Gingival hyperplasia by nifedipine. Report of a case.

We describe a case of gingival hyperplasia in a 36-year-old renal transplantation recipient treated with nifedipine for severe arterial hypertension. The appearance of the gingival disorder was probably related to nifedipine intake. Histologically, in addition to the findings of acanthosis, papillomatosis and connective tissue hyperplasia, there was also an important plasma cell inflammatory infiltrate.

Demodectic infestation of the pilosebaceous follicle.

Scanning electron microscopy revealed Demodex folliculorum on the face surface of twenty healthy subjects. The ultrastructural morphology of Demodex and the follicle parasitization process are briefly illustrated.

Cutaneous manifestations in renal transplant recipients.

Out of 105 renal transplant recipients 100 had skin lesions: 55% iatrogenic, 74% infectious, 12% precancerous or cancerous and 4% miscellaneous. Many of these lesions were at least in part transient, and steroid-related skin lesions became less frequent as years progressed. The more frequent infections were the fungal ones, followed by viral and bacterial infections with different patterns of onset. All the precancerous lesions appeared late and were almost exclusively represented by actinic keratoses; 2 evolved into squamous cell epitheliomata. 2 patients died due to Kaposi's sarcoma and melanoma. The high incidence of skin cancers in transplanted patients and the rapid evolution of dyskeratoses into spinaliomas warrants close dermatological surveillance.

[Leukonychia totalis and ectodermal dysplasia. Report of 2 cases].

The authors describe two brothers showing leukonychia totalis in association with deafness, hyperkeratosis palmaris and plantaris, scalp modifications and dental lesions. The onychopathia began during early childhood and was also present in four other members of the same family. All nails of both hands and feet were affected and appeared white, dull and with inconspicuous lunulae. The hair was rough, dark, bristly, looking like steel wire. In particular, the changes of the hair shaft were studied under scanning electron microscope. Hair dysplasia, accompanied by structural changes such as complete and incomplete twistings, longitudinal grooves and modified imbrication, was found. After briefly reviewing the literature on the subject, the authors point out the suggestive hypothesis that all these manifestations might possibly we ascribed to a single genetic defect, in spite of their different clinical features in the various patients.

Hair casts.

Peripilar hair casts are a pathological occurrence which is probably greater than the few reports seem to suggest. In 2 patients the authors have found fine, whitish nodular accretions of epithelial cells and keratinous debris surrounding the hair shafts of the scalp over a length of 3--7 mm located at various distances from the base of the hair along which they can freely move. Long-term or repetitive tractions incidental to modern hair styling or a moderate trichotillomania due to anxiety for hair loss are probably the causative factors.

A unique cytoplasmic structure in papular histiocytoma.

Intracytoplasmic granules with a unique and highly complex ultrastructure, have been observed within the cellular infiltrate of the lesions of two patients with papular histiocytosis. Examination of serial sections has enabled construction of a three-dimensional model of these granules, which appear to be composed of an elaborate arrangement of membranes and vesicles. Two hypotheses (endocytosis and exocytosis) are proposed for the mode of formation of these unique cytoplasmic structures.
