Increased synthesis of extracellular matrix in mesangial proliferative nephritis.

Extracellular matrix expansion is frequently noted in mesangioproliferative renal diseases. This study investigates the role of immunologic factors in glomerular matrix accumulation. The gene expression of type I and IV collagen, laminin and s-laminin was examined in the rat model of mesangial proliferative glomerulonephritis induced with anti-Thy 1.1 antibody. Northern analysis was performed on glomerular RNA isolated one, three and five days after disease induction and at day 3 following prior complement depletion. Tissue was immunostained for the protein products of these genes as well as for heparan sulfate proteoglycan, entactin and PCNA (a marker of cell proliferation) at days 1, 3, 5, 14, 21 and 42. A seven- to ten-fold increase of collagen IV and laminin mRNA as well as de novo expression of collagen I mRNA occurred at days 3 and 5 corresponding to the time of maximal proliferation. S-laminin mRNA levels only increased three-fold. With the exception of s-laminin, mesangial staining for all examined matrix proteins increased to a maximum at day 5 and decreased thereafter. Focal alterations of the glomerular architecture and matrix persisted at day 42. Complement depletion prevented the histological abnormalities as well as the increased expression of matrix proteins at day 3. These findings indicate that immunologic injury in the mesangium may result in overproduction of extracellular matrix components and may ultimately contribute to the development of glomerulosclerosis.

Acoustic neuroma: nursing implications related to surgical management.

Acoustic neuromas are benign tumors originating from the Schwann cells of the eighth cranial nerve. Symptoms occur related to compression and stretching of cranial nerves which alters their function. Surgical management is by middle fossa, translabyrinthine, suboccipital or combined approach craniotomy. Prevention and early recognition of postoperative complications are important aspects of nursing care for these clients. A holistic approach to client and family nursing needs is presented. Discharge planning should include assessment of client learning and home care needs. The Acoustic Neuroma Association as a source of information and support for these clients is discussed.

C6 depletion reduces proteinuria in experimental nephropathy induced by a nonglomerular antigen.

The role of the complement membrane attack complex, C5b-9, in mediating glomerular injury has been well defined in models of membranous nephropathy induced by antibody to endogenous glomerular epithelial cell membrane antigens. The effect of selective C6 depletion (to prevent C5b-9 formation) on morphologic characteristics and proteinuria in a model of in situ subepithelial immune complex nephritis induced by an exogenous cationized antigen (human immunoglobulin G (IgG)) followed by rabbit antibody to human IgG was studied. Selective C6 depletion was achieved by repeated administration of a goat antibody to rat C6. Other groups were treated with cobra venom factor to induce generalized complement depletion and with sublethal irradiation to deplete circulating leukocytes. In C6-depleted rats, C6 levels were reduced to less than 3% of baseline throughout the 2 days of the study compared with over 100% in controls. At 4 h after disease induction, glomerular deposition of antigen and antibody were similar in C6D and control groups by immunofluorescence and by direct measurement of glomerular deposition of radiolabeled antigen and antibody (cationized 131I human IgG, 9.1 +/- 0.1 micrograms/38,000 glomeruli in C6D versus 9.8 +/- 0.9 in controls; P = was not significant; rabbit 125I-labeled anti-human IgG, 104 +/- 10 ng in C6D versus 80 +/- 9 ng in controls; P = was not significant). Circulating C3 levels and glomerular C3 deposition were also similar in C6D and control groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Gas in an infected vascular prosthesis. An unusual finding on plain radiographs.

Infection of prosthetic vascular grafts is a potentially serious complication, which may prove difficult to diagnose despite modern imaging techniques. We report a case in which the diagnosis was strikingly obvious on plain radiographs, gas being present within the lumen of the graft. The clinical features and investigation of suspected graft infection are reviewed.

An immunogold assay system for the detection of antigen or antibody.

A colloidal gold-based semi-quantitative manual immunoassay method for the detection of antibody or antigen has been developed. The colloidal gold particles are coated with an organic reagent which, in turn, is attached to the antibody by covalent bonds. The antibody or the antigen are immobilized on simple chromatography paper. The paper strips are developed with an appropriate immunogold reagent in a test tube in the presence of urine (Pregnancy or Ovulation test) or serum (Rubella test). The mixture migrates up the strips towards the test band. A purple band develops which indicates the presence of the corresponding antigen (Pregnancy, Ovulation test) or antibody (Rubella test). In this format, 50 mIU or more of hCG in urine can be detected in 5 minutes or less, and antibody to Rubella virus at an HAI titer equivalent of 8 or above in serum can be detected in 10 minutes or less. These test kits, known as Result Plus test systems, are simple, rapid, highly sensitive, and require no instruments to perform.

Colposacropexy with Prolene mesh.

