In vitro effects of retinoic acid.

Retinoids, synthetic and natural analogues of vitamin A, play fundamental roles both in directing the spatial organization of cells during the development of vertebrate limbs and in the maintenance of growth and differentiation of many adult tissues. They also block the phenotypic expression of cancer in vitro; inhibit growth and induce differentiation in many animal and human malignant cell types. They have proved beneficial in skin diseases, cancer prevention and in acute promyelocytic leukemia.

Retinoic acids in the treatment of acute promyelocytic leukemia.

Retinoic acid, the active metabolite of vitamin A has been shown to differentiate in vitro human leukaemic cells from patients with acute promyeolocytic leukaemia (APL). The results obtained in vivo with the 13-cis isomer of retinoic acid in combination with or after chemotherapy in four cases of APL are described. More recently Huang et al from the Shangai Institute of Haematology have treated 24 cases of APL with all-trans retinoic acid alone. They obtained 24 complete remissions. This success prompted us to treat patients with APL and a contra-indication to chemotherapy with all-trans retinoic acid. The results confirm the great efficacy of all-trans retinoic acid in APL.

[Refractory anemia with partial blastosis. A new therapeutic approach].

Refractory anaemia with partial myeloblastosis is classified among myelodysplasia syndromes which are characterized by abnormalities in the differentiation of haematopoietic cells, but it differs from these syndromes in that blast cells are in excess and the prognosis is darker. So far, this type of anaemia has resisted all treatment. The possibility of differentiating leukaemic cells in vitro has opened new therapeutic prospects.

Present results of the treatment of myelodysplastic syndromes with low-dose cytosine arabinoside. Preliminary results of a cooperative protocol (38 patients) and review of the literature.

In vitro differentiating agents causing little bone marrow aplasia have been proposed as a specific therapy of myelodysplastic syndromes with excess of blasts. A critical review of the published data is presented. The preliminary results of a cooperative protocol using very low dosage of cytosine arabinoside (3mg/m2/twice daily) with or without androgens are given.

Kinetics of indium-oxine labelled red cells in haemolytic anaemias.

Indium-labelling of red cells is hampered by its high elution rate and short physical half-life. From a study of 23 pathological cases, it is demonstrated that the simultaneous study of 111In- and 51Cr-labelled red cells could only be useful in haemolytic anaemias in which the mechanisms (corpuscular or extra-corpuscular) of the cell destruction is in question.

Inefficacy of the synthetic aromatic retinoid etretinate and of its free acid on the in-vitro differentiation of leukemic cells.

The differentiation-inducer activity of the aromatic retinoid etretin and of its ethyl ester etretinate (Tigazon) was tested on the human myelomonocytic leukemia cell lines HL-60 and U-937 and compared to that of retinoic acid. Whereas all-trans and 13-cis isomers of retinoic acid were equally potent in inducing the differentiation of about 80% of the cells of both lines, etretin and etretinate were found to be totally ineffective. These results indicate that neither compounds should be used in vivo in the treatment of acute myelocytic leukemia patients.

1-B-D arabinofuranosyl cytosine and all-trans retinoic acid in combination accelerates and increases monocyte differentiation of myeloid leukemic cells.

The effect of the combination of two different agents on myeloid differentiation was studied. Human promyelocytic cells (HL-60) and monoblast-like cells (U-937) were treated with 1-B-D arabinofuranosyl cytosine (Ara-C) alone, or in combination with retinoic acid (trans-RA). The known dual potentiality of HL-60 promyelocytes was confirmed by their ability to mature into granulocytes following induction by retinoic acid and into monocyte-like cells following treatment with 1-B-D arabinofuranosyl cytosine. The U-937 cell line differentiated to monocyte-like cells with either of the two drugs. The differentiation induced by Ara-C involved an irreversible step after 24-h incubation with the drug, was concentration dependent, and far more superior on U-937 cells than on HL-60 cells. The outcome of these two cell lines after treatment with both Ara-C and trans-RA was also different: this combination maintained the monocyte differentiation in the HL-60 cell line, and resulted in a higher sensitivity of U-937 cells to Ara-C, as indicated by a cell response to low Ara-C concentrations and a more rapid expression of monocyte specific properties.

Structure-activity relationships of aromatic retinoids on the differentiation of the human histiocytic lymphoma cell line U-937.

The differentiation-inducer activity of a series of derivatives modified on the terminal ring, the polyene side-chain or the polar end group of the retinoic acid molecule was tested on the human histiocytic lymphoma cell line U-937 and compared with that of all-trans-retinoic acid. Only retinoids with both an unsaturated terminal end ring and a free carboxyl polar end group were found to be active in this system. Introduction of an aromatic ring in place of the first double bond of the side chain increased highly the activity whereas cyclisation of the last two double bonds decreased it. Replacement of the unsaturated terminal ring by an aromatic ring abolished the activity as did the esterification of the carboxyl end group or its replacement by a sodium sulfinate, sodium sulfonate or ethyl sulfone end group. All but only the active retinoids induced the same morphological and biochemical changes on U-937 cells, suggesting that they have the same route of action. However, biologically inactive retinoids were shown to be able to inhibit the differentiation of U-937 cells, induced by active ones, indicating that they can compete for a common "receptor".

