Stability and compatibility studies of zorubicin in intravenous fluids and PVC infusion bags.

The stability of zorubicin (ZOR) in admixtures for continuous intravenous infusion was studied. ZOR was reconstituted and diluted to 600 micrograms ml-1 for simulated infusion and to 250 and 1000 micrograms ml-1 for storage in poly(vinyl chloride) (PVC) bags containing 5% dextrose injection or 0.9% sodium chloride injection (0.9% NaCl). Bags were then stored at refrigerated temperature (4 degrees C) and in the dark for 24 h. ZOR concentrations in each admixture were tested by stability-indicating high-performance liquid chromatographic (HPLC) assay with ultraviolet detection. No substantial loss of ZOR was observed during simulated infusions (n = 4) using PVC infusion bags and administration sets over a 1 h infusion. The drug stored at 4 degrees C in the dark in PVC bags showed that it is highly unstable at 250 micrograms ml-1 in 0.9% NaCl injection and in 5% dextrose injection. On the other hand, under the same storage conditions, at 1000 micrograms ml-1, ZOR is more stable in 0.9% NaCl injection (6 h) than in 5% dextrose (4 h). The reported superior stability of the 1000 micrograms ml-1 in 0.9% NaCl can be explained, at least in part, by the difference in pH. Changes in pH, particularly a decrease, seem to affect adversely the stability of ZOR. In fact, ZOR is rapidly converted into daunorubicin, the dominant degradation product, which is more cardiotoxic than the parent drug. Therefore, several precautions must be observed when the commercial product (Rubidazone) is prepared and reconstituted in i.v. fluids and containers.

Effect of the protein kinase C inhibitor GF 109 203X on elastase release and respiratory burst of human neutrophils.

1. The effects of bisindolylmaleimide GF 109 203X, reported to be a potent and highly selective inhibitor of protein kinase C (PKC), have been investigated on some human neutrophil functions. 2. GF 109 203X prevented O.2- production by NADPH-oxidase whatever the stimulus used for polymorphonuclear neutrophil (PMN) activation: directs PKC activators like phorbol myristate acetate (PMA) and dioctanoylglycerol, calcium ionophore (A23187), or receptor agonists like fMet-Leu-Phe (fMLP) and opsonized zymosan. 3. The effect of GF 109 203X was also examined on elastase exocytosis by neutrophils. PMA-mediated release was prevented by GF 109 203X. However, GF 109 203X had no effect on exocytosis induced by A23187 and the effect of this compound on the fMLP response changed according to its concentration. 4. These data suggest that PKC might be essential for stimulus-mediated O.2- production and also that PKC plays only a minor role in elastase secretion as compared to the role of the cytosolic calcium level.

Comparison of histamine-releasing factor recovered from skin and peripheral blood mononuclear cells of patients with chronic idiopathic urticaria.

BACKGROUND: The pathogenesis of chronic idiopathic urticaria is characterized by defective histamine release. Skin mast cells show an increased release of histamine while circulating basophils are less responsive to immunologic stimulus. OBJECTIVE: The purpose of the study was to examine and compare the production of the histamine-releasing factor in the skin and within the peripheral blood of patients with chronic idiopathic urticaria and normal control subjects, as a possible factor responsible for the difference observed in the releasability of both skin mast cells and basophils. METHODS: Using the skin chamber technique, histamine-releasing factor production and histamine concentration were assessed in normal-appearing skin of patients with chronic idiopathic urticaria (n = 12) and normal controls (n = 5) over a 2-hour observation period. In both groups, histamine-releasing factor production by peripheral blood mononuclear cells was also measured. RESULTS: The weighted average of histamine-releasing factor production during the 2-hour observation period was higher in the non-lesional skin of patients with chronic idiopathic urticaria as compared with normal controls (5.6 +/- 1.4% versus 0.7 +/- 0.6%, P < .01). In contrast, less histamine-releasing factor was produced by peripheral blood mononuclear cells in chronic urticaria as opposed to normal controls (17.2 +/- 2.1% versus 25.7 +/- 2.8%, P < .03). Spontaneous histamine concentration was not significantly different in patients with chronic urticaria than in normal controls. CONCLUSION: Histamine-releasing factor production is increased in the skin, and decreased in the peripheral blood of patients with chronic idiopathic urticaria when compared with nonatopic controls. The lower production of histamine releasing factor in the blood could be explained by the migration of activated T-lymphocytes in the skin.

