Civilian ground and air transport of adults with acute respiratory failure.

The intermountain Respiratory Intensive Care Unit (IRICU), established in 1973 at the LDS Hospital in Salt Lake City, provides specialized care for adults and older children with severe respiratory failure in the intermountain West. Because of the large area serviced by the IRICU, a transportation system for the critically ill was developed. Our report describes the transport system, including team organization, techniques, and experiences. Of 44 attempted transports during the last three years, 43 (98%) were safely accomplished without obvious ill effect to the patient. Patients with severe respiratory failure, if their conditions are adequately stabilized, can be safely transported hundreds of kilometers by ground and air by a well-equipped and well-trained team of physicians and nurses.

The prevalence of farmer's lung in an agricultural population.

Four hundred seventy-one persons associated with farming or dairy production were surveyed by means of a questionnaire for evidence of hypersensitivity lung disease due to inhalation of moldy hay or grain (farmer's lung). Of these, 172 were evaluated with spirometry, serologic studies, and chest roentgenographs. A history typical of the farmer's lung syndrome was given by 14 of the 471 subjects (3.9 per cent). Precipitating antibodies to Micropolyspora faeni were identified in sera from 2 of these 14 subjects (14 per cent), and spirograms were abnormal in 4 (28 per cent). The prevalence of farmer's lung in this community (i.e., 3 per cent) is comparable to that found in epidemiologic studies in Britain and Scotland. Farmer's lung disease may be an important occupational illness of dairy and cattlemen in this country, but the diagnosis may be overlooked frequently because of the lack of patient and/or physician awareness.

Pigeon breeders' disease--a prevalence study and review.

Among fifty-three Salt Lake City, Utah area pigeon fanciers, 21% were found to have the clinical picture of pigeon breeders' disease. This is a much higher frequency than has been observed previously. Abnormal spirometry and increased skin reactivity to pigeon sera were found more often, statistically significantly, in symptomatic than in exposed asymptomatic fanciers. Pigeon breeders' disease is associated with a variety of abnormal findings; skin test, precipitins, pulmonary function changes and radiographic derangements; but unfortunately none of these are diagnostic. Pigeon breeders' disease is best identified by clinical criteria.

Modified ventilator with liquid-oxygen system for transport of adult patients with acute respiratory failure.

The value of modern techniques of respiratory care for patients with acute respiratory failure in specialized intensive care units has been documented. Unfortunately, many patients needing this type of care are located far from such resources and require transportation over large distances. This paper describes a modified bird Mark 14 ventilator and a Linde liquid oxygen attachment used successfully in the air and ground transport of 84 patients over a five-year period.

Computer-assisted medical direction of respiratory care.

As medical technology and the increased demand for respiratory care make concurrent monitoring of all respiratory care services more difficult, computers are coming to the aid of medical directors and quality assurance committees. The respiratory care staff at LDS Hospital, Salt Lake City, has used computers to enhance patient care and quality assurance.
