Audiovisual demonstration for patient counselling in the use of pressurised aerosol bronchodilator inhalers.
The use of video as a method of instructing subjects on correct inhaler technique was compared with the written and personal instruction of 150 volunteer subjects with no previous experience of such a device. Video was found to be as effective as personal tuition by a pharmacist and significantly (P less than 0.05) superior to the manufacturer's package insert in teaching correct inhaler technique. The technique was assessed by a panel of five investigators in each case and was found to deteriorate over a 2-week period regardless of the instruction method. The video, which was prepared 'in-house', was then assessed in both the hospital out-patient and the general practice pharmacy settings. Asthmatic patients found the video useful at identifying mistakes they were making and felt that this approach to their education should be more widely available.
Atopy in subjects with a history of nickel allergy but negative patch tests.
Various markers of atopy were studied in 20 subjects with a history of nickel allergy but negative patch test reactions, and compared with 2 control groups: 13 subjects with no history of nickel allergy and negative patch tests; 11 subjects with a history of nickel allergy and positive patch tests. No significant difference in the incidence of atopy was detected in the 3 groups.
Effect of flosequinan on exercise capacity and symptoms in severe heart failure.
Twenty patients with severe chronic cardiac failure caused by ischaemic heart disease were treated with flosequinan 100 mg daily or placebo in addition to their existing treatment with diuretics and, in some, digoxin in a randomised double blind trial. After eight weeks of treatment, flosequinan significantly improved treadmill exercise time, increased peak achieved oxygen consumption, and improved the New York Heart Association symptom grade when compared with placebo. One patient in the placebo group died and another was withdrawn because heart failure worsened. One patient in the flosequinan group was lost to follow up but there were no other withdrawals. Flosequinan was well tolerated with few adverse effects, and it may prove to be a useful addition to diuretics and digoxin in the treatment of chronic cardiac failure.

Irritant contact facial dermatitis due to nebulizer therapy.
A case is reported of a patient with long-standing, severe airflow obstruction requiring long-term nebulizer therapy developing a facial dermatitis in the area bounded by the nebulizer mask. The facial dermatitis seems to be the result of the combined irritancy of the nebulizer solutions and moisture, and prophylactic measures are suggested for patients requiring long-term nebulizer therapy.

Rifampicin does not suppress the tuberculin reaction in man.
Despite previous findings to the contrary in uncontrolled studies, we have found in a randomized trial that rifampicin does not suppress the tuberculin reaction in patients with tuberculosis.

Seborrhoeic eczema--a disease entity or a clinical variant of atopic eczema?
One hundred and thirty-eight children seen at the Dermatology Clinic between 1969 and 1972 were reviewed. The study comprised two groups--a study group of 76 children diagnosed as having seborrhoeic eczema, and a group of 62 children seen over the same time period at the same clinic and diagnosed as having atopic eczema. The children were examined and a family history obtained with particular reference to past or present history of atopic diseases. Serum IgE, blood eosinophil count, pulmonary function tests and skin prick testing to various allergens were measured in the two groups. In the seborrhoeic group, 19% had eczema, 20% had abnormal pulmonary function clinically; 25% had a raised eosinophil count, 44% a raised IgE level, 89% positive skin prick testing, and 52% a peak flow less than 80% of predicted value. These results indicate a closer than expected association between infantile seborrhoeic eczema and atopic disease.
Respiratory disability in ex-flax workers.
Acute respiratory effects occur in a high proportion of subjects exposed to textile dusts. The extent to which these lead to permanent respiratory symptoms and loss of lung function is unknown. A survey of random population samples was therefore conducted in ten towns in Northern Ireland in which flax processing had been a major source of employment. The MRC questionnaire on respiratory symptoms was administered and Vitalograph tracings recorded on subjects aged 40 to 74 inclusive. An occupational history was taken at the end of each interview. Lung function in ex-flax workers was slightly lower than in control subjects never exposed to flax dust, but the presence of a positive interaction with age meant that differences were apparent only in the younger subjects. Over about the age of 65 the lung function in the ex-flax workers was comparable with that of the controls and overall the loss was at most about half that due to light smoking (1-14 cigarettes a day). The association between a "dust exposure score" and lung function was inconsistent in the two sexes. In men there was a small decrement with increasing dust exposure. In women there was also a small decrement, but a positive interaction with age meant that the women with the highest dust exposure scores had a lower loss with increasing age than the women with the least dust exposure. There was an excess in symptoms in the ex-flax workers but the size of the excess was greater than would be expected from the lung function results. It is possible that, although the survey was conducted without explicit reference to the flax industry, knowledge throughout Northern Ireland that many flax workers have been awarded compensation on the grounds of respiratory disablement may have led to an increased reporting of symptoms in the ex-flax workers.

Anticoagulants in the treatment of small cell carcinoma of the bronchus.
The object of this study was to see if the addition of anticoagulants to a regimen of cytotoxic drugs would improve the prognosis in patients with small cell carcinoma of the bronchus. Twenty-four patients were randomly allocated to receive chemotherapy or chemotherapy plus anticoagulants. The median survival in the group receiving the anticoagulants was not improved.
