Medical student perceptions of diagnostic radiology. Influence of a senior radiology elective.

To assess the impact of a four-week elective on medical student perceptions of diagnostic radiology, we gave questionnaires to 96 senior students on the first and last days of the rotation. Eighty-five anonymous entrance responses and 73 exit responses were obtained during a 13-month interval. Compared with other clinical specialties, the students viewed radiology as third, behind surgery and medicine, in terms of required breadth of knowledge, skill, training, and "glamour." Radiology was also perceived to have the lightest workload with the exception of psychiatry. Responses did not significantly change after completion of the elective. Perceived advantages and disadvantages of radiology were studied in detail in a subset of students. Exit responses indicated that the elective promoted favorable perceptions of radiology but did not change the relative rankings of the various specialties. It appears that at our institution basic attitudes concerning radiology are formed prior to the senior radiology elective and are affected only moderately during the elective.

Is Behçet's disease triggered by childhood infection?

In a comparison of 30 patients with Behçet's disease and 60 age and sex matched community controls an increased risk of Behçet's disease was associated with tonsillectomy, a history of cold sores, large sibship size, late birth order, travel to countries with high incidence of the disease, and first sexual intercourse before 16 years of age. These findings are consistent with a triggering of the disease by infection during childhood or adolescence in an immunogenetically predisposed host.

Hickman nodule: a mimic of metastatic disease.

A characteristic small subcutaneous soft-tissue mass can be seen on the computed tomographic scans of patients after removal of Hickman catheters. The mass is indistinguishable from subcutaneous metastatic deposits seen with a variety of malignant tumors. This potentially confusing finding can be differentiated from true metastatic disease by the characteristic location of the subcutaneous nodule in the second or third anterior intercostal space, along the midclavicular line, and by the absence of other subcutaneous masses.

Use of laser Doppler flowmetry and transcutaneous oxygen tension electrodes to assess local autonomic dysfunction in patients with frozen shoulder.

The laser Doppler flowmeter (LDF), which measures changes in cutaneous blood flow, and the transcutaneous oxygen electrode which measures cutaneous perfusion, were used to study reflex changes in the microcirculation of the shoulder in 38 patients with frozen shoulder and 10 normal controls. In all controls and 22 patients with frozen shoulder, a normal LDF response to inspiration/expiration was observed. In 16 patients with frozen shoulder, LDF responses were either unilaterally or bilaterally absent. Comparison between the two patient groups showed a significant association (chi 2 = 6.43, P less than 0.02) between abnormality of response and the persistence of pain. TcPO2 was in the normal range in all patients and controls. These findings suggest that the LDF together with the TcPO2 may be a useful method of studying the skin microcirculation over the shoulder.

Bone mass in women with hip fractures.

Bone mass was measured by single photon absoptiometry in 52 women with hip fractures and compared to controls of similar age. No difference in the forearm bone mineral content (BMC) was detectable between the groups. Extracapsular fractures had lower forearm BMC than intracapsular fractures, but these differences were eliminated by allowance for age. Clinical management therefore was not influenced by such measurements.

Physical activity and calcium intake in fracture of the proximal femur in Hong Kong.

In Hong Kong physical activity and calcium intake of 400 Chinese men and women with hip fractures were compared with those of 800 controls. Daily walking outdoors, upstairs, uphill, or with a load protected against fracture. This was independent of cigarette smoking and alcohol consumption. Higher levels of reported activity in middle life were also protective. Average calcium intake was around one quarter that in Britain because of the low consumption of dairy products. Differences in calcium intake depended mainly on consumption of green vegetables and small fish. High intake protected against hip fracture. These findings point to the importance of maintaining daily physical activity and calcium intake in urbanised Chinese populations.

Physical activity, muscle strength, and calcium intake in fracture of the proximal femur in Britain.

Regular exercise and high calcium intake possibly help to preserve bone mass. Little is known, however, about their role in preventing hip fracture. The physical activity and calcium intake of 300 elderly men and women with hip fractures were compared with those of 600 controls matched for age and sex. In both sexes increased daily activity, including standing, walking, climbing stairs, carrying, housework, and gardening protected against fracture. This was independent of other known risk factors, including body mass, cigarette smoking, and alcohol consumption. Strength of grip correlated with activity and was inversely related to the risk of fracture. Calcium intake was not related to the risk of fracture in women. Men with daily calcium intakes above 1g had lower risks. These findings point to the importance of elderly people in Britain maintaining physical activity in their day to day lives.

