Human erythrocyte protein 4.2: isoform expression, differential splicing, and chromosomal assignment.

Human protein 4.2 (P4.2) is a major membrane skeletal protein in erythrocytes. Individuals with P4.2 deficiency exhibit spherocytosis and experience various degrees of hemolytic anemia, suggesting a role for this protein in maintaining stability and integrity of the membrane. Molecular cloning of P4.2 cDNAs showed that P4.2 is a transglutaminaselike molecule in erythrocytes but lacks the essential cysteine for cross-linking activity. Two cDNA isoforms have been identified from a human reticulocyte cDNA library, with the long isoform containing a 90-base pair (bp) in-frame insertion encoding an extra 30 amino acids near the N-terminus. Characterization of the P4.2 gene suggests differential splicing as the mechanism for generating these two cDNA isoforms. The donor site for the short isoform (P4.2S) agrees better with the consensus than the donor site for the long isoform (P4.2L) does. Expression of P4.2L was detected by a long-isoform-specific antibody raised against a peptide within the 30-amino acid insert. Western blot analyses showed P4.2L to be a minor membrane skeletal protein in human erythrocytes with an apparent molecular weight (mol wt) of approximately 3 Kd larger than the major protein 4.2, P4.2S. By in situ hybridization of a full-length 2.4-kilobase (kb) cDNA to human metaphase chromosomes, the gene for P4.2 was mapped to bands q15-q21 of chromosome 15, and it is not linked to the gene for coagulation factor XIIIa (plasma transglutaminase, TGase).

Determination of a cephalosporin antibiotic, ceftibuten, in human plasma with column-switching high-performance liquid chromatography with ultraviolet detection.

A column-switching high-performance liquid chromatographic assay combined with a heart-cutting technique and UV detection (LC/LC/UV) was developed to determine ceftibuten, a new oral cephalosporin, in human plasma. Plasma samples were directly injected into the first chromatographic column for sample cleanup and extraction. Thereafter, a switching valve, located at the junction of the first cleanup column and the second analytical column, opened during a 4-6-min interval after injection to transfer the segment containing ceftibuten into the analytical column for quantitation. The method was used routinely in pharmacokinetic studies and has the advantages of simplicity, improved sensitivity, and selectivity. Concentrations of ceftibuten in plasma over a range of 0.1-20 micrograms/mL can be determined with high precision and reproducibility. A single, oral, 200-mg dose of ceftibuten in humans resulted in a maximum concentration of 9.79 micrograms/mL, an area under the plot of concentration of drug in plasma versus time (0-12 h) of 47.77 micrograms.h/mL, and a half-life of 3.2 h. These results document the utility of the LC/LC/UV method in clinical pharmacokinetic studies.

A competitive enzyme immunoassay for the direct determination of mometasone furoate (SCH 32088) in human plasma.

Mometasone furoate (SCH 32088) is a synthetic corticosteroid which has a topical anti-inflammatory activity with a minimal potential for suppressing hypothalamic-pituitary-adrenocortical (HPA) axis. A sensitive competitive enzyme immunoassay (EIA) for measuring SCH 32088 in unextracted human plasma has been developed. The 3-(O-carboxymethyl)oxime (CMO) of SCH 32088 was synthesized and conjugated with bovine thyroglobulin, and the product was used for the production of antibodies in rabbits. SCH 32088-3-CMO was also conjugated with horseradish peroxidase, which was used as the tracer. The EIA thus developed can detect 1 pg SCH 32088 per assay or 25 pg per ml of human plasma. It can reliably quantitate SCH 32088 in human plasma from 50 pg ml-1 to 2.5 ng ml-1 with good linearity, accuracy and precision. The assay can be extended to measure SCH 32088 in plasma of laboratory animals. The availability of this sensitive assay makes it possible to evaluate the pharmacokinetics and toxicokinetics of SCH 32088 in laboratory animals and man.

Serial changes of serum interleukin-6, interleukin-8, and tumor necrosis factor alpha among patients with Kawasaki disease.

To determine the role of cytokines in Kawasaki disease, serial measurements of serum cytokine levels were done in 60 patients treated solely with aspirin. Coronary artery aneurysms later developed in 12 of them. The results suggest that elevated serum interleukin-6 and interleukin-8 levels during the first week of illness may be associated with a higher risk of coronary aneurysm formation.

