Mobile myelographic filling defects: spinal cysticercosis.

Cysticercosis usually affects the brain and is easily demonstrated by CT. Spinal cysticercosis is much rarer and is usually diagnosed only at surgery. Myelographic demonstration of multiple rounded filling defects, some of which were mobile, allowed diagnosis of spinal extramedullary cysticercosis in an unsuspected case. The literature on spinal cysticercosis is briefly reviewed. Diagnosis is important in view of the recent development of medical treatment.

Fibrin glue vs. nylon in the anastomosis of rabbit fallopian tubes.

Fibrin glue was used in end-to-end tubal anastomosis on eight sexually mature female New Zealand white rabbits. The right fallopian tubes were anastomosed with 10/0 nylon and the left with fibrin glue (Tissucol). The animals were mated with males of proven fertility six weeks after surgery and killed two weeks later. The gestational sacs and corpora lutea on the right and left sides were counted separately and the nidation index calculated for each. The presence of adhesions was also evaluated. All the tubes were finally examined under a scanning electron microscope. The fibrin glue was satisfactory in terms of patency, pregnancy and morphology of the epithelium.

[Angiographic study of the vascularization of primary malignant tumors of the liver: histological correlation].

During the last seven years 118 patients with histologic diagnosis of malign tumor in the liver were examinated by selective angiography, amongst other diagnostic recourses: 14 patients had primitive tumors: 12 hepatoma, 1 cholangiocarcinoma and 1 hepatocholangiocarcinoma. In 12 patients resection was contraindicated preoperative by the angiographic demonstration of the extension of the tumor to both lobes. In 1 patient with cholangiocarcinoma, the tumor was resected with an outlive of 4 years. In 1 patient with hepatoma very vascular a hard league of the right hepatic artery was done. Two months later angiography demonstrated a rich revascularisation of the tumor with development of the colateral circulation. In our experience the hiper or hipovascularization is related with the degree of fibrosis which accompanies the tumor and no so much with the histologic type, the degrees of differentiation or the existence of intratumoral necrosis. Emphasis is been done to show the diagnostical difficulties of hipovascularized tumors and the importance of angiography in the preoperative evaluation of hepatic tumors.

Building national electronic medical record systems via the World Wide Web.

Electronic medical record systems (EMRSs) currently do not lend themselves easily to cross-institutional clinical care and research. Unique system designs coupled with a lack of standards have led to this difficulty. The authors have designed a preliminary EMRS architecture (W3-EMRS) that exploits the multiplatform, multiprotocol, client-server technology of the World Wide Web. The architecture abstracts the clinical information model and the visual presentation away from the underlying EMRS. As a result, computation upon data elements of the EMRS and their presentation are no longer tied to the underlying EMRS structures. The architecture is intended to enable implementation of programs that provide uniform access to multiple, heterogeneous legacy EMRSs. The authors have implemented an initial prototype of W3-EMRS that accesses the database of the Boston Children's Hospital Clinician's Workstation.

Estimating software development costs for a patient multimedia education project.

The authors compare alternative methods of cost estimation for a patient multimedia education (PME) program, using a computerized weight-reduction PME project as an example. Data from the project planning and budgeting process and actual costs of the completed project are analyzed retrospectively to calculate three different estimates-pre-work, post-work, and actual work. Three traditional methods of estimating the cost of computer programs (the lines-of-code, function point, and task ratio analyses) underestimate costs in this example. A commercial program (Cost Xpert) that calculates the cost of developing a graphical user interface provided a better estimate, as did a tally reflecting the complexity and quality of media material in the project.

Assisted reproductive technologies in severe male infertility.

The increasing incidence of male subfertility as an indication for ART is first discussed. The tendency to use assisted reproduction techniques in such cases is attributed to the disappointing results of classical treatments. The Authors deal with two problems: the choice of the best treatment methods of the sperm and the choice of the best technique of ART for treating male infertility. The analysis of 138 couples treated for male subfertility showed that the centrifugation on discontinuous Percoll gradients (CDPG) and especially on the mini-Percoll (mini CDPG) offers the best results if compared with pellet swim up and other techniques. With reference to the technique of choice, an accurate analysis of tubal (TET and ZIFT) and uterine (IVF/ET) transfers shows that no advantage seems to be obtained with the more sophisticated and exacting tubal transfer. This final conclusion is presently evaluated on the basis of a retrospective study.

Effect of acetylcarnitine treatment in oligoasthenospermic patients.

Acetylcarnitine (AC), present in human spermatozoa and seminal fluid, plays an important role in sperm metabolism. To further investigate the effect of AC on sperm quality, AC (4 g/day) was given to 20 patients with idiopathic oliogasthenospermia for 60 days. AC had no effects on sperm density and total motility, but it did significantly increase progressive sperm motility (mean +/- SEM: 21.7 +/- 3.2% vs 38.2 +/- 4.7). The increment in sperm motility was sustained ( > or = 40%) in 12 patients (mean increment 2.7 fold). This parameter returned to basal value 4 months after therapy discontinuation. Five pregnancies occurred during treatment and only 2 during the 4 months follow-up ensuing therapy discontinuation.

The use of three different therapeutic protocols according to the immunologic pathology present in three women with recurrent spontaneous abortion and fetal death for immunologic reasons: flucortolone and salicylates; high-dose intravenous gammaglobulins; subcutaneous heparin.

The authors report three cases of immunologic spontaneous abortions and fetal death. In the first patient there was lupus-like anticoagulant activity with a diagnosis of sub-clinical autoimmune disease; the second showed inadequate blocking factor activity, while the third subject presented excessive lymphocytotoxicity. In these cases three different therapeutic protocols were successfully used: flucortolone and salicylates, high-dose intravenous gammaglobulins and subcutaneous heparin.

Immunologic recurrent abortion: comparison between immunotherapies.

The Authors record the results in a group of patients with immunologic recurrent abortion (IRA) treated with two different immunoprophylaxis regimen. The first one is an active prophylaxis therapy with preparation and administration from donor mononucleates. According to their previous original experience, the Authors started giving high doses of IV-Ig (HD IV-Ig) in a second group of pregnant patients with the same diagnosis of immunologic recurrent abortion (IRA). The results of this not randomized study show better reproductive outcome in the group treated with use of HD IV-Ig.

Hemangioblastomas: clinical characteristics, surgical results and immunohistochemical studies.

A surgical series of 59 patients with cerebellar or spinal cord hemangioblastomas or von Hippel-Lindau's (VHL) syndrome is analyzed. The presence of the tumor is easily detected by Computerized Tomography (CT) and Nuclear Magnetic Resonance (NMR), but angiography is still necessary for a correct surgical planning. The value of a sharp distinction among patients with single hemangioblastomas and the ones with Lindau's disease and VHL syndrome is stressed. In fact patients with single cerebellar or spinal hemangioblastomas have a good prognosis, while patients with disseminated hemangioblastomas have a rather poor outlook. Neuropathological studies with immunohistochemical techniques have been performed to identify the nature of the stromal cells of the hemangioblastomas: their origin from glial, endothelial and monociticphagocitic elements seems excluded.
