Surgical management of acute injury of the ligaments of the knee joint.

Objective loss of stability is the indication for surgical reconstruction of acute knee ligament injuries. The final diagnosis is established under anaesthesia preceding the operation. This examination also determines the incision and approach. All structures of the knee joint are checked, lesions are precisely reconstructed. This is done by fine sutures in the case of interligamentary ruptures, pull-out sutures with cruciate lesions present, and fixation plate reinsertions of avulsions with or without osseous flakes. Menisci torn from their capsular base should be reinserted whenever possible. Lesions of the cartilage call for adequate primary management. Incisions are closed subsequent to suction drainage. The leg is positioned in a splint in elevation. Early functional therapy is indicated.

[Possibilities of geometric X-ray examination of the pelvis for partial pelvis replacement].

Patients with tumors of the pelvis, which cannot be treated by chemotherapy or radiation, must undergo hemipelvectomy. The postoperative prosthetic treatment was difficult und not always sufficient. So, a complete internal hemipelvectomy was done by replacement of an artificial part of half pelvis. Before the operation it was necessary to know the measurements of the pelvis exactly. This was done by taking X-ray pictures in anterior--posterior, lateral and axial direction which brought out the relevant diameters for the construction of the artificial pelvis. By using the Computer-Tomography we could get exact slices of the pelvis in the transversal plane. The slices were centrally positioned, then the model was covered with modelling-mass. The result was an exact positive feature of the pelvis which must be replaced.

[Nebacetin and taurolin as intraoperative disinfectant solutions in surgery of the locomotor system (author's transl)].

The anti-infective preventive action of intraoperative disinfection with physiological solution containing antibiotics, appears to be firmly established, judging from numerous publications. Since germs are showing a world-wide increasing resistance, the search for antibiotics which continue to be effective must be kept up, but alternative suggestions are equally necessary. Some publications have already been presented indicating that success similar to that previously obtained with antibiotics is possible by using disinfectants in the prophylaxis and treatment of infections. Hence, a comparative study was conducted using the non-absorbable broad-spectrum antibiotic Nebacetin and the disinfectant Taurolin in a 1% solution with a group of patients subjected to a total of 7699 major and minor surgical operations. Prospectively, early infections during the stay of the patient in the hospital, als well as disturbed wound healing, were included. No significant difference was found in the number of infections and the rate of disturbed wound healing. Indirectly, it is possible to conclude with the help of results from literature that the disinfectant Taurolin is suitable as an addition to intra-operative rinsing in the prophylaxis of infections.

[Experimental and clinical experiences with the synthetic skin cover of polyvinylalcohol-formaldehyde foam (PVA)].

Animal experimental research and clinical experience in polyvinylalcohol formaldehyde foam (PVA) as a temporary skin cover: The usefulness of PVA as a temporary skin cover of burns and infected wounds was studied in animals. On the back of 20 pigs full thickness burns were produced and wounds were infected, then examined macroscopically, histologically and bacteriologically over a period of 18 days. PVA showed a better wound cleansing, a significantly reduced bacterial flora and a more rapid growth of the epithelium in comparison with three other materials. The clinical use of PVA in the treatment of compound fractures with a severe damage of soft tissue, of infected wounds and of burns also demonstrated a quick cleansing of the wound and an induction of a dense and vascularised granulation tissue. Thus considerable infections could be avoided and early grafting was possible.

[Cortico-cancellous grafts in infected defect-pseudarthrosis].

Biomechanical studies of stabilizing effect of cortico-cancellous-grafts showed that the resistance of internal fixation is not endangered acutely under a one-foot weight bearing situation. The stability of defectosteosynthesis was significantly raised not only when a lateral joist was left but also when a medial cortico-cancellous-graft was fixed. 29 patients with chronical defectosteitis were treated by internal fixation and cortico-cancellous-grafts on the opposite side to improve mechanical stability and to place biological material for bridging.

[Biomechanical and histologic evaluation of refixation of ruptured collateral ligaments (author's transl)].

This paper describes the use of a new one-hole plate for the refixation of ligaments. The anchorage of ligaments was of sufficient strength as judged by biomechanical experiments on medial collateral ligaments of sheep in vivo and knees of human cadavers. Microangiographic and histologic investigations showed good vascularization and normal structure of the ligament under the one-hole plate.

[The use of carbon fiber strands for replacement of collateral ligaments at the knee joint (author's transl)].

Two different anchorage systems of carbon fiber strands were tested on 16 human cadaver knees. To pull the C-strands out of the scale and channel anchorage almost the same force was necessary as to break the natural collateral ligaments. The ingrowth of connective tissues and newly formed bone around the C-fibers was studied in 11 sheep. A biomechanical strength test revealed, 3 months after implantation, a 60% strength compared to the natural ligaments. Histologically a C-fiber connective tissue composite with collagen fibers was found. Newly formed bone closely surrounded the carbon. These results indicate that carbon fiber strands are suitable for ligament replacement, which allows early joint motion. Good long-term results could be assumed as a result of the high biocompatibility.

[In vivo synthesis of cartilage after transplantation of chondrocytes in animal experiments (author's transl)].

Long-term results after cartilage transplantation are not satisfactory owing to poor viability of the chondrocytes. In vitro experiments with isolated fetal and neonatal rabbit chondrocytes revealed a much better proliferative ability compared with adult cartilage derived cells. Cryopreservation of cell suspensions did not decrease the proliferative ability. In in vivo experiments, cultured neonatal, fetal, and cryopreserved rabbit chondrocytes were tested in transplants and compared with cell-free controls or radiated chondrocytes. These chondrocytic transplants showed good viability and seemed to synthesize cartilage matrix, which could be stained with safranin, whereas only fibrous tissue was found in controls.
