Analgesic properties of electromagnetic field therapy in patients with chronic pelvic pain.

AIM: Demonstration of analgesic effects of electromagnetic field treatment in cases of chronic refractory pelvic pain. STUDY DESIGN: Prospective non-controlled trial, 64 women complaining about pelvic pain of at least 6 months duration, resistant to standard therapies, submitted to electromagnetic field applications on both iliac regions by Thelf Systems apparatus by two applications daily lasting 2 hours each for 20-40 days. Control visit after 3 months. RESULTS: Complete subsidence of pain in 39 cases (61%), in 15 patients (23%) relief during treatment, then mild endopelvic tension after a 3-month control; in 10 cases (16%) symptoms reduced only during application hours, unchanged at follow-up. Outcome of treatment appears to be independent of pre-existent psychosocial variables. CONCLUSION: Magnetic therapy shows a real analgesic effect on pelvic pain, and seems to contribute to resolution of complex interactions between somatic nociceptive stimuli and psychosocial implications affecting pain perception in these patients.

Rational use of cryosurgery and cold knife conization for treatment of cervical intraepithelial neoplasia.

In recent years a variety of conservative treatment modalities are being used to remove Cervical Intraepithelial Neoplasia (CIN). A series of 568 cases of patients affected by CIN, detected in our Department over a period of 10 years and treated following a balanced use of colposcopy guided cryosurgery and cold knife conization, has been reviewed. One-hundred-fifty-three patients, 111 with CIN 1 and 42 with CIN 2, were treated by the use of a nitrous oxide cryoprobe. The remaining 415 patients underwent cold knife conization, under general anesthesia and hospitalization. Routine follow-up for all cases included cytology, colposcopy and aimed biopsy when required. Cryotherapy had a success-rate of 90.1% (CIN 1-CIN 2 only) and "complete" conization 94.6% (included CIN 3 cases). Complications of conization, such as late haemorrhage and cervical stenosis, were more frequent with the "open technique" than the "suture technique". Nine pregnancies after cryotherapy and 34 after conization have been reported, no significant differences were noted in the pregnancy outcome following either treatment.

Grading colposcopic appearance: paired comparison between two methods for differentiating benign papillomaviral infection from high-grade dysplasia of the uterine cervix.

Histologic features of 134 cases of human papilloma virus (HPV) cervical infection and cervical intraepithelial neoplasia (CIN) were reviewed and correlated with colposcopic diagnosis on the basis of two different grading methods: International Nomenclature for Colposcopy showed 79.1% predictive accuracy and Reid's improved colposcopic index 86.6% of accurate predictions (significant difference at P less than 0.05). Overestimation difference was the most significant, with 23 overestimates in International Classification and 11 in Reid's score (P less than 0.01).

Removal or preservation of ovaries during hysterectomy: a six year review.

During six years (1982-1987) in our Department 965 consecutive hysterectomies were performed: 583 abdominal and 260 vaginal hysterectomies performed for benign indications (total = 843) have been reviewed. Preservation of ovaries was chosen in 234 cases (27.8%), while 609 women had their ovaries removed (72.2%). Indications and choice of surgical procedure have shown more significant correlation with ovariectomy than different age groups and parity. Incidence of subsequent signs and symptoms of menopause and osteoporosis has been detected and compared between premenopausal patients who had oophorectomy and those who had not. In removed ovaries no undiagnosed malignant ovarian diseases were found.

Detailed ultrasound as a screening method for craniospinal abnormalities.

High resolution diagnostic ultrasound was assessed as a screening method for craniospinal anomalies during the second trimester of pregnancy in a population at low risk for neural tube defects (83,403 mothers). The effectiveness of the test was about 60% and the failure rate mainly due to late attendance. In a subgroup (9325) where the screening purposes were satisfactorily fulfilled, the detection rate (87%) was substantially greater. The significance of the results and the cost/benefit ratio, especially compared with serum alpha-feto protein screening services, are then discussed.

Rapid growth of myxoid leiomyosarcoma of the vulva during pregnancy: a case report.