Localization of the human CYP17 gene (cytochrome P450(17 alpha)) to 10q24.3 by fluorescence in situ hybridization and simultaneous chromosome banding.

The gene for human P450(17 alpha) (CYP17) was previously mapped to chromosome 10 through analysis of somatic cell hybrids. Using a modified procedure of fluorescence in situ hybridization, this gene has now been visualized on simultaneously banded chromosomes and localized to a specific subband of chromosome 10 at q24.3. This precise assignment may facilitate the understanding of the molecular basis of 17 alpha-hydroxylase/17,20-lyase deficiency and the evolution of the CYP superfamily of genes.

Mapping of 50 cosmid clones isolated from a flow-sorted human X chromosome library by fluorescence in situ hybridization.

Fifty cosmids have been mapped to metaphase chromosomes by fluorescence in situ hybridization under conditions that suppress signals from repetitive DNA sequences. The cosmid clones were isolated from a flow-sorted human X chromosome library. Thirty-eight of the clones were localized to chromosome X and 12 to autosomes such as chromosomes 3, 7, 8, 14, and 17. Although most of the cosmids mapped to the X chromosome appeared to be scattered along both the short and long arms, 10 cosmids were localized to the centromeric region of the chromosome. Southern blot analysis revealed that only two of these clones hybridized to probe pXBR-1, which detects the DXZ1 locus. In addition, 4 out of 5 cosmids mapped on chromosome 8 also localized on the centromeric region. While localization of X-specific cosmids will facilitate the physical mapping of the human X chromosome, cosmids mapped to the centromeric regions of chromosomes X and 8 should be especially useful for studying the structure and organization of these regions.

Correlation of in vitro release rate and in vivo absorption characteristics of four chlorpheniramine maleate extended-release formulations.

An in vitro/in vivo correlation was established for four formulations of chlorpheniramine maleate (histamine, H1-blocker) extended-release tablets exhibiting different in vitro release rate characteristics. In vitro release rate data were obtained for 12 individual tablets of each formulation using the USP Apparatus 2, paddle stirrer at 50 rpm in 1000 ml of distilled water at 37.0 +/- 0.5 degrees C. Inspection of the individual and mean release rate data indicated that the in vitro release rate of chlorpheniramine maleate was consistent with the intended design of the four extended-release formulations. The in vivo bioavailability and pharmacokinetics of these formulations were evaluated in 24 healthy subjects under fasting conditions. Wagner Nelson analyses of the in vivo data revealed extended release absorption profiles for all four formulations. Linear regression analyses of the mean percentage of dose absorbed versus the mean in vitro release resulted in a statistically significant correlation (r2 greater than 0.98, P less than 0.001) for each formulation. Qualitative rank-order correlations were observed among all combinations of in vitro and in vivo parameters. These data support a Level A correlation between the in vitro release rate profiles and the in vivo absorption for chlorpheniramine maleate determined under fasting conditions.

Extended thoracoscopic T2-sympathectomy in treatment of hyperhidrosis: experience with 130 consecutive cases.

A new method of thoracoscopic T2-sympathectomy mentioned in a previous report was used on 36 cases of hyperhidrosis at Tainan Municipal Hospital in Taiwan between October 1, 1989 and July 31, 1990. To reduce the possibility of incomplete resection of sympathetic nerve tracts, including ganglions and their regeneration, the method was modified on August 1, 1990. Thereafter, routine total removal of T2 and T3 sympathetic ganglions, as well as wide lateral incisions of the pleura on the second, third, and fourth rib beds were performed for treatment of hyperhidrosis. This newly modified method, "extended thoracoscopic T2-sympathectomy," can be performed easily by thoracoscopic approach in the treatment of hyperhidrosis palmaris. From August 1, 1990 to May 31, 1991, 130 consecutive cases of hyperhidrosis (56 males and 74 females) ranging in age from 8 to 51 years underwent extended thoracoscopic T2-sympathectomy. In addition to a nearly 100% cure rate of hyperhidrosis palmaris, significant saving in operative time and hospital stay were achieved. High simultaneous cure rate (70.6%) and subjective improvement (17.4%) of excessive sweating of feet (hyperhidrosis plantaris) were also noted in the 109 cases followed up, and complications were minor. Extended thoracoscopic T2-sympathectomy is not only a time-saving method but also a very simple and effective method in the treatment of hyperhidrosis. It is worthy of being propagated worldwide.