Effect of dialysis on exogenous dopamine in haemodialysed critically ill patients.

A sensitive and specific high performance liquid chromatographic method (HPLC) was developed for measuring dopamine (DA) in human plasma samples. Dihydroxybenzylamine (DHBA) was employed as an internal standard. Following solvent extraction and separation on a C18 ion-pairing reversed phase column, the drug was detected by a fluorescence detector with excitation and emission wavelengths of 233 nm and 345 nm, respectively. This method was employed to evaluate dopamine dialysability in seven haemodialysed patients under continuous infusion of a positive inotropic agent. Results showed a high dialyser extraction ratio averaging 38.3%, but a low dialysis clearance of 67 ml/min. The fraction removed by dialysis is 2.5%. We concluded that haemodialysis has little effect on dopamine blood levels, and consequently on drug pharmacokinetics, when the drug is delivered to haemodialysed patients under intravenous infusion.

Determination of clozapine in serum by radioreceptor assay versus high-performance liquid chromatography: possible detection of hydroxy-metabolites.

Clozapine is an antipsychotic drug with few extra-pyramidal motor side-effects, used to treat schizophrenia which is resistant to classical neuroleptic therapy. This report shows that norclozapine but not clozapine-N-oxide has the same D2 receptor affinity as clozapine. Assay results suggest a bimodal distribution which may be explained by CYP1A2 polymorphism. Extensive metabolizers could produce other active metabolites, probably other hydroxy-clozapine derivatives.

[Gunshot injuries of the ureter].

The increased incidence of gunshot injuries of the ureter (GIU) can be explained by increased of armed violence in some large cities and by the performance of intensive care teams, both in civilian practice and in a context of war. The discovery of a GIU, during salvage laparotomy for vascular or visceral lesions is no longer exceptional. We report 5 cases of abdomen gunshot wounds with ureteric trauma treated between 1987 and 1994 by three surgical teams. The data in the literature and the principles of ballistic wounds are analysed. Theses lesions are initially misdiagnosed diagnosis in 10 to 20% of cases, as there are no specific clinical signs, radiological opacification of urinary tract is rarely performed, and septic nature of associated lesions and the ballistic context of the trauma guide the treatment of GIU. When the ureteric lesion is short and associated lesions are limited, the continuity of the urinary tract can be restored after debridement of the extremities by end-to-end anastomosis for the upper 2/3 and direct vesical reimplantation or into a psoas bladder for the lower 1/3. Drainage is ensured either by a bladder catheter or by a double J stent, for a minimal duration of 3 weeks. When there is a defect of the upper two-thirds of the ureter, mobilization of the kidney and its pedicle or transureteroureterostomy may be required. Urinary diversion by nephrostomy or in situ ureterostomy is indicated when the haemodynamic state is unstable and the associated lesions are very septic or in the presence of multiple lesions. Extensive contusion of the ureteric wall must be intubated to prevent fistula formation due to necrosis. Nephrectomy should be avoided in these patients with a mean age of 27 years.

[Entero-mesenteric infarction: diagnostic and predictive criteria of severity. Therapeutic implications. Multifactor prospective study of a series of 61 cases].

The study includes a group of 61 patients (31 males, 30 females), mean age 72 +/- 13 years with entero-mesenteric ischemia. The authors studied prospectively clinical, biological and radiological parameters in order to reinforce the diagnostic of the disease. Correlations between these parameters and peroperative findings and anatomopathology after resection were able to point out factors of severity. These factors should have a therapeutic incidence in order to improve the outcome usely bad.

