Contrast sensitivity in age-related macular degeneration.

Low-contrast charts were used to investigate the possibility that patients with drusen have visual deficits not detected by standard Snellen charts. We compared performance on Regan letter charts between 52 eyes with drusen and Snellen acuity of 20/20 and 27 control eyes. The drusen group read fewer letters than the control group on all of the charts tested. This difference increased as the contrast of the charts decreased. The loss of performance on all of the Regan charts correlated with drusen severity. Twenty-one eyes with drusen and normal Snellen acuity also were tested with a Ginsburg contrast sensitivity chart and compared with age-matched normal controls. The results showed a loss of contrast sensitivity at high spatial frequencies and a loss of peak contrast sensitivity with increasing drusen severity. These results suggest that in patients with drusen, low-contrast charts may be useful for measuring visual loss not detected by standard Snellen charts.

Hospitalization requirements after vitreoretinal surgery.

Two hundred patients undergoing vitreoretinal surgery were prospectively studied to determine the postoperative hospitalization requirements and to analyze preoperative and intraoperative factors that influence the need for inpatient care. Fifty-two percent of the patients had postoperative events that were best treated in an inpatient setting. Forty-four patients (22%) had pain requiring intramuscular injections five or more hours following surgery, 35 patients (18%) had nausea requiring intramuscular medications five or more hours following surgery, 30 patients (15%) required physician consultation for medical illness five hours or more following surgery, and 41 patients (20%) required more than one postoperative day of hospitalization because of ocular abnormalities. We concluded that although vitreoretinal surgery can sometimes be performed on an ambulatory basis, most cases require hospitalization for optimum care.

Subfoveal neovascularization in the ocular histoplasmosis syndrome. A natural history study.

The visual outcome of 74 eyes with the ocular histoplasmosis syndrome and active subfoveal subretinal neovascular membranes was studied retrospectively. Follow-up time ranged from 12 to 109 months (median, 36.5 months). Ten eyes (14%) retained visual acuity of 20/40 or better. Fifty-seven eyes (77%) suffered visual loss of 20/100 or worse and 36 eyes (49%) suffered visual loss of 20/400 or worse. Factors significantly associated with retaining vision of 20/40 or better were: age less than 30 years (P = 0.008); smaller membrane size (P = 0.0002); and absence of visual loss secondary to ocular histoplasmosis in the fellow eye (P = 0.053). Factors significantly associated with vision decreasing by four or more lines were: older patient age (P = 0.006); better initial visual acuity (P = 0.090); and more than 50% involvement of the foveal avascular zone (P = 0.059). These results were confirmed by multiple logistic regression. Final vision of 20/40 or better was associated with better initial visual acuity using univariate analysis, but this result was not confirmed by multiple logistic regression.

Multivariate analysis of factors associated with unplanned admission in 'outpatient' ophthalmic surgery.

We performed a case-control study of patients undergoing planned "outpatient" ophthalmic surgery in order to identify risk factors associated with the need for unplanned admission to the hospital. factors associated with admission included time of completion of surgery, duration of operation, fentanyl dose, use of enflurane, and leukocytosis. A multiple logistic regression model found each of these variables except duration of operation to to be independently associated with unplanned admission with admission included age, present or past medical conditions, use of medications, blood pressure, hematocrit, serum sodium or potassium, and ASA Class. Identification and modification of these factors may help anticipate and/or reduced the incidence of unplanned admission to the hospital in patients undergoing planned outpatient surgery.

The effects of D-penicillamine and daunorubicin on conjunctival fibroblast proliferation and collagen synthesis.

Two drugs, D-penicillamine and daunorubicin, were tested for their effect on proliferation and collagen synthesis of cultured conjunctival fibroblasts. This cell type is likely responsible for scar formation and ultimate filter surgery failure in glaucoma patients. Both drugs were antiproliferative; however, D-penicillamine required 2000 times the concentration of daunorubicin to achieve a similar degree of inhibition. D-penicillamine had a more consistent effect on intracellular collagen synthesis than daunorubicin at the doses tested. In contrast, at concentrations of daunorubicin where all proliferation ceased, intracellular production and extracellular transport of collagen were maintained.

Screening for glaucomatous visual field loss with automated threshold perimetry.

We examined 27 glaucomatous eyes and 154 normal eyes with automated threshold perimetry. Previously suggested algorithms for detecting mild to moderate glaucomatous visual field loss, primarily by comparing the sensitivity of corresponding clusters of points above and below the horizontal meridian, yielded results remarkably consistent with those of our earlier series: 93% (25 of 27) of abnormal and 88% (135 of 154) of normal eyes were correctly identified. Excluding unreliable examinations increased specificity from 88% to 93%. Test duration was similar to results with the Field Analyzer's older, purportedly slower operating system.

Effects of ALO 2145 on intraocular pressure following argon laser trabeculoplasty.

A prospective, randomized, double-masked study compared topical 1% ALO 2145, an alpha 2-agonist, with placebo in therapy for immediate postoperative intraocular pressure (IOP) rise after argon laser trabeculoplasty. Seventy-three eyes (73 patients) underwent 360 degrees of treatment utilizing 80 spots of 800 to 1000 mW of power. Intraocular pressure rise was measured hourly for the first three hours after operation, at one week, and at one month. Eyes treated with ALO 2145 had both significantly lower mean IOPs and greater IOP decreases from baseline than placebo-treated eyes during the first three hours after operation. No eyes treated with ALO 2145 and six eyes (18%) treated with placebo experienced an IOP rise of 10 mm Hg or greater. Twenty eyes (59%) in the placebo group and eight eyes (21%) treated with ALO 2145 had an IOP elevation. No change was detected in the mean heart rate. ALO 2145 appears to be effective in eliminating large, acute IOP elevations after argon laser trabeculoplasty.

Recognizing glaucomatous field loss with the Humphrey STATPAC.

The results generated by the STATPAC program of the Humphrey Field Analyzer (Allergan Humphrey, San Leandro, Calif) from 72 eyes with documented early visual field loss and 98 normal eyes were examined. When global summary indexes and their significance levels were used to compare the tested eyes with age-matched normal values, STATPAC yielded a sensitivity of 93% and a specificity of 84%. In comparison, analysis by an independent, previously reported method yielded a sensitivity of 97% and a specificity of 90%. STATPAC results were strongly influenced by the reliability of the tested subjects.