Smooth muscle tumors arising in the vulva are rare. Leiomyosarcoma is the most common variant of vulvar sarcoma, and very few cases have been reported during pregnancy. A 36-year-old woman presented with a progressively enlarging vulvar mass during pregnancy, diagnosed as a Bartholin's gland cyst. The lesion was resected at 38 weeks of gestation during cesarean section and diagnosis of myxoid leiomyosarcoma of the vulva was made. Six weeks later the patients were referred to our center and submitted to wide vulvar excision with groin lymph node dissection that revealed the presence of a small residual focus of leiomyosarcoma. At 30 months of follow-up the patient was well without any sign of recurrent disease. Leiomyosarcoma should be included in the differential diagnosis of vulvar masses; progressively enlarging vulvar lesion should be biopsied even during pregnancy. Leiomyosarcoma should be considered in the differential diagnosis of vulvar mass.

Expression and amplification of HER-2/neu oncogene in uterine carcinosarcomas: a marker for potential molecularly targeted treatment?

Surgery is the treatment of choice for uterine carcinosarcomas; nevertheless, the poor effect of chemotherapy and radiotherapy represents an insidious problem for patients with metastatic or unresectable disease, and indeed, new therapeutic approaches are clearly required to improve survival of uterine carcinosarcoma patients. The HER-2 oncogene, located on chromosome 17, encodes for a tyrosine kinase growth factor receptor. We analyzed HER-2/neu overexpression by immunohistochemistry in 28 uterine carcinosarcomas. HER-2/neu amplification with fluorescence in situ hybridization (FISH) was tested in positive cases. The expression of HER-2/neu was correlated with disease-free interval and survival (Kaplan-Meier estimates). We observed HER-2/neu overexpression in nine cases (32.1%) and HER-2/neu amplification in all the four HER-2/neu 3+ score positive cases tested by FISH. HER-2/neu expression was not correlated with clinical outcome. Patients with disease limited to the uterus (stages I-II) displayed a significantly better disease-free survival (P= 0.004) and actuarial survival (P= 0.01). Demonstration of HER-2/neu overexpression and amplification in uterine carcinosarcoma may represent the first rationale step for further investigations. Hence, the results of this analysis may support the challenge of a new therapeutic approach, which could test the role of anti-HER-2 (trastuzumab) in patients with advanced or metastatic uterine carcinosarcoma.

Structured versus long-chain triglycerides: a safety, tolerance, and efficacy randomized study in colorectal surgical patients.

BACKGROUND: After trauma or surgery, researchers have suggested that medium-chain triglycerides have metabolic advantages, although they are toxic in large doses. To try to reduce this potential toxicity, structured lipids, which provide a higher oxidation rate, faster clearance from blood, improved nitrogen balance, and less accumulation in the reticuloendothelial system, could be used. Therefore, we evaluated, through a blind randomized study, the safety, tolerance, and efficacy of structured triglycerides, compared with long-chain triglycerides (LCT), in patients undergoing colorectal surgery. METHODS: Nineteen patients were randomized to receive long-chain or structured triglycerides as a lipid source. They received the same amount of calories (27.2/kg/d), glucose (4 g/kg/d), protein (0.2 g/kg/d), and lipids (11.2 kcal/kg/d). Patients were evaluated during and after the treatment for clinical and laboratory variables, daily and cumulative nitrogen balance, urinary excretion of 3-methyl-histidine, and urinary 3-methylhistidine/creatinine ratio. RESULTS: No adverse effect that required the interruption of the treatment was observed. Triglyceride levels and clinical and laboratory variables were similar in the two groups. A predominantly positive nitrogen balance was observed from day 2 until day 5 in the LCT group and from day 1 until day 4 in the structured triglycerides group. The cumulative nitrogen balance (in grams) for days 1 to 3 was 9.7+/-5.2 in the experimental group and 4.4+/-11.8 in the control group (p = .2). For days 1 to 5 it was 10.7+/-10.5 and 6.5+/-17.9 (p = .05), respectively. The excretion of 3-methylhistidine was higher in the control group but decreased in the following days and was similar to the experimental group on day 5. CONCLUSIONS: This study represents the first report in which structured triglycerides are administered in postoperative patients to evaluate safety, tolerance, and efficacy. It suggests that Fe73403 is safe, well tolerated, and efficacious in terms of nitrogen balance when compared with LCT emulsion.

Survival in early gastric cancer: multivariate analysis on 72 consecutive cases.

