Hepatic disposition of methohexitone in patients undergoing coronary bypass surgery.

In order to clarify the relative contribution of hepatic metabolism to the short term disposition of methohexitone, we have measured hepatic blood flow during induction of anaesthesia with a 1.5-mg kg-1 i.v. bolus dose of methohexitone. Median hepatic clearance was 1.01 litre min-1 and hepatic extraction 87%. As a consequence of the high hepatic extraction, the hepatic clearance of methohexitone was closely dependent on hepatic plasma flow.

Intra-articular and soft tissue injections.

Some practical knowledge of joint and soft tissue aspiration and injection techniques is useful in general practice. These techniques aid rapid diagnosis and facilitate treatment of common musculoskeletal disorders. Adverse effects are uncommon if the operator has sound anatomical knowledge and strictly adheres to an aseptic technique.

Detection and incidence of neutralizing antibodies to interferon-alpha-n1.

Sera obtained from 612 patients enrolled in 40 different clinical trials with an interferon-alpha-n1 (IFN-alpha-n1) preparation have been tested for neutralizing activity against this IFN in a cytopathic effect inhibition assay. Positive samples were assayed in triplicate. Of 391 patients treated for human papillomavirus infections, 13 developed neutralizing antibodies (3.3%); two suffering from condylomata acuminata and 11 from juvenile laryngeal papillomatosis (JLP). None has positive titers prior to receiving IFN-alpha-n1 and none appeared to lose the benefits of IFN treatment. Serial specimens from four JLP patients demonstrated that titers rose during IFN treatment and declined postherapy. None of the 221 cancer patients from whom serum samples were available had positive titers. The overall incidence of IFN neutralizing sera from these IFN-alpha-n1 recipients was 2.1%.

A multipurpose muscle ergometer.

A fast (0.1 mm steps within 2 ms), strong (40 N continuously) and accurate (resolution 0.002 N and 1.0 micron) muscle ergometer was developed to test dynamic and static properties of mammalian muscle. Both for twitches and for tetani isometric, isokinetic and isotonic contractions can be measured accurately. Force-velocity data and time to peak-force data of four EDL muscles, as well as force-extension data of their serial tendinous structures are shown to demonstrate the machine.

The painful swollen calf. A comparative evaluation of four investigative techniques.

Complications of popliteal cysts may closely mimic the clinical features of a deep venous thrombosis. We assessed the sensitivity and specificity of the non-invasive procedures of radionuclide venography and popliteal space ultrasound examination compared with those of contrast venography and arthrography, respectively, and then prospectively studied 23 non-surgical patients with acutely painful, swollen calves to determine the utility of these techniques. The cause of this symptom was popliteal cyst complications in 10 patients, deep venous thrombosis in seven patients, and both conditions in two patients. Radionuclide venography was highly reliable and ultrasound examination was specific but only moderately sensitive in these studies. The painful, swollen calf may be investigated adequately in most cases by means of noninvasive invasive techniques; contrast venography and arthrography should be reserved for only a minority of patients.

Cryptococcal sacroiliitis.

Disseminated cryptococcal disease is known to occur in patients with altered immune status. We have reported an unusual case of cryptococcal disease manifested solely by sacroiliitis, which occurred in a patient on chronic steroid therapy for autoimmune haemolytic anaemia. This case stresses the importance of considering unusual organisms as a cause of sepsis in patients with altered immune status.

Influences on measured intelligence in Down's syndrome.

The measured IQ and parental educational level of 57 children with Down's syndrome were compared. The phenomenon of decreasing IW with increasing chronological age in Down's syndrome was considered in the data analysis. There was no trend toward better cognitive performance by children of higher-educated parents. Thirty-one of the children had attended an early, continuous stimulation program. This experience accounted for most of the observed IQ variance.
