[The leukocyte migration inhibition test (LMIT) in recurrent herpes simplex labialis. Comparison of the results of treatment with BCG and Levamisole (author's transl)].

Forty six patients suffering from recurrent herpes simplex labialis (HSLR) have been treated by either BCG or Levamisole. Therapy was equally successful in both groups: 10 patients remained free of symptoms during the observation period, 11 patients improved clearly and 2 patients did not respond to the therapy. Sensitization of lymphocytes toward herpes simplex virus antigen (HVS-AG) was determined in vitro by means of the leukocyte migration inhibition test (LMIT); test were performed before therapy and in certain intervals after the end of it. While lymphocytes of 10 control persons could be stimulated by HSV-AG, those of patients suffering from HSLR did not react to this antigen before onset of any therapy. Clinical success of therapy correlated well with the in vitro results of the LMIT: after therapy, the lymphocytes of symptom-free and improved patients also reacted in vitro to the antigen added.

Cell-mediated immunity in patients with cervical cancer.

In 16 patients with cervical carcinoma (stages II and III) tumor-specific and general immune reactivity was investigated. Cancer patients' skin reactivity was different from that of controls insofar as there was a decreased response to streptokinase-streptodornase as well as to DNCB. Absence of DNCB sensitization was mainly in stage III patients. Cell-mediated immunity to autologous and/or allogeneic tumor-associated antigens (TAA) was demonstrated in vivo and in vitro. 3 of 8 patients had positive skin reactions to autologous TAA and 8 of 14 reacted to allogeneic TAA. In the lymphocyte migration inhibition test, 8 of 14 patients reacted to autologous tumor membrane preparations and 3 of 10 to allogeneic pooled preparations. In addition, there was some indication of cross-reaction with antigens from a human squamous cell carcinoma of the lung. There was no reaction in vitro to normal cervical tissue and no association between tumor-specific reactions and herpes simplex virus type II antibodies could be demonstrated. Patients with DNCB sensitization and in vivo as well as in vitro lymphocyte reactivity to TAA had a better prognosis than nonreacting patients, indicating the value of combined immunological tests.

[Immune stimulation with vitamin A therapy in patients with pulmonary neoplasms].

Based on a clinical trial, in which patients with unresectable bronchogenic cancer were treated with a combination of vitamin A plus chemotherapy, or vitamin A plus radiotherapy, a study was initiated in which vitamin A alone was given for tumor treatment. 9 male patients with metastatic unresectable squamous cell carcinoma of the lung were treated with vitamin A palmitate or 13-cis vitamin A acid. Up to seven treatment courses were given during a period of 60 weeks. Through weekly evaluation of the patients' immune status, an immune potentiating effect of the vitamin A therapy could be demonstrated. An increase of lymphocyte blastogenesis response to PHA which is significant (p less than 0.001) compared with the pretreatment values, was found in all patients at the end of one vitamin A treatment course. Increased delayed cutaneous hypersensitivity reactions were observed also in all patients. The immune potentiating effects of vitamin A therapy, as well as the demonstrated direct effect on the tumor, introduces a wide range of combination therapies.

[Therapy of inoperable bronchial carcinoma].

The results of a prospective randomized study on carcinoma of the bronchus are reported. In a total of 56 patients polychemotherapy, high-dosage vitamin-A treatment, Endoxan and immunotherapy, as well as the application of proteolytic enzymes in altogether 5 combinations of therapy were tested.

[Clinical experiences with Holoxan in small cell carcinoma of the bronchus (author's transl)].

Clinical experiences obtained until now in the treatment of 47 patients with small cell carcinoma of the bronchus with Holoxan are reported. Special emphasis is laid to the results of a prospective randomized study; two groups of patients, one of them undergoing Holoxan therapy, the other receiving symptomatic treatment, have been investigated in respect of relationship between tumour remission, quality of life and survival time.

Immunosuppression by human placenta lactogen (HPL) and the pregnancy-specific beta 1-glycoprotein (SP-1). Inhibition of mitogen-induced lymphocyte transformation.

The in vitro effect of human placental lactogen (HPL) and the gravidity-specific beta1-Glycoprotein (SP-1) on the lymphocyte transformation was investigated in 20 healthy women. It was shown that simultaneous incubation of the lymphocytes with HPL or SP-1 together with PHA has no influence on the incorporation rate of H3-Thymidine. When the fetal calf serum (FCS) in the lymphocyte cultures was substituted by a 3% human albumin-solution, there was a reduction of the incorporation rate of the radioactive nucleic acid precursors to 20%, as compared to the controls with FCS. After addition of HPL and SP-1 to the cultures there was a further dosis dependent decrease of the incorporation rate. When lymphocytes were preincubated with the proteins for 24 h, there was a marked reduction of the blastogenesis after additon of PHA, HPL and SP-1 under these conditions had a immunosuppressive effect on the lymphocyte transformation.

Demonstration of tumor-associated antigens on tumor cell hybrids by means of the leucocyte-migration-inhibition-test.

