Comparison of efficacy of ginger with various antimotion sickness drugs.

UNLABELLED: Ginger and several other medications were compared with scopolamine and d-amphetamine for effectiveness in prevention of motion sickness. METHODS: Double-blind techniques were used. The subjects were given the medications two hours before they were rotated in a chair making head movements until a symptom total short of vomiting was reached. Standardized N.A.S.A. techniques were used for speed of rotation and end-point of motion sickness. RESULTS: The three doses of ginger were all at the placebo level of efficacy. Amitriptyline, ethopropazine and trihexyphenidyl increased the tolerated head movements but the increase was not statistically significant. Significant levels of protection were produced by dimenhydrinate, promethazine, scopolamine and d-amphetamine. Protection was further increased by combination of these latter drugs with d-amphetamine. Efficacy was greatest as the dose was increased. CONCLUSIONS: The medication of choice in this study was scopolamine 0.6 mg with d-amphetamine 10 mg. This combination provided good protection with acceptable side effects.

Nuclear medicine evaluation of motion sickness and medications on gastric emptying time.

Diminished gastric motility and lack of bowel sounds have been observed in astronauts aboard the Space Shuttle (4). In this study subjects were given scopolamine 0.6 mg with d-amphetamine 5 mg with and without neostigmine 15 mg. Neostigmine 15 mg alone was also compared with placebo for effect on gastric emptying time. In an additional test, subjects performed head movements in a rotating chair to an end-point of motion sickness short of vomiting. Ten ounces of isotonic saline containing 1 mCl of Tc 99mDPTA was ingested 2 h after the medications and immediately after rotation. The counts from stomach contents were monitored with a Picker small field of view gamma camera every 30 s for 1 h. Gastric motility was inhibited by scopolamine and amphetamine with 14% residual count at the end of 1 h. When neostigmine was added to this combination the results were in the placebo range. Motion sickness produced a profound inhibition of gastric emptying with a 47% residual count. The results indicate that the gastric stasis encountered in space is due mainly to motion sickness with a minimal contribution from the antimotion sickness drugs.

Mechanisms of antimotion sickness drugs.

UNLABELLED: Eight subjects, male and female, were rotated using the step method to progressively increase the speed of rotation (+2 rpm) after every 40 head movements to a maximum of 35 rpm. The end-point for motion sickness was the Graybiel Malaise III total of symptoms short of frank nausea. The drug treatments were placebo, scopolamine 0.6 mg and 1 mg, scopolamine 0.6 mg/d-amphetamine 10 mg, scopolamine 1 mg/d-amphetamine 10 mg and amphetamine 10 mg. RESULTS: Scopolamine increased tolerated head movements over placebo level by +81, scopolamine 1 mg + 183, d-amphetamine + 118, scopolamine 0.6/d-amphetamine + 165, and scopolamine 1 mg/d-amphetamine 10 mg + 201. DISCUSSION: The drugs effective in preventing motion sickness are divided into those with central acetylcholine blocking activity and those which enhance norepinephrine activity. A combination of both of these actions produces the most effective antimotion sickness medications. CONCLUSIONS: The balance between the acetylcholine and norepinephrine activity in the CNS appears to be responsible for motion sickness.

The effect of antimotion sickness drugs on habituation to motion.

The effect of antimotion sickness drugs on habituation was studied. Subjects were rotated once a day for 5 d to the malaise III end-point after receiving placebo, 1 mg scopolamine, 10 mg d-amphetamine, or the combination of 0.6 mg scopolamine with 5 mg of d-amphetamine. The placebo scores had a Spearman coefficient of correlation of 0.88 with the initial untreated tests. This demonstrated a high reliability for the M-III end-point and that little habituation resulted from the test design. The combination of 0.6 scopolamine with 5 mg amphetamine produced the fastest rate of habituation closely followed by the dose of 1 mg scopolamine. 10 mg of d-amphetamine also produced an increase in habituation over placebo scores. When the medications were discontinued on day 5 a rebound in sensitivity to vestibular stimulation occurred with scopolamine and scopolamine with d-amphetamine. The increased habituation appears to be due to the greater exposure to vestibular stimulation permitted by the medications.

Role of vein patch angioplasty in isolated operations for profunda femoris stenosis and disabling claudication.

This report has described the treatment of disabling claudication by correction of profunda femoris artery stenosis. Vein patch angioplasty and endarterectomy with primary closure were the procedures used. An overall success rate of 71 percent was noted with no significant complications, however, the angioplasty group had a higher likelihood of success (83 percent versus 40 percent). They also were noted to have a significantly improved ankle-arm Doppler index, with a mean follow-up of 21 months. Possible explanations for these observations have been considered and technical recommendations discussed.