[Rectopexy to the left inguinal ligament. Technique and preliminary results].

We report the procedure and the results of a new surgical technique for adult rectal prolapse by left inguinal ligament rectopexy. Seven patients were operated on, and presented with total rectal prolapse (n = 6) or internal procidentia associated with a solitary rectal ulcer syndrome (n = 1). Mean follow-up was 64 months (56-78 months). One patient experienced a postoperative complication: a regressive temporary dysesthesia of the left lateral femoral cutaneous nerve. Three patients had mild postoperative constipation. There was no recurrence. Preliminary results show that it is a suitable procedure. It may be successfully applied to the treatment of rectal prolapse and internal procidentia complicated by a solitary rectal ulcer syndrome, particularly when the use of synthetic material is precluded after a rectal wound. Further studies are required to confirm these preliminary results.

Effects of systemic corticosteroids on cutaneous histamine secretion and histamine-releasing factor in patients with chronic idiopathic urticaria.

BACKGROUND: Enhanced skin mast cell releasability of histamine, increased production of histamine releasing factor (HRF), and cutaneous inflammatory process are the hallmarks of chronic idiopathic urticaria (CU). Although H1-antihistamines are known to alleviate the symptoms effectively in most cases, systemic corticosteroids (CS) are given in more resistant patients. Their mode of action remains a matter of controversy. OBJECTIVES: In the present study, the effects of a 7-day course of CS or placebo on histamine content and HRF production in non-lesional skin of 19 CU patients were examined. METHODS: Using the skin chamber technique, HRF production and histamine content were assessed in normal-appearing skin of patients with CU over a 2-h observation period. Those two parameters were measured before and after treatment, in a double-blind fashion. RESULTS: No significant changes occurred in any parameters after placebo treatment. In contrast with this, significant decrease of HRF activity was observed after 1 week of oral methylprednisolone while no change was documented for histamine secretion. CONCLUSION: These data suggest that CS therapy improves symptoms of CU in association with a decreased production of HRF in uninvolved skin.

Free gadolinium and gadodiamide, a gadolinium chelate used in magnetic resonance imaging: evaluation of their in vitro effects on human neutrophil viability.

Gadolinium (Gd3+) is known to be the most paramagnetic ion but is also a very toxic cation often used in pharmacology as a putative stretch-activated channel inhibitor. Gadodiamide, a nonionic Gd3+ chelate, is frequently injected i.v. into magnetic resonance imaging (MRI) to enhance contrast. To determine whether this complex is innocuous in humans, a cytotoxicity study was performed on artificially stimulated human neutrophils (HN). HN were incubated with gadodiamide and with free Gd3+ (GdCl3). The purpose of the study was to estimate possible cell damage after incubation, and to validate further trials based on stimulated cellular models. Measurement of lactate dehydrogenase (LDH) release and the Trypan blue exclusion test were used to assess viability. HN were separated from the blood of healthy volunteers and stimulated by phorbol 12-myristate 13-acetate, a pharmacological reactive which induces protein kinase C activation, superoxide generation, and degranulation by leukotcytes. This study demonstrated that an acellular model is necessary to interpret LDH results. In addition, the experimental conditions of the study demonstrated GdCl3 toxicity on HN viability, while gadodiamide was not harmful.

Oxygen species scavenging activity of phenolic extracts from hawthorn fresh plant organs and pharmaceutical preparations.

Different extracts of fresh vegetative and reproductive organs from Crataegus monogyna harvested during a whole season and from some pharmaceutical hawthorn preparations exhibit in vitro antioxidant activities using three different models of oxygen reactive species generation (superoxide anion, hydrogen peroxide and hypochlorous acid). All the tested samples show low IC50 values, the most efficient being fresh young leaves, fresh floral buds and pharmaceutical dried flowers. The activities seem to be especially bound to the total phenolic proanthocyanidin and flavonoid contents.