A retrospective analysis of 59 patients who underwent colposacropexy (CSP) using Prolene (polypropylene) mesh is presented. Fifty-eight of the patients had undergone previous surgical treatment, including either vaginal or abdominal hysterectomy. Twenty-two patients underwent CSP alone, 24 had CSP and retropubic urethropexy (RPU), eight had CSP with anterior or posterior repair, or both, and five had CSP and RPU with anterior and posterior repair. The operations were associated with a minimum of intraoperative complications and acceptable postoperative problems. A postoperative questionnaire was sent to the patients with an 89 per cent response rate. None of the patients complained of protrusion from the vagina. It is concluded that, in the hands of experienced surgeons, CSP is a safe, efficacious operative procedure that should remain the procedure of choice for vaginal vault prolapse since it restores the normal vaginal axis, maintains existing vaginal length and provides permanent care.

Short-term malnutrition in neonatal rabbits: effect on function and synthesis of free radical metabolizing enzymes in the gastrointestinal tract.

Oxygen-derived free radicals are an important component of gastrointestinal injury in necrotizing enterocolitis (NEC). To assess the effect of a 72-h fast on the ability of neonatal small bowel to metabolize free radicals, the activity of superoxide dismutase (SOD), catalase, and the glutathione cycle were quantitated in mucosal scrapings from proximal and distal small bowel of fed and fasted neonatal rabbits. Hexose monophosphate shunt activity, quantitated in enterocytes from fed and fasted neonatal rabbits, was significantly less, p less than 0.01, in fasted animals. SOD activity was lower in distal small bowel from fasted animals than fed. The two mechanisms available to metabolize H2O2, catalase and the glutathione cycle, were significantly lower in both proximal and distal small bowel from fasted animals than in those from fed animals. To determine if fasting caused decreased enzyme activity at the level of gene expression, gastrointestinal tract DNA, RNA and protein, and specific mRNA levels for catalase, glutathione peroxidase (GPx), and SOD were quantitated. DNA, total RNA, and mRNA for SOD were lower in mucosal scrapings from fasted animals. However, mRNA for catalase, and GPx were not lower in fasted animals. Thus, a 72-h fast in neonatal rabbits causes a regional-specific decrease in SOD activity, which may be explained by decreased transcription. Changes in transcription do not completely account for regulation of catalase and glutathione cycle enzymes.

Carboplatin, ifosfamide and etoposide with mid-course vincristine and thoracic radiotherapy for 'limited' stage small cell carcinoma of the bronchus.

Forty-two patients with small cell lung cancer were treated with a combination of carboplatin, ifosfamide and etoposide. Vincristine was given on day 14 of each course, the courses being repeated every 28 days for a maximum of six. Thoracic radiotherapy was given 4 weeks after the last course of chemotherapy but no prophylactic cranial radiotherapy was administered. Thirty patients had clinically limited state disease, the remaining patients having contralateral neck lymphadenopathy and/or pleural effusions. Elevated enzyme levels (alkaline phosphatase, LDH, ALT, GGT) were noted in 69% of patients. Twenty-four patients (57%) achieved a complete response (CR) when assessed one month after the end of treatment. A further 21% of patients had a partial response (PR). Median duration of CR was 14 months and of PR 8 months. Cerebral metastases were the sole site of relapse in 13% of the CR patients. Myelosuppression was severe with a median nadir of neutropenia of 0.2 x 10(9) cells 1-1. However, 74% of the patient group received all six courses of chemotherapy and only 16 courses (7%) were delayed because of toxicity. There were three deaths associated with treatment-related neutropenia. The median survival of the total group was 14 months, with an actuarial 2 year survival of 37% and a minimum follow-up of 18 months. [A recent analysis, March 1989, demonstrated a 33%, 2 year actual survival.]

Short term neonatal starvation altered cholera toxin binding in rabbits.

Acute neonatal malnutrition alters lumenal glycoproteins as demonstrated by altered lectin binding. To determine the effect of a 72-h fast on lumenal glycolipids, specifically the monosialoganglioside GM1, we quantitated cholera toxin (CT) binding and adenylate cyclase activity. The calculated number of specific sites for CT binding to microvillus membrane (MVM) from newborn rabbits fasted for 72 h was decreased in MVM from proximal small bowel (7 +/- 0.8 x 10(8)/micrograms protein) compared to 72-h control neonatal rabbits (18 +/- 3.3 x 10(8) micrograms protein). In distal small bowel there was no difference in the calculated receptor sites/micrograms MVM protein between fasted (8 +/- 1.7 x 10(8)) and fed (11 +/- 4 x 10(8)) groups. MVM prepared from proximal small bowel of fed animals bound significantly more CT than MVM prepared from distal small bowel of fed animals. The affinity for CT was the same in all MVM preparations. Neuraminidase treatment of MVM resulted in increased CT binding in fed and fasted rabbit proximal and distal MVM preparations, but the greatest increase occurred in MVM prepared from proximal small bowel from fasted animals. There was no difference in adenylate cyclase activity in fed, fasted, and proximal or distal small bowel crude membrane preparations. Refeeding (120 h) resulted in normalization of CT binding in MVM from proximal small bowel of fasted animals. We conclude a 72-h fast in neonatal rabbits resulted in decreased regional CT binding in MVM prepared from proximal small bowel of fasted animals, but no change in adenylate cyclase activity. Refeeding reverses CT binding abnormalities.