Immunoglobulin M and immunoglobulin G secretion by human B cells exposed to RU 41.740, a glycoprotein extract from Klebsiella pneumoniae.

RU 41.740, a glycoprotein extract from Klebsiella pneumoniae, was seen to activate human B cells to immunoglobulin secretion in vitro. The effects of RU 41.740 on human B cells were compared to those induced by pokeweed mitogen, a T-cell-dependent polyclonal B-cell activator, and Epstein-Barr virus, a T-cell-independent polyclonal B-cell activator. Exposure of human B cells to all of these agents resulted in increased immunoglobulin M (IgM) and immunoglobulin G (IgG) secretion. IgM and IgG secretion induced by RU 41.740 appeared to be T cell dependent when B cells were isolated from human peripheral blood. However, this activity may have been T cell independent when B cells were isolated from human spleen. RU 41.740-induced IgM secretion by peripheral blood B cells was seen to peak after 6 days in culture; IgG secretion peaked after 7 days in culture. The optimal concentration of RU 41.740 for the induction of IgM and IgG secretion by human B cells in vitro was seen to be 200 micrograms/ml.

Carbohydrate metabolism and the hemodynamic response to shock.

To investigate the temporal relationship of changes in carbohydrate metabolism to changes in the hemodynamic stability of the dog in protracted hemorrhagic shock, 30 animals were studied. Blood pressure was maintained at 60 mm Hg for 12 hr during which hemodynamic and metabolic parameters were studied. In early shock cardiac output was decreased and peripheral resistance increased. After 2 hr cardiac output began to increase and peripheral resistance began a progressive decline despite the continuing hypotensive state. Hemodynamic decompensation was essentially complete at 5 hr. There was early and marked mobilization of carbohydrate stores. Muscle glycogen was rapidly mobilized over a 5-hr period. Serum glucose and blood lactate levels rose and remained elevated for 8 hr. Amino acid degradation remained at high levels throughout the experimental period. Hemodynamic decompensation began well before significant depletion of carbohydrate stores and was complete well before the relative exhaustion of the available reserves.

Sequential methotrexate and 5-fluorouracil in the treatment of non-small cell carcinoma of the lung.

The sequential administration of methotrexate (MTX) and 5-fluorouracil (5-FU) has been claimed to be synergistic. To investigate the potential synergism in non-small cell carcinoma of the lung (NSCCL), 16 patients were treated with a 2-hr infusion of MTX 200 mg/m2 followed after 2 hr by 5-FU 1g/m2. All patients had an Eastern Cooperative Oncology Group performance status of greater than 2 and had received no previous chemotherapy. No responses were seen. This study demonstrates that sequential MTX and 5-FU in this dosage and schedule is ineffective in NSCCL.

Relative value of glucose and amino acids in preserving exercise capacity in the postoperative period.

The value of crystalline amino acids compared with glucose in maintaining functional muscle mass (maximum exercise capacity) in the perioperative period was studied. Twelve surgical patients received 100 g of glucose (Group 1) for 7 to 10 days perioperatively, and 12 (Group 2) received 90 g of crystalline amino acids for a similar period. Maximum exercise capacity, nitrogen balance, and serum albumin were studied. The use of amino acids instead of glucose spared nitrogen. Net nitrogen loss was 64.7 +/- 6.7 g in Group 1 compared with 34.7 +/- 4 g in Group 2 (p less than or equal to 0.001). Exercise capacity decreased 13.8 +/- 4.5 percent in Group 1 and 13.3 +/- 2.9 percent in group 2. The serum albumin level decreased by 0.30 +/- 0.2 g/100 ml in Group 1 compared with 0.34 +/- 0.15 g/100 ml in Group 2. These differences were not significant. Changes in serum albumin were correlated with changes in exercise capacity (r = 0.7, p less than or equal to 0.002), but neither was significantly correlated with nitrogen loss. We concluded that the use of amino acids instead of glucose during moderate periods of semi-starvation associated with moderate trauma will not influence loss of exercise capacity significantly, although some nitrogen will be spared; patients undergoing moderately severe surgical procedures accompanied by moderate periods of semistarvation will lose approximately 14 percent of their exercise capacity; and loss of exercise capacity is not correlated with loss of nitrogen under these conditions but is loosely correlated with changes in serum albumin levels.

Evaluation of antimotion sickness drug side effects on performance.

