Complete resolution of breast cancer bone metastasis through the use of beta-interferon and tamoxifen.

A case report is described referring to a patient affected by single bone metastasis to the cranium from breast cancer. A traditional pharmacological approach was tried but proved clinically disappointing, though radiological and scintigraphic investigation confirmed a complete regression of the metastasis following a combined therapy with beta-interferon and tamoxifen for six months. Results suggest that beta-interferon and tamoxifen may offer an alternative effective approach in treating metastatic breast cancer.

Pre-Cushing's syndrome: a case report.

A 67-year-old man affected by prostate cancer was incidentally found to have a nodular enlargement of the left adrenal gland without apparent changes in hormonal status. The adrenal mass was found to be scintigraphically active, the radiolabelled compound being concentrated in its context with a consensual suppression of the contralateral uptake. The patient underwent a resection of the adrenal tumor. Histologically and biochemically, the adrenal mass was found to be a non-functioning adenoma. The radioisotopic uptake along with the non-hormonal activity prompted us to call this tumor "Pre-Cushing's syndrome" of the adrenal cortex.

[Original method of the treatment of achalasic megaesophagus with EEA stapler].

Esophageal sutures require the same general criteria applied for the realization of an intestinal suture. The lack of a serous membrane and the particular vascularization of the organ, though, make this portion of the gastroenteric tract very prone to serious complications. Undoubtedly, the introduction of new systems of suture renders easier, faster and more efficacious the work of the surgeon. In particular, the Authors stress the advantages that staplers can offer for the treatment of achalasic megaesophagus.

A rational approach to the management of lymphedema.

Because of the complexity of peripheral lymphedema disorders and the variability in severity and clinical appearance, no standardized management plan of these patients is universally accepted. Based on our lengthy experience, we attempt in this report to outline a protocol for "staging" and managing lymphedema including the role of microsurgical lymphatic angioplastic shunts.

Use of autologous interposition vein graft in management of lymphedema: preliminary experimental and clinical observations.

We report preliminary experimental studies in rats and rabbits together with clinical observations in 39 patients with chronic lymphedema undergoing interposition autologous lymphatic-venous-lymphatic (LVL) anastomoses. This microsurgical operative technique is an alternative to other lymphatic shunting methods particularly where venous disease coexists in the same limb and where direct lymphatic-venous bypass is accordingly inappropriate. Preoperative diagnostic evaluation including lymphatic and venous isotopic scintigraphy, Doppler venous flow metrics and pressure manometry play an essential role in delineating the status of both the lymphatic and venous systems and in determining which microsurgical procedure, if any, is indicated. Our microsurgical method consists of inserting suitably large and lengthy autologous venous grafts between lymphatic collectors above and below the site of blockage to lymph flow. The data demonstrate the feasibility of the LVL technique experimentally and in 39 patients with obstructive lymphedema (either arm or leg). Using LVL shunt, improvement was seen in both limb function and edema, and in some, edema regression was permanent for as long as 5 years.

Immunohistochemical study of carcinoembryonic antigen (CEA) in gastric tumors: correlation with preoperative serum levels, histologic type, and grade of anaplasia of the tumor.

The occurrence of carcinoembryonic antigen (CEA) was studied in 45 cases of gastric tumors by the immunoperoxidase technique. CEA-positive staining was found in 48.8% of tumors. A correlation was found between preoperative CEA values and tumor CEA staining. All patients with serum CEA values below 2.5 ng/ml showed CEA-negative staining of tumor. In patients with serum CEA values between 2.6 and 10 ng/ml, the tumors showed a minority of CEA-positive cells; but in patients with serum CEA values above 10 ng/ml, the tumors contained a majority of CEA-positive cells. CEA-positive staining was found in 34.4% of tumors of the diffuse type, and in 75% of tumors of the intestinal type. A high percentage of CEA positivity was seen in well-differentiated tumors (87.7%) compared to the moderately differentiated (69.2%), and to the undifferentiated (28.7%). A faint CEA-positivity was observed in intestinal metaplasia, while normal gastric mucosa was CEA-negative.

[Lymphedema. Diagnostic aspects and therapeutic choices].

The Authors refer about their experience concerning the present problems in relation to the microsurgical treatment of chronic lymphoedema of limbs. They deal with the diagnostic protocol they perform today in order to state precisely the correct indications and dwell upon the specific microsurgical methods, among which the personal one of "lymphatic-venous-lymphatic-plastic", which, in the last few years, allowed to overcome the limits and the contra-indications of the methods of lympho-venous microanastomoses. Finally, the Authors point out the important role played by the medical-physical therapy as an advantageous support to the microsurgical therapy of lymphoedema.

[Chyledema].

The principal types of reflux of chyle are reviewed in relation to the different pathological localizations at which it may occur, these including chylothorax, chylopericardium, chyloperitonitis, chylous ascites, exudative enteropathies, chyluria, genital chyledema and chylometrorrhea and chylocele, etc. A new approach to therapy of these conditions has been provided by microsurgical techniques which constitute presently an effective procedure for many cases.