Short-term malnutrition in neonatal rabbits: effect on gastrointestinal epithelial proliferation.

We determined DNA synthesis, cell turnover and cell migration in 72-h fasted and control neonatal rabbits because acute neonatal malnutrition causes specific regional alterations in brush border structure and function. Decreased cell turnover and/or migration might partly explain these observations. We injected fed and fasted neonatal rabbits with [3H]thymidine. At 1 and 72 h after injection we assessed cell proliferation and migration. Villus height was less in proximal small bowel from fasted rabbits (PF) than in fed controls (PC). Villus height was lower in proximal than in distal small bowel from fasted rabbits (DF). DNA synthesis, as measured by incorporation of [3H]thymidine into DNA, was less in PF and DF than in PC and distal small bowel from fed rabbits (DC) whether expressed per microgram DNA or per intestinal segment. Similarly, thymidine kinase activity was lower in PF and DF than in PC and DC. For both fed and fasted rabbits, more cells were present in the proliferative zone from proximal small bowel than in that from distal. Small bowel labeling index was less in PC than in DC, and less in DF than in DC. The ratio of the number of cells in the proliferative zone to the number of cells in crypt was greater in PC than in DC and greater in DF than in DC. The migration zone was significantly greater in PC than in PF and greater in the fed group in the proximal small bowel than in the distal small bowel. Thus, a 72-h fast in neonatal animals resulted in decreased DNA synthesis and cell migration. Regional alterations in cell proliferation occurred.(ABSTRACT TRUNCATED AT 250 WORDS)

Does tissue-type plasminogen activator have direct beneficial effects on the myocardium independent of its ability to lyse intracoronary thrombi?

Tissue-type plasminogen activator (t-PA) is a widely used thrombolytic agent for treating acute myocardial infarction. Some previous studies suggest that t-PA benefits the heart independently of lysing coronary artery thrombi. The purpose of this study was to determine whether t-PA directly affects infarct size independently of lysing coronary thrombi, affects the no-reflow phenomenon, and exacerbates intramyocardial hemorrhage. We used a canine model of 2 hours of occlusion of the left anterior descending coronary artery followed by 4 hours of reperfusion. t-PA was administered 30 minutes after occlusion and was continued for 2 hours. Myocardial infarct size as a percentage of the risk zone was similar between saline (28 +/- 8%) and t-PA (35 +/- 9%) groups in a low-dose study and between saline (46 +/- 12%) and t-PA (44 +/- 12%) groups in a high-dose study. t-PA did not improve no-reflow. Intramyocardial hemoglobin level within the infarct was similar between saline (16 micrograms/mg) and high-dose t-PA (12 micrograms/mg) groups. The extent of hemorrhage assessed by intramyocardial hemoglobin correlated with infarct size. Histologic evaluation revealed that microscopic hemorrhage was confined to zones of contraction band necrosis. Neutrophil infiltration during early reperfusion was prominent. In conclusion, t-PA did not directly benefit the myocardium or no-reflow. Its effects in patients are likely due to its ability to lyse thrombi. t-PA did not cause infiltration of hemorrhage into noninfarcted tissue. Reperfusion accelerates the inflammatory response after myocardial infarction and results in early, intense neutrophil infiltration.

Depletion of C6 prevents development of proteinuria in experimental membranous nephropathy in rats.

To study the possible role of the complement membrane attack complex, C5b-9, in an experimental rat model that is morphologically indistinguishable from membranous nephropathy in man (passive Heymann nephritis [PHN]), an antibody to rat C6 was used to deplete C6 levels to less than 5% of pretreatment values (C6D) during disease development. C3, C7, C8, and C9 levels were not different in C6D and control rats. After injection of nephritogenic quantities of 125I-anti-Fx1A antibody, the kinetics of disappearance of labeled IgG from the blood were identical in the complement deficient and sufficient groups, and glomerular deposition of 125I-antibody was the same in both groups at 5 days. Glomerular deposits of sheep IgG and C3 were also similar in C6D and controls, but glomerular deposits of C6 and C5b-9 neoantigens were markedly reduced or absent in C6 depleted rats. However, despite equivalent antibody deposits, proteinuria was abolished in C6D rats compared with normocomplementemic controls. Similar results were obtained when F(ab')2 anti-rat C6 IgG was used to deplete C6 during development of PHN. These results demonstrate that C6 is required for the development of the increased glomerular permeability that occurs in PHN, presumably because C6 is required for formation of C5b-9. We conclude that glomerular injury in the PHN model of membranous nephropathy in the rat is mediated by C5b-9.