The time-dependent distribution of 125I-asialo-orosomucoid-horseradish peroxidase and 131I-immunoglobulin A among three endosomal subfractions isolated from rat liver.

Three discrete endosomal fractions showing a time-dependent uptake of radioactive ligand were partially purified from rat liver. The 3,3'-diaminobenzidine (DAB)-induced density-shift protocol of Courtoy, Quintart & Baudhuin [(1984) J. Cell Biol. 98, 870-876] was used to study the distribution among these three endosomal fractions of two ligands with different intracellular destinations. Rats received both 125I-asialo-orosomucoid-horseradish peroxidase (125I-ASOR-HRP) and 131I-dIgA simultaneously by intraportal injection. The liver was fractionated at various times after injection, the three ligand-containing endosomal fractions (A, B and C) were separated and each was subjected separately to the DAB-induced density-shift procedure in which only vesicles containing 125I-ASOR-HRP are increased in density. Information on whether 131I-dIgA was co-localized or segregated from 125I-ASOR-HRP was obtained. The two ligands in the A fraction were partly segregated and partly co-localized, and this distribution appeared to be relatively unchanged with time. The two ligands in the B fraction were co-localized at all times studied. We have tentatively identified the B fraction as a compartment in which vesicle fusion has occurred. The two ligands in the C fraction were also partly co-localized and partly segregated, but the 131I-dIgA became increasingly segregated with time. This represents the first report of the purification of an endosomal subfraction specifically involved in the accumulation of multiple ligands.

Bone mass and dementia in hip fracture patients from areas with different aluminium concentrations in water supplies.

The relationship between bone mass and dementia in elderly hip fracture patients from areas with different aluminium concentrations in water supplies has been investigated, to determine whether a high concentration of aluminium in the drinking water and the negative calcium balance of age-related osteoporosis together predispose to senile dementia. Mental test scores were recorded on admission to hospital, and Singh indices of radiographs taken at that time assessed blind by a single trained observer. No significant difference in either mental test score (chi 2 = 1.253, df = 2, P = 0.53) or Singh index (chi 2 = 1.039, df = 5, P = 0.96) was found between patients resident in high versus low water aluminum areas. No correlation was detectable between mental test score and Singh index (chi 2 = 0.95, df = 1, P = 0.33).

The scientific status of the defence mechanism test: a reply to Kline.

The Defence Mechanism Test, one of the better validated techniques for studying the operation of Freudian defences, has recently been impugned on theoretical grounds. This paper reconsiders two of the most telling points made by Kline (1987) and concludes that his alternative explanations are not well supported by theory or experiment.

Alterations in appendicular skeletal mass in patients with rheumatoid, psoriatic, and osteoarthropathy.

Appendicular bone mass was measured in a series of 50 patients with non-steroid treated rheumatoid arthritis, 20 with polyarticular seronegative psoriatic arthritis, and 30 with osteoarthrosis, and compared with that of 40 controls of similar age and sex. Distal forearm bone mineral content was reduced in patients with rheumatoid and psoriatic arthritis and increased in those with osteoarthrosis. The increase in bone mass in patients with osteoarthrosis was confined to those with isolated large joint disease and was not found in those with primary generalised osteoarthrosis.

Photoaffinity labelling and phosphorylation of a 165 kilodalton peptide associated with dihydropyridine and phenylalkylamine-sensitive calcium channels.

Partially purified fractions of dihydropyridine and phenylalkylamine receptors associated with voltage-dependent calcium channels in rabbit skeletal muscle were found to contain two glycopeptides of similar molecular weight. A peptide of approximately 165 kDa was photoaffinity labelled with an arylazido-phenylalkylamine Ca channel inhibitor and also was phosphorylated with cAMP-dependent protein kinase. Another peptide of 170 kDa could be distinguished from the 165 kDa peptide by peptide mapping and differences in electrophoretic mobility. The results suggest that the 165 kDa peptide contains the sites responsible for regulation of calcium channel activity by calcium channel inhibitors as well as by neurotransmitters that regulate its activity in a cAMP-dependent manner.

Osteoporosis, falls, and age in fracture of the proximal femur.

The relative importance of osteoporosis and risk of falling in causing hip fracture is not known. Femoral neck bone mass was measured in 708 elderly people, all of whom had fallen and injured a hip. Below 75 years of age there was a steep increase in relative risk of fracture with reduced bone mass, measured by Singh grade. Above that age the increase in risk was small, and neuromuscular responses which protect the skeleton against trauma may be more important than bone mass.
