numerous electron dense granules, and irregular outline. On the other hand, a minority of Leu-7+ cells did not express the Leu-15 antigen and showed a clearly different ultrastructural feature in comparison to Leu-7+-Leu-15+ cells. Thus, the presence of the Leu-15 antigen on Leu-7+ cell surface corresponds to ultrastructural features specific to differentiated NK cells and may represent an expression of Leu-7+ cell differentiation. An alternative hypothesis may be that Leu-7+-Leu-15+ and Leu-7+-Leu-15- cells represent distinct cell lineages within non-A/non-Er Leu-7+ cells. Finally, the results of the present study provide proof that Leu-15 antigen is more frequently represented on non-A/non-Er Leu-15+ cells than on Er Leu-15+ cells.
