A reappraisal of routine marrow examination therapy of acute lymphoblasticv leukaemia.
Out of 557 routine marrow examinations performed in children receiving maintenance therapy, or having completed therapy, for acute lymphoblastic leukaemia or non-Hodgkin's lymphoma, only 9 marrow relapses were found. Out of 14 marrow examinations performed because of haematological or clinical indications of possible relapse, only 3 failed to confirm relapse. It is suggested that routine marrow examination to detect relapse is not worthwhile and should be stopped.

Fatal pulmonary fibrosis following 1,3-bis(2-chloroethyl)-1-nitrosourea (BCNU) therapy.
A case is reported of a child with fatal pulmonary fibrosis following BCNU therapy. This side effect has only once previously been reported with the nitrosourea group of compounds.
Management of congenital adrenal hyperplasia. Urinary steroid estimations--review of their value.
A retrospective study was made of 16 children with 21-hydroxylase-deficient congenital adrenal hyperplasia of the salt-losing variety, who were treated with fludrocortisone and prednisone and were in good health during the period under review. The height velocity of the children was subnormal, height achievement was poor, and their bone ages retarded. Urinary 17-oxosteroid and pregnanetriol excretion were used to monitor the therapy of the children and these data have been related to growth velocities. In spite of urinary steroid figures in excess of those published as desirable for monitoring therapy, the children failed to grow properly, probably as a result of glucocorticoid overdosage. Published urinary steroid criteria are considered too strict and in order to achieve them one would need to give unnecessarily high doses of steroid. Regular measurement of height velocity and skeletal maturation rate are better indicators of therapeutic control and should lead to more satisfactory growth and ultimate height.

[Medulloblastoma therapy studies MBL 80 and MED 84 of the Society of Pediatric Oncology and the Société Internationale d'Oncologie Pédiatrique (SIOP)].
In both trials the principle of "sandwich" chemotherapy (administered between surgery and postoperative irradiation) is studied in medulloblastoma. GPO-MBL 80 was essentially a one-arm trial; results after a mean observation period of 3 years show an expected event-free survival rate of 46% at 6 years. "Maintenance" chemotherapy with CCNU and vincristine did not further improve the results. Preliminary results of the joint, prospective, randomised forearm SIOP-GPO trial MED 84 are presented. It is apparently too early to answer any of the two main questions asked by this trial: a) is "sandwich" chemotherapy (as administered) of any value? b) can radiotherapy doses to the supratentorial area and to the spine be moderately reduced in so-called "low risk" patients without compromising long-term results? Thus far, we cannot observe any prognostic influence of the "classical" risk factors as established by other studies.
Video loop MRI of ocular motility disorders.
OBJECTIVE: Our aim was to evaluate a novel means of assessing ocular motility disorders using MRI. MATERIALS AND METHODS: A GE image of the orbit was acquired in a set plane each time the patient fixed on a series of points spread across the field of view. The images were transferred in sequence to a video recorder to create a cineloop of ocular movement. The technique was used in 33 patients with a range of ocular motility disorders including thyroid eye disease, posttraumatic diplopia, and Duane syndrome. RESULTS: In addition to anatomical detail, functional information could be derived from motion of the globe, motion of the optic nerve, and contractility of individual muscles. This is of particular use to the ophthalmic surgeon in disorders of ocular motility following eye surgery. CONCLUSION: A method of recording eye movements using MRI is described that offers useful functional information in the evaluation of disorders of ocular motility.
