[Functional respiratory and blood gas analytical studies of the effects of fenspiride, in oral and intramuscular administration, in chronic bronchopneumopathic subjects].

A group of 20 chronic bronchopneumopathics was treated for 15 days with fenspiride orally and i.m. The behaviour of a set of functional respiratory and haemogasanalytic parameters was monitored at various times (basic, 5th, 10th and 15th days). Progressive, significant improvements in VC, FEV1, RV and in related parameters were observed. These were attributed to the drug's anti-inflammatory effect in the respiratory ways as well as to its direct antibronchospastic action. Stress is laid on the excellent clinical tolerance of fenspiride following its oral and i.m. administration.

[Value of allergic intradermal reactions to antibiotics in preparation for surgery (in patients with a history of drug allergy)].

An allergological examination technique for application in patients with a history of hypersensitivity to antibiotics is described with reference to a personal series of 250 subjects admitted to the Bologna University obstetric and gynaecological pathology department for small pelvis surgery. The method of investigation is described and the results obtained are reviewed. It is felt that the investigation is of value in preoperative screening insofar as it eliminates or reduces the risk of allergic shock, and enables more precisely directed antibiotic management to be employed.

[Occupational asthma caused by dioctyl-phthalate in a bottle cap production worker].

The authors describe a clinical case of occupational asthma due to dioctyl-phthalate in a worker using a conveyor belt for bottle stoppers. The dioctyl-phthalate is used for the production of PVC and as a compound of many other substances. The literature describes many cases of occupational asthma do to phthalate anhydride but few cases do to dioctyl phthalate. The clinical history, the environmental monitoring data, the pulmonary function tests such as bronchodilatation test by salbutamol, peak flow meter daily monitoring for three weeks, and stop and start work test showed a good correlation with the work and the worker's asthma. The authors emphasize that a detailed clinical history and a careful analysis of the production cycle are very useful and, as simple and non invasive tests, can play an important role in the diagnosis of occupational asthma.

[Four-year follow-up of a group of workers with allergic sensitization employed in breeding of insects for biological pest control].

In two previous papers we reported allergic diseases in workers employed in breeding insects for biological pest control. It was found that Ephestia kuehniella, Orius laevigatus and other insects caused allergic rhinoconjunctivitis and asthma. The aim of this new study was to assess the relationship between the implementation of the recommended preventive measures and the decrease in sensitization in exposed workers. We performed a four-year follow-up of exposed workers, using the prick test and RAST with specific allergens and pulmonary function test at the end of the work shift. The results showed a decrease in skin test sensitization for Orius and Ephestia, a decrease in allergic diseases in the previously studied workers, and no symptoms in recently employed workers. These results confirm the validity of the recommended preventive measures: semi-closed cycles, gloves, protective masks, overalls and sunglasses.

[Bronchial hyperreactivity to methacholine in a group of bakers sensitized to wheat flour].

BACKGROUND: Bakers' allergic diseases are a well known occurrence. Wheat flour is the most important allergen responsible for rhinoconjunctivitis and IgE mediated asthma. The good correlation between atopy, wheat flour sensitization and allergic diseases is also well known. In a previous study we described the results concerning sensitivity to wheat flour in a group of 300 bakers, followed up for 8 years. In this study we showed that the same subjects who were sensitive to wheat flour had no symptoms. AIM: The aim of this study was to carry out the metacholine test for bronchial hyperreactivity in a group of 14 bakers selected from the group of 300 previously studied. METHODS: The subjects underwent: pulmonary function test, IgE test, skin prick test with grass pollen, Dermatophagoides farinae, Dermatophagoides pteronissinus, wheat flour, and the metacholine test. RESULTS: The results of the metacholine test showed only one positive case in a subject with positive skin prick test. This agreed with the assumptions of our previous study that showed a sensitization (10%) to wheat flour but a low prevalence of allergic symptoms. CONCLUSIONS: Limited to the subjects studied, the results indicate an absence of a relationship between positive prick tests and aspecific bronchial hyperreactivity. Recent research that suggest a probable different genetic control of atopy, bronchial hyperreactivity and asthma is discussed. In our opinion effective primary and secondary prevention could account for this absence of relationship.

[Primary lymphoma of the small intestine. Radiographic image of enteroclysis with double-contrast media].

The authors retrospectively reviewed the radiographic (double-contrast small-bowel enema) examinations of 20 patients with primitive small intestinal lymphomas. The diagnosis was histologically confirmed in all cases. The technique was very sensitive (100%) in detecting small-bowel abnormalities. On the contrary, a definite differential diagnosis was difficult to make (65%), except for 4 cases of lymphoma complicating celiac disease in which the presence of a nodular pattern and of thickened and irregular mucosal folds seemed to be highly specific. The most difficult differential diagnosis was with Crohn's disease. In all the cases with surgical confirmation (17), disease extent exactly corresponded to that suggested by small-bowel enema. Primitive small intestinal lymphoma is not radiologically distinguishable from secondary involvement during systemic lymphoma. From the analysis of radiological abnormalities, the authors observed that, different from previous reports, severe luminal narrowing was frequent, whereas "aneurysmal" dilatations of intestinal loops were never observed. CT was confirmed as the best technique for staging. Clinical implications of double-contrast enema in the diagnosis of primitive small intestinal lymphomas are discussed; radiographic patterns are reviewed.

[Role of ultrasonography in the diagnosis of carcinoma of the breast].

Breast ultrasonography is today a diagnostic technique whose results are either under or overestimated by different Authors. In this paper the Authors report their experience with ultrasound for the diagnosis of breast cancer. In 214 patients with palpable breast nodule ultrasonography employed as the only diagnostic technique allowed the diagnosis of breast cancer in 89.7% (61/68 patients), and in particular in 82.4% of T1 in 96.9% of T2 and in 100% of T3 tumors. From these data it can be resumed that breast ultrasonography reaches a good diagnostic reliability. Unfortunately the limits in defining malignancy of solid appearing mass don't allow its routine use as the gold standard for breast cancer diagnosis. We believe indeed that ultrasonography has an important role as an integrative diagnostic method among those commonly used in this field.

[Skin pathology caused by the use of limb prostheses].

Skin problems affecting the limb stump of amputee are common but often they are observed a little. The Authors describe our experience in order of these skin pathologies caused by use of prostheses during stage at INAIL Prosthese Center in Vigorso di Budrio (Bologna). The aims of this report are to renew interest in these problems and to facilitate earlier recognition and treatment of troublesome skin disorders in the amputee.
