Clinical and neurobiological findings in children suffering from tic disease following treatment with tiapride.

Tiapride, a substituted benzamide derivative, possesses good clinical antidyskinetic properties due to its DA2-blocking activities. It has been shown to be clinically effective in the treatment of tic disease in children. In order to study tiapride's antidyskinetic properties in the treatment of the tic syndrome in children, we conducted a simple, placebo-controlled study on 10 children followed by a double-blind crossover study on 17 children. Tiapride was shown to have a positive therapeutic effect on tics in children; whereas it has no adverse effects on neuropsychologically measurable cognitive performances in children. Neurophysiological parameters such as the EEG frequency analysis and sensory evoked potentials were not affected by tiapride, nor was the neurosecretory, hypothalamic-hypophyseal regulation of the sex hormones, thyroid stimulating hormone, growth hormone, or thyroid hormone impaired. The hyperprolactinemia caused by tiapride's dopaminergic properties was moderate and restricted to the duration of therapy.

[Pediatric depression].

In 12 children between 6 and 12 years of age who were treated as inpatients for depression (diagnosed according to the Weinberg-criteria, a child-adapted modification of DSM-III-criteria), a close relationship was found between family pathology, psychodynamics and depression. The conflicts in the interactions between the depressed children and their caregivers became evident in the children's drawings, in the Scenotest and in play therapy. In play therapy the repressed feelings of powerlessness, helplessness, disappointment, resignation and anger came to light. The children had a pseudo-stabilizing function in the family that placed too heavy demands on them, with the result that they became dependent and helpless and tended to despair. A situation developed that can be characterized as "learned helplessness" and that is a useful behavioral-physiological and neurobiological model of depression for different age groups.

[Autologous spongiosaplasty in the treatment of chronic osteomyelitis].

The most promising method of filling up osteomyelitic bone deficiencies is the transplantation of fresh autologous spongiosa as soon as the infection is under control. The fundamental principles of surgical therapy of septic focuses must not be neglected. A spongiosa graft into the florid infection has only little chances of success. The stability of the deficiency is decisive for the success of the transplantation. In case of infection it is necessary as far as possible to choose a point remote from the septic focus for the grafting of the osteosynthetic material.

[Regression trends of neurologic damage in the surgical emergency management of patients with injuries of the cervical vertebrae with spinal cord involvement].

Sixty-five patients with cervical spine injuries and varied neurological deficits were treated operatively. Evaluation revealed an improvement in neurological findings dependent upon the promptness of anatomical reduction in patients with incomplete lesions. The more frequent neurological improvement seen with open reduction and internal fixation as compared with closed reduction was not statistically significant but was felt to justify the additional resources required for internal fixation. In complete lesions, there was no evidence that the time of anatomical reduction was related to improvement in neurological findings.