Denture stomatitis--a review of the aetiology, diagnosis and management.

Denture stomatitis is a common recurring problem of the denture wearers. The aetiology of the disease includes infection, trauma and probably a defect in the host defence mechanism. Current thinking suggests an interplay of most of these factors in the pathogenesis of the disease. The extent of interplay of these factors is still a controversy. Candida albicans has been implicated as the causative organism. However, in the light of recent research it is debatable if it is the only causative organism. Recently, cases resistant to antifungal therapy have been reported. In such cases other micro-organisms have been isolated. At the moment, comprehensive management includes meticulous denture hygiene together with anti-fungal or antibacterial therapy and correction of denture faults.

Studies on folk medicine "thang-kau-tin" from Taiwan. (I). The anti-inflammatory and liver-protective effect.

Five species of crude drugs are used as "Thang-kau-tin" on Taiwan market: (1) the stem of Mallotus repandus (Willd.) Muell.-Arg, (2) the stem and root of M. repandus (Willd.) Muell.-Arg, (3) the stem of Bauhinia championii Benth, (4) the stem with hooks of Uncaria hirsuta Haviland and (5) the stem with hooks of U. rhynchophylla Miquel. To clarify the effect of these crude drugs as anti-inflammatory and liver-protective agents, studies were conducted on water extracts of these five crude drugs. The statistical analysis (ANOVA) indicated that the stem of M. repandus showed the best anti-inflammatory activity against the paw edema induced by carrageenan. Nevertheless, the acute increase of GOT and GPT levels caused by CCl4 were markedly decreased by the treatment of M. repandus (stem), B. championii and U. hirsuta as a recipe group. The pathological changes around the central vein including fatty change, ballooning degeneration, cell necrosis, the increase in lymphocytes and Kupffer cells were improved by the treatment with the group of crude drugs as mentioned above.

Development of natural crude drug resources from Taiwan (X). Pharmacognostical studies on the Chinese crude drug "han-lian-cao".

In a previous paper(1), it was reported that the drug "Han-lian-cao", in Taiwan was derived from the entire plant or aerial part of Eclipta prostrata L. and Alternanthera sessilis R. Brown. In this paper, the histological studies of "Han-lian-cao" derived from Wedelia and Hypericum plants, and of 3 species of Wedelia related plants distributed in Taiwan were made. The results showed that it was possible to classify these species by their external and internal characteristics. In addition, a key to the characteristics of the whole plants of Eclipta, Alternanthera, Wedelia and Hypericum plants are hereby given.

Botany

The pharmacological and pathological studies on Taiwan folk medicine (VII): The anti-inflammatory effect of Echinops grjiisii.

The hepatotoxic-protective effects of "San-fang-feng" (the root of E. grijisii) and "Lou-lu" (the root of E. latifolius) on CCl4 induced hepatotoxicity have been proposed in our previous paper (Lin et al, 1990). The anti-inflammatory effects of these two crude drugs were investigated in this experiment. The results indicated that both of them displayed pronounced anti-inflammatory activities against carrageenan-induced edema. Furthermore, in order to isolated the main active components of E. grijisii, fractions obtained from the methanolic extract of E. grijisii were investigated in mice for their 24-h LD50 and 95% confidence limits, which could be used as a guiding for further animal experiments. Our findings demonstrated that n-hexane (100,300 mg/kg), chloroform (30,100, 300 mg/kg) and ethyl acetate (30,100, 300 mg/kg) fractions could markedly inhibit the carrageenan-induced inflammation, and the main active principle was found to be concentrated in the chloroform fraction, which possessed significant inhibitory activities even more than does indomethacin.

Animals

The pharmacological and pathological studies on Taiwan folk medicine (VIII): The anti-inflammatory and liver protective effects of "mu-mien".

