[Operative treatment of distal intra-articular humerus fractures; results of 412 follow-up cases (AO-collected statistics)].

Intraarticular fractures require a surgical therapy. The functional results are depending on the anatomical reconstruction of the joint surfaces and on early kinesitherapy. Remaining disabilities may be controlled in 2/3 of the cases to such an extent that no alterations are necessary in private or professional life. The overall result was very good in 23.8%, good in 36.0%, moderate in 22.3% and bad in 17.9%. These bad results are due to the rate of polytraumatized patients (18%) and the high rate of open comminuted fractures (27%).

[Thromboembolism prevention in surgical medicine: heparin or heparin-dihydroergotamine?].

This review summarizes clinical studies on prevention of venous thromboembolism by heparin or heparin and dihydroergotamine. Advantages and disadvantages of both drugs are described. The value of low-dose-heparin can no longer be seriously disputed. Up to now, however, there is no clear evidence that heparin-dihydroergotamine improves the efficacy and reduces the frequency of complications. The later drug is a new approach still under investigation.

The influence of various carbon fibre braiding techniques and methods of fixation on the extensibility of ligament prostheses.

An alloplastic ligament prosthesis with carbon fibres should not only be biocompatible but should also restore the biomechanical function of a natural system, which includes restoring its elasticity. Tests with new types of braided carbon fibre strands showed their elastic extensibility to increase with increasing number of filament tows and increasing braid angle. The best results were obtained with 32 tows and a 43 degrees braid angle. This strand type was tested in vitro as a medial collateral ligament prosthesis with two methods of fixation in human knees and, in like manner, in vivo in sheep knees. Biomechanical tests in vitro using human cadaver knees and sheep knees after 12 weeks' implantation showed an elasticity similar to that of normal natural ligaments.

Treatment by external fixation of open fractures associated with severe soft tissue damage of the leg. Biomechanical principles and clinical experience.

External fixation is the treatment of choice for fractures of the leg involving severe damage to the soft tissue. Each of the various patterns of application has advantages and disadvantages. Based on various biomechanical studies and clinical experience, the authors use a right-angle arrangement for the screws. If the type of fracture permits, internal and external fixation may significantly increase stability. During the period from 1978 to 1980, 56 patients with second- and third-degree open fractures were treated. A combination of external and internal fixation was used in 42 patients. Of the 44 patients re-evaluated at follow-up examinations, 19 had returned to unrestricted weight-bearing within 19.8 weeks. If no signs of bony union are noted ten to 12 weeks after the initial operation, cancellous bone grafting and internal fixation instead of external fixation are indicated. Twenty-three patients were fully weight-bearing an average of 14 weeks after internal fixation and cancellous bone grafting. Two patients had difficult problems and did not return to full weight-bearing until about 45 weeks after operation.

[Carbon fiber ligament replacement in the treatment of chronic instability of the external ankle joint ligament].

Ligament repair should be considered when a patient has disabling pain with giving way and chronic instability of the ankle from old ruptures of the lateral ligaments. Because of some incontestable disadvantages using Watson-Jones-or similar techniques we prefer the alloplastic with carbon fibres to reconstruct the anterior talofibular and calcaneofibular ligaments. Imitating the physiological course and insertions of the ligaments we managed to achieve a permanent lateral stability in 51 cases out of an operated group of 54 patients.