Type III aminoterminal propeptide of procollagen in some haematological malignancies.

Marrow fibrosis is involved in some haematological malignancies. Either because of sampling errors, variations of focal distribution of fibrosis or the discomfort for patients of bone biopsies, conventional histology appears to be unsuitable for the follow-up of myelofibrosis. During collagen synthesis by marrow fibroblasts, the aminoterminal propeptide is removed from procollagen III and released in the serum. Thus, a sensitive radioimmunoassay of type III aminoterminal propeptide of procollagen (PC III) has been tested in myeloproliferative and lymphoproliferative disorders with a marked bone marrow fibrosis. In polycythaemia vera, PC III level was significantly increased as compared to controls and was related to marrow fibrosis of grade I. The more increased PC III values were observed in spent polycythaemia cases initially treated by phlebotomy alone. Follow-up showed a transformation into myeloid metaplasia. In contrast, PC III remained stable in patients treated with radiophosphorus 32P or hydroxyurea who did not transform. In myeloid metaplasia, results of PC III were significantly higher than in controls or polycythaemia vera cases. Myelofibrosis of recent onset (less than 2 years) gave higher values than chronic myelofibrosis. Increased PC III values were also emphasized in chronic myelocytic leukaemia, and in a few cases of refractory anaemia with excess of blasts, hairy cell leukaemia and chronic lymphocytic leukaemia.

Aplastic crisis and erythema infectiosum (fifth disease) revealing a hereditary spherocytosis in a familial human parvovirus infection.

We report two clinical observations on a father and son, who both presented an aplastic crisis at the same time during a human parvovirus (HPV) infection. This erythroblastopenia revealed hereditary spherocytosis in both patients. The son also had a clinical status of erythema infectiosum (fifth disease). This data shows that HPV may be responsible for these two different pathologies in the same person.

Diagnostic value of bone marrow imaging with 111indium-transferrin and 99m technetium-colloids in myelofibrosis.

111Indium--transferrin (111In) and 99mTechnetium-colloids (99mTc) bone marrow imaging of 55 myelofibrosis (MF) cases has been compared with clinical, histological, and iron-kinetics data. The best correlations are seen between the splenic uptake of 111In with the spleen/sacrum ratio of 59Fe at the first hour (r = 0.69, P less than 0.001) and also with the splenic erythropoiesis histologically assessed in ten splenectomized patients (r = 0.75, P less than 0.01). Moreover, sacrum uptake of 111In, when compared with sacrum uptake of 59Fe (r = 0.51, P less than 0.001) and with hematopoietic cellularity of the bone marrow (r = 0.57, P less than 0.001) reflects faithfully the hematopoietic cell content of the marrow. Thus, 111In bone marrow imaging provides a noninvasive and useful tool for the diagnosis of myeloid metaplasia in MF. Ferrokinetic studies still appreciate with better insight the amount of ineffective erythropoiesis or hemolysis and remains therefore of great value when splenectomy is discussed.

An analysis of prognostic factors in preleukemia: interest of bone marrow scintigraphy.

Simultaneous bone marrow scintigraphy with 99m Technetium colloids and 111 Indium transferrin was performed on 34 cases of preleukemic anemias and was shown to be of good prognostic value. Groups of different outcome were defined: for a normal and parallel uptake of the two markers, 90% of the patients died of acute leukemia; for a low Indium and high Technetium uptake, only 1 patient out of 15 died of leukemia (P less than 0.001). Standard clinical and hematological data were of no predictive value. Iron kinetic data and CFU/GM colony growth were correlated to the scintigraphic results. Taken together, these three kinetic parameters have a good sensitivity and specificity for the prognosis of preleukemic states.

[Cutaneous necrosis of the breast: a complication of oral anticoagulant treatment].

Cutaneous necrosis of the breast is a rare complication of oral antivitamin K drugs. It occurs shortly after instituting therapy with well-limited, irreversible skin necrosis developing rapidly. The scar is superficial and only involves the subcutaneous tissue; early and extensive surgery does not therefore seem to be justified. Normal scarring is the eventual spontaneous outcome but this is always a long process. Several arguments are advanced in favour of a toxic effect of the drug itself on the vascular walls of the vessels in the dermis.

Retinoic acid modulates RAR alpha and RAR beta receptors in human glioma cell lines.

To identify retinoic acid (RA) signalling pathways involved in growth and differentiation in cells of the glial lineage, two human glioma ceh lines were studied. The three RA receptors (RARs) mRNAs were constitutively expressed, and of the three RXRs, RXR beta appeared predominant. Western blotting analysis confirmed the constitutive expression of RAR alpha and RAR beta. Treatment with all-trans-RA induced morphological changes in the two cell lines, which progressed from their normal pattern of randomly oriented spindle-shaped cells to fibroblast-like glial cells. RA up-regulated RAR alpha and RAR beta mRNAs in both cell lines. Interestingly, RA treatment up-regulated RAR beta proteins but not RAR alpha proteins, suggesting post-transcriptional regulations of RAR transcripts in glioma cells.