Involvement of the extracellular calcium in the release of elastase and the human neutrophils oxidative burst.

Some proteases (particularly elastase) and metabolites of very reactive oxygen species (superoxide anion, hydrogen peroxide, hypochlorous acid and hydroxyl radicals) are generated by polymorphonuclear neutrophils (PMNs) during inflammatory disorders. Divalent cations, especially calcium (Ca2+) play an important regulatory role in the different PMNs functions. The aim of this study is to determine the role of extracellular calcium during the liberation of elastase and of reactive oxygen species production by human PMNs. Consequently, in order to stimulate PMNs, phorbolmyristate acetate (PMA), formyl-methionyl-leucyl-phenylalanine (fMLP) and opsonized zymosan (OZ) have been used. PMNs stimulated by OZ did not release elastase to reverse the PMA and fMLP systems. The production of elastase by PMNs stimulated by PMA to reverse the fMLP system is independent from the extracellular calcium, between 0.0 and 1.5 mM. With various higher concentrations of calcium, varying from 1.5 to 4.0 mM, the release of elastase by PMNs stimulated by PMA is extracellular calcium-dependent to reverse the fMLP system. The superoxide anion (O2-) generated by PMNs activated by fMLP is dependent from the extracellular calcium in the medium, whereas O2- production by PMA or OZ stimulated neutrophils was extracellular calcium-independent. These observations suggest that an influx of calcium may play an important role in the production of elastase and in the capacity of PMNs stimulated by fMLP to produce O2- to reverse the PMA and OZ systems.

Isolation of prostatic kallikrein hK2, also known as hGK-1, in human seminal plasma.

To demonstrate the presence of kallikrein hK2 in the human prostate and seminal plasma, we used mouse monoclonal antibodies (MAb) against a recombinant hK2-fusion protein. Using one of these MAb 9D5, we detected the presence of several major immunoreactive spots of 22 kDa and minor ones of 31 and 55 kDa in prostate cytosol and seminal plasma. After ion exchange and immunoaffinity chromatography of seminal plasma proteins, the 22-kDa immunoreactive proteins were isolated along with 55- and 75-kDa proteins. The NH2-terminal amino acid sequencing permitted identification of fragments of hK2 and protein C inhibitor, respectively, in the 22- ad 55-kDa bands. Furthermore, immunoblotting experiments in one and two-D gels with two different anti-hK2 MAbs and one polyclonal anti-PCI antibody suggested that the major 55- and 75-kDa bands were covalent hK2-PCI complexes containing either the full-length hK2 chain or only its carboxyterminal fragment in the presence of mercaptoethanol. These results demonstrate for the first time the existence of kallikrein hK2 and suggest that PCI may regulate its activity in seminal plasma.

Scavenging of reactive oxygen species by letosteine, a molecule with two blocked-SH groups. Comparison with free-SH drugs.

Acute production of reactive oxygen species by polymorphonuclear neutrophils during the respiratory burst may induce tissue injuries. In this in vitro study, it was demonstrated that letosteine, a mucolytic agent containing two blocked thiol groups, had antioxidant activity, but only when it was first submitted to alkaline hydrolysis. In a cell-free system, hydrogen peroxide, hypochlorous acid and hydroxyl radical concentrations were reduced by half by letosteine concentrations of 200, 15 and 350 mumol/l, respectively. The mechanism of letosteine action may be related to the -SH group liberated in vitro by hydrolysis, which seemed to react by scavenging the reactive oxygen species in the same way as acetylcysteine and MESNA, free-thiol drugs known for their antioxidant properties. So, letosteine, a compound with blocked -SH groups which in vivo can metabolically become free, may have a therapeutic application in preventing oxidative tissue injury damage induced by the respiratory burst.