This project has employed a computerized pursuit meter which has a high correlation with operational performance (2) to test the principal antimotion sickness drugs. Proficiency scores on the pursuit meter task were improved over placebo scores in subjects with d-amphetamine 10 mg and 5 mg, the combination of promethazine 25 mg plus scopolamine 0.4 mg with d-amphetamine 10 mg, and the combination of scopolamine 1 mg with d-amphetamine 10 mg. Scores were not significantly different from placebo scores in tests with scopolamine 0.25 mg, 0.5 mg, or 0.6 mg; marezine 50 mg; meclizine 50 mg; or dimenhydrinate 50 mg. This was also true for the combination of scopolamine 1 mg with d-amphetamine 5 mg, and that of promethazine 25 mg with d-amphetamine 10 mg. A statistically significant decrement of performance scores was seen with scopolamine 1 mg or 0.8 mg, and with promethazine 25 mg oral or 25 mg I.M. The combination of promethazine 25 mg with scopolamein 0.4 mg, and that of promethazine 25 mg oral plus 25 mg I.M. with d-amphetamine 10 mg, also gave significant decrements from placebo scores. These results indicate that selected doses and combinations of antimotion sickness drugs can be used without loss of operational proficiency.

Influence of RU 41.740, a glycoprotein extract from Klebsiella pneumoniae, on the murine system. II. RU 41.740 facilitates the response to Con A in otherwise unresponsive T-enriched cells.

RU 41.740, a glycoprotein extract from Klebsiella pneumoniae, is a polyclonal B cell activator in the thymus-independent category, and both RU 41.740 and its fraction F1 induce the production of interleukin 1 (IL 1). RU 41.740 alone appears to have no direct effect on T-enriched cells. However, we show here that when given in conjunction with concanavalin A (Con A), RU 41.740 enabled nylon wool-passed T cells, which had been accessory cell depleted and lost responsiveness to Con A, to proliferate in response to Con A. Analysis of this observation indicated that RU 41.740 probably acted via a residual accessory cell population and that contact of this cell with the T cell was necessary for Con A activation of the T cell. Because both lectin/antigen and a source of IL 1 are required to stimulate accessory cell-depleted T cells, the mechanism of action of RU 41.740 in this system may be by induction of IL 1 from residual accessory cells in the nylon wool-passed, T-enriched cell population. However, two other agents that stimulate IL 1 production, lipopolysaccharide (LPS) and F1, do not enable T-enriched cells to respond to Con A in this system.

An assessment of nutritional depletion following major colonic surgery.

This study was undertaken to determine the changes in basic nutritional indices associated with major colonic surgery accompanied by periods of semi-starvation. Changes in weight, serum albumin, nitrogen balance, and maximum exercise capacity were studied. Weight loss was 5.5 +/- 1 per cent, serum albumin decreased 0.20 +/- 0.15 gm per cent. Nitrogen loss was 5.9 +/- 0.9 gm per day and maximum exercise capacity decreased by 13.5 +/- 1.8 per cent. Nitrogen balance improved when amino acids were substituted for glucose as the maintenance regimen, but no corresponding improvement in exercise performance could be demonstrated. It is concluded that major colonic surgery associated with moderate periods of semi-starvation is associated with an average nitrogen loss of 5.9 +/- 0.9 gm per day and a 13.5 +/- 1.8 per cent loss in maximum exercise capacity or effective muscle mass.

Tearfulness: a psychoanalytic interpretation.

In our view, the tearful feeling state is occasioned by the ego temporarily threatened with being inundated by complex memories and affects. For the moment, the ego cannot provide appropriate satisfying and integrated verbal expression. Words and verbal thinking are delayed. In this brief period, a controlled regression occurs during which the ego relates the current "precipitating experience" to temporally older conflictual experiences in the presence of the current object as a transference object and with a series of relationships so closely connected to that experience that they are interchanged in whole or part by the less selective, regressed, ego operations. The conscious experience may be sadness, gratitude, love, pride, or joy. But beneath these, there is the controlled regression, which interrupts verbal expression while the ego works through the earlier related conflicts, now remembered. The tearful feeling, a step in the direction of overt weeping, primitively expresses the frustration of an early wish for relief from pain, the pain of the imbalance of a complex conflict. The tearful feeling both expresses the wish for relief of pain and, at the same time, interrupts current verbal expression until the needed defense operations reduce the threat of ego disruption by working through old conflicts once again. Our thesis, then, is that within the analytic or psychotherapeutic hour the patient's feeling of tearfulness is a "too much" ego phenomenon that leads to a psychophysiological outlet.

Influence of RU 41.740, a glycoprotein extract from Klebsiella pneumoniae, on the murine immune system. I. T-independent polyclonal B cell activation.