BACKGROUND/AIMS: Few reports from the Western hemisphere have investigated the impact of pathological features and surgical modalities on the prognosis of patients affected by early gastric cancer (EGC). In particular, the extent of lymphadenectomy (limited vs. extended) and the type of gastric resection (subtotal vs. total) remain controversial issues in the management of EGC. The aim of this study was to identify factors influencing prognosis in patients affected by EGC. METHODOLOGY: Hospital records and pathological specimens of 72 patients with EGC undergoing resective surgery during the period 1981-1995 were retrospectively reviewed. Patient status was determined by follow-up examination or by telephone contact. Univariate and multivariate analysis was used to calculate the 5-year survival probabilities with respect to the following variables: age (< or = 65, > 65), sex, depth of invasion (mucosal, submucosal) tumor location (upper, middle and lower third), gross appearance (type I, type II and type III), size (< or = 1.5 cm, > 1.5 cm), presence or absence of lymph node metastasis, histological type (intestinal, diffuse), extent of lymphadenectomy (limited or extended), and type of gastrectomy (total or distal subtotal). Survival was the outcome variable studied. RESULTS: Multivariate logistic regression analysis showed that age, nodal involvement and depth of invasion were independently associated with poor survival. CONCLUSIONS: Results showed a significant dominance of host- and tumor-related factors over the type of surgical procedure on prognosis of EGC patients.

Extrahepatic bile duct carcinoma: a western experience with 118 consecutive patients.

BACKGROUND/AIMS: This study was designed to evaluate the effectiveness of a diagnostic and therapeutic approach adopted in 118 consecutive patients affected by primary malignancy of the extrahepatic bile duct. METHODOLOGY: After diagnostic procedures were performed (ultrasound examination, endoscopic retrograde cholangiopancreatography and computed tomography scan) 25 patients underwent surgical resection. For the remaining 93 patients considered unresectable for cure, stenting by endoscopic means was almost always the only palliative treatment performed. RESULTS: The morbidity and mortality rates were 28% and 8% for patients treated with surgical resection and the curative and overall 3-year survival rate was 30% and 22% respectively. The procedure related morbidity and mortality rates were 13% and 4% for patients endoscopically treated and the median survival rate was 7.3 months. The quality of life evaluated in 68 patients out of 93 was good in 57% of cases, fair in 19% and poor in the remaining 22%. CONCLUSIONS: The results of the present study demonstrate the safety and efficacy of endoscopic stenting in the palliative treatment of extrahepatic bile duct cancer while potential cure can only be achieved by radical surgical resection.

Radical surgery for liver hydatid disease: a study of 89 consecutive patients.

BACKGROUND/AIMS: This study was designed to evaluate the effectiveness of radical surgery for liver hydatid disease. MATERIALS AND METHODS: Hospital charts of 104 consecutive patients with liver hydatidosis observed in our unit during the period 1982-1994 were reviewed. A total of 121 cysts in 89 patients were treated surgically: with cystopericystectomy in 66 and liver resection in 23. RESULTS: The overall incidence of postoperative complications was 19%: 19.7% and 17.1% after cystoperi-cystectomy and liver resection respectively (p = 0.32). Overall postoperative mortality was 1%. Among the 72 patients available for follow-up, only one (1%) had a local recurrence of the disease. CONCLUSIONS: Results suggest the safety and efficacy of radical procedures in the surgical management of liver hydatid disease. Total cystopericystectomy is the treatment of choice but liver resection is justified in selected cases.

Solitary necrotic nodule of the liver: diagnosis and treatment.

In 1983, Shepherd and Lee described the solitary necrotic nodule of the liver, an uncommon non-malignant lesion with a characteristic histological appearance. This nodule can often be misinterpreted as a necrotic secondary tumour. Limited liver resection can make differential diagnosis and cure the lesion.

[Clinical experience with uterine sarcoma].

BACKGROUND: To report and compare personal experience with international literature on uterine sarcomas. METHODS: 17 cases of uterine sarcomas diagnosed during seven years in our Institute, have been clinically histopathologically, prognostically and therapeutically analyzed. RESULTS: 10 cases of LMS, 6 of MMT, and 1 of ESS have been found. Women affected were mostly 42 to 84 years old. Most of the youngest patients were affected by LMS, while the oldest by MMT. Almost all of them were in menopause and pluriparae. The symptomatology consisted mainly of vaginal bleeding (58.9%). The diagnosis was preoperative for MMT and ESS; in 8 cases it was adopted for stage I, surgery and RT for stage II, surgery, RT and chemotherapy for a woman of stage III, and for the last two surgery and chemotherapy. The patients of stage III and IV survived respectively 8 and 10 months; three patients in stage I still have a 5 years survival and the other are still in follow-up. CONCLUSION: This work is a clinical contribution since the rarity of these neoplasms does not allow the standardization of a diagnostical and therapeutical iter and consequently the improvement of prognosis.