[Perforated duodenal ulcer: subtotal anterior linear and posterior tuberous gastrectomy].

OBJECTIVE: To evaluate emergency surgery procedures for perforated duodenal ulcers. METHODS: Emergency surgery was performed for perforated duodenal ulcers in 25 patients (19 males and 6 females) with a mean age of 36 +/- 15 years. The procedure consisted of simple closure followed by anterior and partial posterior linear gastrectomy using a stapling device. There were 16 patients who smoked, 11 who drank and smoked, 3 who were under treatment with non-corticosteroid anti-inflammatory agents, 2 who had a history of other disease (myocardial infarction and arteritis) and 4 who were in septic shock. RESULTS: The delay in closure of perforation was less than 6 hours in 17 cases. Peritoneal leakage was confined to the supramesocolic level in 19 cases. The mean follow-up was 14 +/- 2 months with endoscopic control at 12 months. One recurrence (4%) was observed at 8 months. One patient died. Morbidity occurred in 7 patients (28%); epigastric bloating in 5 cases, subphrenic abscess in 1, abscess of the stomach wall in 1 and oesophageal reflux treated without surgery in 1. The mean duration of hospitalization was 13 +/- 3 days.

High-performance liquid chromatographic method for the determination of budesonide in bronchoalveolar lavage of asthmatic patients.

A simple, sensitive and selective method for the determination of budesonide in bronchoalveolar lavage (BAL) using high-performance liquid chromatography (HPLC) with UV detection was developed. BAL samples were extracted twice with methylene chloride, the extracts were centrifuged and the organic layer separated and dried under nitrogen. The samples were reconstituted in the mobile phase and 80 microliters were injected on to a Spherisorb ODS column with UV absorbance detection at 250 nm. The mobile phase was methanol-aqueous buffer (69:31, v/v). Inter-assay coefficients of variation were measured at 7.81 and 500 ng/ml with ranges of 0.89-7.31%. Average recoveries were 97% or greater. This method was successfully implemented for the analysis of BAL from asthmatics, in order to establish the amount of budesonide available to the lung and to investigate the efficacy of inhaler systems. Patients (n = 9) inhaled four puffs of 200 micrograms of budesonide and BAL was performed 10 min after the last inhalation. Only four BAL out of the nine presented detectable amounts of budesonide. The concentrations in BAL in these four patients were 13.44-84.18 ng/ml, corresponding to total amounts of 0.847-7.997 micrograms.

Popliteal artery entrapment syndrome. Anatomic and embryologic bases, diagnostic and therapeutic considerations following a series of 15 cases with a review of the literature.

We report 15 examples of popliteal artery entrapment syndrome observed in 11 patients. The anatomical causes were as follows: in one case, the popliteal artery presented an aberrant course medially to the medial head of the gastrocnemius muscle. In 5 cases, there was a small fibrous band linking the medial head of the gastrocnemius muscle to the lateral condyle and crossing behind the popliteal artery; in 5 cases this anomaly was also found in association with an abnormally high and/or internal insertion of the medial head of gastrocnemius muscle. In the last 4 cases, there was a muscular insertion anomaly associated with muscular hypertrophy causing arterial compression. Arteriography performed in the 11 patients showed evocative signs of the diagnosis in all cases where the artery was patent. Two popliteal arteries were occluded. CT scan and MRI examination of the popliteal fossa enabled us to define the muscular origin of the popliteal compression. All of the patients were operated upon; two received a reversed saphenous bypass and all of the others were treated by liberation of the popliteal artery and/or vein by a posterior approach. Follow-up in all patients at long term showed good prognosis. All of the patients were able to take up their previous physical activities without sequelae. Our review of the literature, which is based on 374 cases of popliteal artery entrapment observed in 280 patients, made it possible to define the frequency of the various anomalies observed, their symptoms and the different therapeutic possibilities. The multiple anatomical classifications as well as the arterial and muscular embryology are also described.