The bark, xylem of stem, and root of (1) Bombax malabarica DC. and (2) Ceiba pentandra GAERTN. are marked as "Mu-mien" in Taiwan. In order to clarify the pharmacological effects of these three parts, anti-inflammatory and liver protective effect were evaluated with carrageenan-induced paw edema and CCl4-induced hepatotoxicity in rats. The statistical analysis (ANOVA) shows that all of the treatment used exhibited significant anti-inflammatory activity against carrageenan-induced edema. Furthermore, the administration of root and xylem of stem of B. malabarica showed the activity even better than indomethacin group did. However, only three used parts of B. malabarica significantly alleviated liver injury induced by CCl4. Meanwhile, the histological changes in rat hepatic tissues such as fatty change, ballooning degeneration, cell necrosis, lymphocytes and Kupffer cells were also observed.

Animals

Pharmacological and pathological studies on hepatic protective crude drugs from Taiwan (V): The effects of Bombax malabarica and Scutellaria rivularis.

Bombax malabarica DC and Scutellaria rivularis B. were extracted in boiling water and concentrated into 1g/ml solution to investigate their hepatoprotective effect. Carbon tertrachloride (CCL4) was injected into rat subcutaneously with a dose of 3.0 ml/kg to induce experimental acute hepatotoxicity in the animal. The activities of serum glumtamic-oxaloacetic transaminase (SGOT) and serum glutamicpyruvic transaminase (SGPT) were measured after 72 hours of CCL4 administration. The hepatopathological changes of the liver tissue were observed simultaneously with liver enzyme activities determination. The pharmacological effect of B. malabarica and S. rivularis of Taiwan was compared with that of the Bupleurum chinense from mainland China. B malabarica (p < 0.001), S. rivularis (p < 0.001) and B. chinense (p < 0.05) all demonstrated a significant reduction in the CCL4-induced SGOT and SGPT. Pathological studies of these three drugs extracts demonstrated a marked hepato-protective effects on CCL4-induced liver fatty degeneration and cell necrosis.

Alanine Transaminase

Crude drugs used for the treatment of diabetes mellitus in Taiwan.

A survey of hypoglycemic effective crude drugs in the market in Taiwan revealed 24 species of crude drug prescriptions, 34 kinds of crude drugs derived from 48 species of medicinal plants belonging to 25 families, all concerned with the treatment of diabetes mellitus. This paper updates a list of anti-diabetic crude drugs used as a remedy for the treatment of diabetes mellitus in Taiwan by means of ethnobotanical and pharmacological studies. Results showed that many of these crude drugs have hypoglycemic effects. Further investigation will be made.

Data Collection

Enhanced chemiluminescence with decreased antibody-dependent cellular cytotoxicity of human alveolar neutrophil in patients with adult respiratory distress syndrome.

In order to elucidate the intrinsic defects of inflammatory cells in adult respiratory distress syndrome (ARDS), 10 ARDS patients, 6 non-ARDS respiratory failure patients and 30 healthy controls were included in this study. The peripheral blood neutrophils (PBNs) were isolated from heparinized venous blood. Human alveolar neutrophils (HANs) were taken during bronchoalveolar lavage (BAL). Chemiluminescence was then measured with a luminometer (LKB Wallac) and antibody dependent cellular cytotoxicity (ADCC) was assayed using a Cr51 cytotoxic assay. The bacterial killing test utilized heparinized PBNs and Staphylococcus aureus. The results showed that the chemiluminescence of PBNs and HANs was higher in ARDS patients than in the normal control group (p < 0.05). However, ADCC levels in the PBNs and HANs of ARDS patients were much lower than in normal subjects (p < 0.01). The bacterial killing ability of PBNs was also impaired in ARDS patients, compared with either the normal control subjects or non-ARDS respiratory failure patients. In conclusion, chemiluminescence is increased in both PBNs and HANs of ARDS patients, which could partly explain the pathogenesis of acute lung injury. Furthermore, the suppression of ADCC in both PBNs and HANs as well as the impaired bacterial killing of PBNs may explain the impaired bacterial defenses and the consequent increase in pulmonary infection among ARDS patients.

Bilateral multiple sclerosing hemangiomas of the lung.

A Chinese woman had bilateral coin lesions of the lung, which grew slowly. At pathologic examination, the lesions were considered to be multiple sclerosing hemangiomas. Multiplicity is rare, and bilaterality has not been previously reported.

Female
