Prostaglandin E2 and cyclic AMP in tumor and plasma of breast cancer patients.

Prostaglandin E2 and cyclic AMP (cAMP) levels were measured in tumors and plasma of 78 patients with benign and malignant breast tumors. Two groups of malignant tissues were found, one with a high level of PGE2 (M = 55.4 pg/mg) and one with a low level (M = 10.7 pg/mg). The low level did not differ significantly from the benign tissue level (M = 8.7 pg/mg). Two malignant groups could not be detected in the plasma levels. Plasma PGE2 concentration (in form of the 13,14-dihydro-15-Keto metabolite) did not reflect the tissue levels, and no difference was found between the benign (M = 59.9 pg/ml) and the malignant (M = 62.3 pg/ml) patients, but both concentrations were higher than those of healthy controls (M = 34.4 pg/ml). The stage of the cancer, the histological classification and, most important, the period of survival, could not be related to the differences in the PGE2 tissue levels. Neither could plasma cAMP be nominated as a breast cancer market because no difference was found between the cAMP levels of benign tumor patients (M = 16.48 pmol/ml), of malignant tumor patients (M = 21.14 pmol/ml) and of healthy controls (M = 19.07 pmol/ml). The conclusion is that although high amounts of PGE2 appear in some malignant breast tumors, they do not affect the clinical situation. These results may explain the failure to treat human breast cancer patients with prostaglandin synthetase inhibitors.

Serum immunoglobulin levels in splenectomized Hodgkin patients and in subjects following post-traumatic splenectomy.

Mean serum immunoglobulin levels (IgG, IgA, IgM) in splenectomized Hodgkin's disease patients in remission were compared with a control group of splenectomized healthy subjects following trauma. The controls showed a higher IgA and lower IgM than the normal. The IgM of the Hodgkin's group were decreased below the normal but showed no difference from the control group. IgA and IgG levels were not significantly different from the levels in the splenectomized healthy subjects. These results suggest that changes in immunoglobulins in splenectomized Hodgkin patients could be attributed to the effect of the removal of the spleen.

Liver abscess as a late complication of percutaneous liver biopsy.

Percutaneous liver biopsy is a widely used diagnostic procedure. Severe complications accompanying its performance with Menghini's needle are rare. In one patient, a liver abscess developed one month following a diagnostic biopsy. To our knowledge, this unusual complication has not been mentioned in large series reviewing liver biopsy complications.

pH-induced platelet ultrastructural alterations. A possible mechanism for impaired platelet aggregation.

After the observation that lavages with alkaline solutions exert a beneficial effect on the bleeding tendency and increase platelet aggregation in patients with intragastric hemorrhage, studies were undertaken to find a relationship between changes of pH and platelet morphology. Transmission and scanning electron microscopy showed that at a pH lower than 7.4, normal human platelets change their internal structure as well as their shape, becoming spheres deprived of pseudopodia. On the other hand, a pH higher than 7.4 induces transformation of platelet internal organelles similar to that caused by thrombin. At an alkaline pH, the platelets develop multiple pseudopodia that facilitate their attachment to each other. These findings may explain the increased platelet aggregation in alkaline medium demonstrated in one of our previous works.

Healing of the operative fascial defect after repair of huge midline hernias with the reverse sheath method.

An experiment was carried out to ascertain the fate of the bared muscle following the repair of huge midline hernias with the "reverse sheath" method. Thirty-five patients have undergone operation by this method over the past five years, with excellent results. Experimental stripping of abdominal fascia was done on 35 rats. The bare rectus muscle was examined 1, 3, 5, 8 and 11 months after the operation. No regeneration of fascia was seen. The missing fascia was replaced by scar tissue. The results are important with regard to the support of the abdominal wall after repair of midline abdominal hernia by the above-mentioned method.
