[Stenosing hypertrophic antromyopathy].

The authors describe a case of insufficiency of gastric evacuation, due to the a very rare injury of the antrum which they called:--"hypertrophic and stenosing antromiopathiae". By similarity with the pilor hypertrophy, but without altering the pilor which is intact, the hypertrophy involves especially the gastric antrum, the muscular tunic--mainly the circular fibres in the absence of any acute or chronic inflammatory process. In the interstice between the muscular fibres, there appear collagen fibres in spiral disposition, probably in a contraction. Due to the similitude of the clinical syndrome, the described disease could be ranged among the stenosant diseases of the pyloric pole of the stomach. Out of these, more frequently pointed out, although very rare too, seems to be the pilor hypertrophy at adults. Mention must be made that the two diseases are completely different, because in the "Hypertrophic and stenosing antromiopathiae", the pilor is macroscopic and at the same time normal from the histopathologic aspect point of view.

[Splenectomy in chronic hepatitis with an autoimmune component].

The authors present a synthesis of studies performed on the role of the spleen in chronic active-digestive hepatitis with auto-immune participation. The study was based on a group of 48 patients out of which a lot of 20 patients was selected, with histological diagnosis of chronic active and aggressive hepatitis, in whom the presence of AgAu was ascertained, as well as of antiliver antibodies. From the immunological viewpoint, the effect of splenectomy was manifested, by a rapid and significant decrease of anti-liver antibodies mainly produced by the spleen, immediately following the intervention. In most of the patients the IgM antibodies disappeared somewhat later. At the same time the chronic hepatitis lesions were stabilized, the biological signs were improved and the clinical evolution of the patients was good. The authors consider that splenectomy in this category of hepatopathies is an immuno-suppressive therapy that may arrest the evolution towards cirrhosis.

[Gastric localization of Crohn's disease].

The authors present a case of Crohn disease with gastric location. The lesion was complicated by a blind fistula in the body of the pancreas. Surgery, which consisted in gastrectomy, solved the case for the time being, and the patient is followed up.

[Current problems of surgical tactics and technics in gastric neoplasms].

The authors have investigated 413 cases with gastric neoplasms that have been operated over a period of 20 years. It was noted that the operability index, and the resectability index have not improved in time. Only in 39,3 percent of the patients subtotal gastrectomy was possible, while 12,1 percent underwent total gastrectomy, 6,2 percent palliative interventions, and the remaining 42,4 percent had only exploratory laparotomy. The authors conclude that the principle of exeresis within the limits of oncological security applies only to limited tumours, without extraparietal extension. The maintenance of a gastric sector, either in the para-cardial or in the juxta-pyloric area, depending on the location of the tumour, is almost always possible, providing a good-quality anastomosis without diminishing the oncologic characteristics of the intervention.

[Splenic hemangiosarcoma].

The authors present a case of splenic hemangiosarcoma which was encounter-a group including some 600 splenectomies. Hemangiosarcomas represent only 3% of all malignant splenic tumours. This clinical observation is of particular interest because of the extreme rarity of such cases. It is also interesting because of the conditions which have determined the intervention, as well as by the prolonged survival of the patient.

[Gastric neoplasm associated with pancreatic cystadenoma].

A case of juxtacardial gastric neoplasm, associated with pancreatic cystadenoma is reported. This rare, benign cystic formation of the pancreas is sometimes associated with synchronous or metachronous neoplasia of the gastrointestinal tract. Simple enucleation of the cystadenomas was followed by an uneventful clinical evolution.

Adenocarcinoma