The lymphocytes of patients with inoperable bronchus carcinoma were investigated for their in vitro reactivity toward tumor-associated antigens. The leucocyte-migration-inhibition-test (LMIT) was applied using formalinised tumor cells and tumor cell hybrid cell lines. 34 patients with inoperable suqamous cell carcinoma of the lung and 21 healthy controls were tested for their leucocyte sensitisation to these cells. 55 percent of the patients with lung cancer showed a specific LMI toward the E-14 cell line, (an established cell line from a human squamous cell carcinoma of the lung), compared to only 9 percent of the controls. There was about the same percentage of specific reactions of the lymphocytes to three different hybrid cell lines which have only a few human chromosomes left over. There was no reactivity to the parental hamster fibroblasts. It is assumed that the hybrid cells carry the same tumor-associated antigens on their cell surfaces as the E-14 tumor cells.

Active specific and active non-specific immunotherapy in patients with malignant melanoma.

Patients in clinical stage I-III of malignant melanoma were treated after resection of the tumor mass with membrane extracts of autologous tumor tissue and BCG or BCG alone. They were monitored immunologically by delayed cutaneous hypersensitivity reactions, i.e. skin tests with recall antigens, with autologous tumor membrane extracts and 2-4Dinitrochlorobenzene (DNCB). The lymphocyte reactivity was assessed in vitro with direct lymphocyte migration inhibition assay; purified tuberculin and autologous or allogeneic tumor extracts were used as antigens. In this study, which includes 50 patients up to now, it could be demonstrated that it is possible to increase tumor-specific and general immune reactivity by this form of treatment.

[Immunological investigations in lichen planus patients (author's transl)].

30 patients suffering from generalized lichen planus (= L.p.) have been examined by the following methods in order to more precise information regarding their immune-status: 1. Quantitative determination of immuno-globulins including complement fraction (beta 1C/A) 2. Intracutaneous tests with recall-antigens 3. Investigation of cutaneous sensitivity to DNCB 4. Lymphocytemigrationinhibition- and Lymphocytetransformationstests. The results obtained were not statistically significant when comparing the results of this investigation in patients suffering from L.p. with the control group. Therefore, the theory that immunological processes are the basis of these occurences is still under discussion. Further studies applying more specific methods should be conducted in order to clarify this problem.

Time lapse cinematographic demonstration of humoral and cell mediated immunity in squamous cell carcinoma of the lung.

Out of 42 attempts in one case a cell line derived from biopsy material of a squamous cell carcinoma of the lung has been established. Tumor-associated-antigen could be demonstrated by indreict immunofluorescence. The effect of allogeneic cytotoxic antibodies, absorbed with normal human lung and sera, on the target cells is shown. Blood lymphocytes obtained from patients with squamous cell carcinoma of the lung proved to be cytotoxic against this cell line. The cell to cell interactions of this phenomenon and the effect of cytotoxic antibodies are shown by time lapse cinematography.

[The in vitro influence of dehydrobenzperidol (droperidol) on lymphocyte transformation (author's transl)].

Droperidol, an agent which is used in neurolept anaesthesia was tested in vitro for its effect upon the lymphocyte transformation test. Using peripheral blood of healthy volunteers droperidol was added simultaneously with PHA (phytohaemagglutinin) respectively after the lymphocytes had been incubated with PHA already for a period of 47 hours. Lymphocyte transformation was not impeded significantly when the first method was used, when clinically comparable concentrations were administered. When droperidol was added after PHA even lower concentrations caused an inhibition. The significance of these different modes of testing is discussed. It is concluded from these results that droperidol in clinical concentrations as used in Neurolept anaesthesia does not have a distinct and lasting influence upon lymphocyte transformation and thus has no considerable immunosuppressive effects.

[Halo Nevus (Morbus Sutton): model of an immunological tumor regression].

Spontaneous regressions of malignant tumors are rare in the history of malignant disease. In malignant melanoma immunological mechanisms were accounted for the regressions observed. By the clinical phenomenon called Halo nevus a benign pigment-cell-tumor is described which shows spontaneous involution. The characteristic feature is the depigmentation of the surrounding uninvolved skin and a centripetal progression of the tumor-regression-phenomenon. In the present study it is demonstrated by histopathological and immunological investigations that lymphocytes are responsible for this tumor-regression. Lymphocyte-reactivity of 16 patients with Sutton's disease in different stages were investigated in the microcytotoxicity-assay against culture-grown melanomatargetcells. In 11 patients a specific cell-mediated immunity against the melanoma cells could be demonstrated in vitro. 5 patients did not show any reactivity, the Halo nevus had been removed by surgery 2 years earlier. The results of the immunological assay are confirmed by histopathological light- and electron-microscopic incestigations and documented by typical examples.

Induction of rat CD4+ proliferative and mouse CD8+ cytotoxic T-cell lines specific for human papillomavirus type 16 antigens.

Infection by human papillomavirus type 16 (HPV16) has been associated with cervical dysplasia and carcinoma. To detect a possible cellular immune response against HPV16, we investigated the induction of T cells specific for antigens encoded by the virus. Two CD4+ T-cell lines with a specific proliferative response were isolated from the spleens of rats vaccinated using vaccinia virus vectors expressing E6 or E7 and challenged with syngeneic HPV-transformed cells. Two CD8+ T-cell lines with a specific cytotoxic activity were obtained from mice immunized using a syngeneic squamous cell tumour cell line transfected with the full-length HPV16 DNA. These results demonstrate that both CD8-mediated cytotoxic responses and CD4-mediated delayed-type hypersensitivity responses are involved in immunologic reactions to HPV antigens.