A glycoprotein extract from Klebsiella pneumoniae, RU 41.740, has been shown clinically to reduce infectious episodes in patients prone to frequent infections, and experimentally to reduce mortality in animals infected with various bacteria or viruses. The method of action of RU 41.740 in conferring protection from infection is as yet unknown, but in experimental animals, RU 41.740 appears to influence B and T cells as well as macrophages. We report here that RU 41.740 is a strong polyclonal B cell activator and that it activates B cells in a T-independent manner. RU 41.740 also activates spleen cells from the LPS-nonresponder mouse strains, C3H/HeJ and C57BL/10ScCr, which indicates that the polyclonal B cell activating ability of RU 41.740 is not due to contamination by LPS.

Side effects of antimotion sickness drugs.

The possible influence on operational proficiency of the side effects of the anti-motion sickness drugs was investigated using a computerized pursuit meter as the test device. The medications and doses tested were scopolamine (Hyoscine) 0.25 mg and 0.50 mg oral doses, promethazine (Phenergan) 25 mg oral and 25 mg I.M. doses. Combinations of promethazine 25 mg with 10 mg d-amphetamine (Dexadrine) oral were also used, as was a combination of promethazine 25 mg oral with promethazine 25 mg I.M. and 10 mg of oral d-amphetamine. The proficiency on the pursuit meter task was not significantly altered by 0.25 mg or 0.50 mg doses of scopolamine. The combination of promethazine 25 mg oral with 10 mg d-amphetamine did not produce a significant decrement of performance. When promethazine 25 mg was given as an oral or I.M. dose without amphetamine the decrement of performance was significant and was approximately equivalent to an alcohol blood level of 25-50 mg% (one-two drinks). When a combination of promethazine 25 mg oral plus promethazine 25 mg I.M. with 10 mg d-amphetamine was tested, the error scores were significantly lower than with the oral or I.M. dose of promethazine. This combination also produced an error score significantly higher than placebo level. Promethazine by the I.M. route was slowly absorbed and 6 of the 10 subjects reported drowsiness at 6 h post injection.

Continuous postoperative lavage in the treatment of peritoneal sepsis.

The problem of peritonitis after perforation or trauma to the colon continues to be an important one for colon and rectal surgeons. Treatments vary, but mortality and morbidity have always been high. For these reasons, the usefulness of continuous peritoneal lavage as adjuvant therapy in the treatment of peritonitis was examined. Twenty patients with gross peritoneal contamination were treated with continuous postoperative peritoneal irrigation for 17 to 72 hours. No patient died of sepsis or developed an intra-abdominal abscess. Three patients died: two of advanced cancer and one of a pulmonary embolus. Three additional patients developed complications: mild congestive heart failure in two and transient respiratory failure in one.

Acetazolamide and CO2 in hyperbaric oxygen toxicity.

The role of CO2 in hyperbaric oxygen toxicity was investigated by administering acetazolamide (Diamox), Tris buffer [tris(hydroxymethyl)aminomethane], and sodium bicarbonate by i.p. injection, and by exposure of other groups of animals to an atmosphere of 5% CO2 and 95% O2. All animals were placed in a pressure chamber and maintained at 50 psig in 100% O2 until death. The Tris buffer and the sodium bicarbonate buffer significantly extended time to onset of convulsions and to time of death. Acetazolamide and also 5% CO2 shortened time to onset of convulsions and significantly shortened survival time. These results suggest that increased tissue levels of CO2 play an important role in hyperbaric oxygen toxicity. The cause of death in our animals exposed to hyperbaric oxygen was pulmonary edema secondary to a systemic hypertension.

Antimotion-sickness efficacy of scopolamine 12 and 72 hours after transdermal administration.

The antimotion sickness remedy, transdermal therapeutic system-scopolamine, administered in this experiment was scheduled to deliver 1.0 mg of scopolamine over a period of 3 d, and this paper compares its efficacy 12 and 72 h after administration. In a double-blind study, six male college students were individually exposed to a standardized provocative test in a slow rotation room after six apparently identical treatments comprising four placebos and two medications. Efficacy was categorized as beneficial, inconsequential, or detrimental. None of the responses was detrimental. Following the first administration of the therapeutic system, there were four beneficial responses after 12 h but none was beneficial after 72 h. Following the second treatment regimen, there were four beneficial responses after 12 h and three beneficial responses after 72 h. Great individual differences were demonstrated, two subjects accounting for six beneficial responses and two accounting for only one beneficial response. The difference in efficacy after 12 and 72 h has practical and theoretical significance.
