[Analysis of complications of thyroid surgery: recurrent paralysis et hypoparathyroidism. On a series of 588 cases].

The purpose of this report is to study the incidence of two main complications in thyroid surgery without systematic search for recurrent laryngeal nerve, by intracapsular thyroidectomy: temporary and chronic laryngeal recurrent paralysis (after six months), temporary hypoparathyroidism and chronic hypoparathyroidism (unrecovering normal function after six months). We have retrospectively analysed 588 patients from 1981 to 1994. There were 247 total or subtotal thyroidectomies and 341 loboisthmectomies (isolated thyroid nodules). We obtained over this 588 cases 0.3% of chronic laryngeal paralysis and 1.4% of chronic hypoparathyroidism. If the cost of surgery for thyroid nodules is low (0.3% of chronic recurrent paralysis and 0% of chronic hypoparathyroidism), it is more important for total or subtotal thyroidectomies (respectively 1.2% and 3.2%). We conclude that in thyroid surgery, there is no higher risk for the recurrent nerve without dissection of this one, but we note that, without obvious explanation, rates of permanent hypoparathyroidism are not better than in others publications.

[Congenital cholesteatoma and associated ossicular malformations].

15 cases of congenital cholesteatoma surgically treated from 1982 to 1992 are presented. Clinical diagnosis criteria, pathology, localisations and extensions to the middle ear are analyzed. Ossicular malformations were associated in one third of the patients of this series. These ossicular malformations underscore the congenital origin of such cholesteatoma. A review of the literature and pathogenesis of congenital cholesteatoma is discussed.

[Thrombophlebitis of the lateral sinus. Apropos of a case].

About one case of septic sigmoid sinus thrombosis, the authors present various clinical and paraclinical elements of this rare pathology, whose entity are generally subtle for benign but also for serious forms. Although uncomplicated dural venous thrombosis carries an excellent prognosis with proper treatment, delay in diagnosis and institution of appropriate therapy may lead to serious, or even fatal, consequences. The modern literature is reviewed and the pathology, presentation and management of lateral sinus pathology is discussed.

[Correlations between anatomy and x-ray computed tomography in cancers of the pharyngo-larynx. Apropos of a prospective study of 35 patients].

A prospective study has been conducted in 35 cases of pharyngolaryngeal's carcinoma, between december 1990 and june 1992 in the department of Head and Neck Surgery of Strasbourg. The authors compared, in a statistic analysis, Computed Tomography and histopathologic's correlations. The operative specimen were sectioned in the horizontal plane to correspond with the CT sections. 11 structures were studied more particularly. Results revealed an excellent sensibility for the study of the pre-epiglottic spaces (100%). Inversely, the study of the para glottic space is less good (69% of sensibility). We found that the CT Scan was of no value for the study of the thyroid cartilage, particularly for detection of subclinical lesions. Overall, this study allowed to confirm the value of the CT Scan in the investigation of pharyngolaryngeal tumors but its limits also.

Postlaryngectomy voice restoration. A prospective study in 83 patients.

From July 1985 until May 1990, 83 patients underwent a total laryngectomy. We prospectively studied voice restoration in 81 of them (two died postoperatively). Esophageal voice was used by 19 patients; a tracheoesophageal procedure (myomucosal shunt, primary or secondary puncture with Blom-Singer prosthesis) by 41; and 21 patients had no voice restoration. Results were assessed according to voice quality and usage. Tracheoesophageal speech had a success rate of 73% (good voice, daily use) after 1 month, while esophageal voice proved to have only a 5% success rate. Thirty patients (37%), however, remained without a substitute voice. The reasons for their exclusion are presented; they include a high rate of refusal.

[Neuroendocrine giant-cell epithelioma. Description of a clinical case and review of the literature].

Neuroendocrine tumors are rare neoplasms of the larynx and hypopharynx. This tumors are divided in paragangliomas (usually benign, although malignant case have been reported), large cell neuroendocrine carcinoma, described in this case as a primary pharyngo-laryngeal locate, and small cell neuroendocrine carcinoma (characterized by early and diffuse metastatic disease, and best treated by radiotherapy and chemotherapy). Through the international literature, the authors reviewed the characteristics of the large cell neuroendocrine tumors and their management.

[Laryngeal sarcoidosis: apropos of a case].

The authors report an unusual case of laryngeal sarcoidosis. After having explained it, they describe the elements of diagnosis about this rarely isolated pathology, its complications and treatment as well.

[Sudden deafness. Analysis of a series of 117 cases].

A retrospective study have been conducted in 117 cases of sudden sensori-neural hearing loss, between 1988 and 1991, int he department of Head and Neck Surgery (Strasbourg). After a brief review of actually treatments, they show, in a statistic and comparative analysis, that efficacy of therapeutics are never significant. This results are compared with international bibliography, and show that treatments of sudden loss remains doubtful.

[Difficult intubations. A prospective study].

A prospective tubing of tracheal tubing in view of general anesthia was performed in 441 adults undergoing scheduled surgery. Before surgery, the anesthesists and ENT specialists tried to independently predict the cases of difficult tubing, according to different criteria. Tubing actually was difficult in 38 patients (8.6ù) instead of the 21 (55.2%) expected by the ENT specialists and/or the anesthesists. These difficulties were solved by bronchoscopy, which allowed tubing to be performed with a guide-probe (3 cas), by scheduled fiberendoscopy (6), tracheotomy (1), nasotracheal tubing (1), spontaneous ventilation (2), and orotracheal tubing in 25 cases. Current knowledge of the predictive criteria is incomplete: anesthesists must be aware of the assistance techniques required in case of difficult tubing in a non-ENT context.

[Use of madreporic material in ear surgery. An initial study].

Numerous surgical techniques now call for biomaterials. Included in this term is material foreign to the receiver, of natural or artificial origin, which is implanted in a living organism in order to repair the impaired function of the organ. By using a recent material, coral, the authors here by relate their experience within the scope of ear surgery. The analysis of the preliminary results encourages the further use of this material, bio-compatible, easily-used and giving excellent functional results.

[Cancer of the maxillary sinus. A retrospective study of 29 cases].

Representing 2% of malignant tumors and 6% of head and neck cancer, almost always diagnosed at a late stage, maxillary sinus cancer still remains a rather rare pathology but fearsome prognosis. From the 29 cases, over 10 years of follow up at the clinic and Paul Strauss cancer treatment center, this study focuses mainly on two criteria: diagnosis, in the study of the presenting symptoms in order to treat this pathology as soon as possible; therapeutic, by analysing our results, and by focusing on the current treatment.
