Regional variation in adipose tissue lipolysis in lean and obese men.

Biopsies of adipose tissue were obtained from two subcutaneous regions (abdominal and femoral) in a sample of 54 men (32 obese and 22 lean subjects). Clonidine-induced antilipolysis in femoral adipose cells was similar in both groups, whereas subcutaneous abdominal adipocytes of obese individuals showed a higher alpha 2-adrenergic response than did subcutaneous abdominal adipose cells from lean subjects. In addition, epinephrine had a biphasic effect in subcutaneous abdominal adipocytes from obese individuals, as it induced antilipolysis at low concentrations, and a net lipolytic response at higher doses. In contrast, the physiological amine promoted lipolysis in subcutaneous abdominal adipose cells of lean subjects. Epinephrine- and clonidine-induced antilipolysis of subcutaneous abdominal adipocytes was positively associated with the level of subcutaneous abdominal fat measured by computed tomography (CT). Finally, men with a high alpha 2-adrenergic response of subcutaneous abdominal fat cells were fatter than those with a low alpha 2-adrenergic component. These results suggest that, in men with a wide range of body fatness, variations in the lipolytic response of subcutaneous abdominal adipose cells to epinephrine appear to involve changes in the functional balance between alpha 2- and beta-adrenoceptors.

Normalization of the metabolic profile in obese women by exercise and a low fat diet.

This study was performed to evaluate the additive effect of exercise and a low fat diet on body weight, body composition, and the metabolic profile in four obese women who were previously exercise-trained for 15 months. This study therefore included regular aerobic exercise for 15 months and a low fat diet plus exercise for an additional period of 14 months. After 15 months, mean body weight and fat losses corresponded to 6.4 and 8.4 kg, respectively. Significant reductions (P less than 0.05) in plasma insulin, cholesterol, apo B, and LDL-C were also observed. Following the second part of the study, mean cumulative body weight and fat losses were 11.0 and 11.3 kg, respectively. At this time, the subjects were still overweight, but their plasma glucose and insulin during an oral glucose tolerance test were essentially similar to values obtained in a sample of 22 nonobese women. With the exception of plasma apo B and HDL-C levels, plasma lipid and lipoprotein levels were also comparable to those observed in nonobese controls. These results thus indicate that aerobic exercise-training and a low fat diet can normalize the metabolic profile of obese women, even if their adiposity remains higher than that of lean women.

Genetic influences on body composition and regional fat distribution.

It is clear that some people are more at risk of becoming overweight or Type I, II, III or IV obese because of the fact that they store ore mobilize fat more readily than others under identical energy intake, dietary composition and level of physical activity conditions. Much work is needed to elucidate the relative importance of genetic versus nongenetic determinants of overweight and obesity.

[Cervico-vaginal cytology and condylomata. Reproducibility and predictive value].

Human Papilloma Virus (HPV) is associated with two groups of intra-epithelial lesions of the cervix: the condyloma or the very low grade lesion, and intra-epithelial cervical neoplasia (CIN) or a higher grade of lesion. Cervico-vaginal cytology can be used to screen for these two types of lesion. Further to this research is to evaluate the reproducibility and the predicted value of cervico-vaginal cytology to detect condylomas. The study is based on 610 patients who were referred because of abnormal cytology. 270 (44%) of these patients were referred because the cytology suggested condylomas and 241 (40%) had cytological signs of CIN. The reference cytology and the control cytology did not accord well. Control cytology showed signs of condyloma in only 116 (43%) of the 270 patients referred for this type of lesion. On the other hand in the case of CIN there was better correlation. Of the 241 patients who were referred for cytological signs of CIN 185 (70%) also had these signs on controlled cytology. The positive predictive value of cytology to diagnose condyloma is relatively poor. In only 126 (47%) of the women referred with diagnosis of condyloma could this diagnosis be confirmed histologically, 20% of these 270 patients had CIN. In contrast the positive predictive value of cytology with signs of CIN were better-182 (76%) of 241 patients who were referred with CIN had the diagnosis confirmed histologically.

Maximal anaerobic performance of the knee extensor muscles during growth.

The extent of the growth changes in maximal work output during 10 s (MWO10), 30 s (MWO30), and 90 s (MWO90) of maximal repetitive knee flexions and extensions assessed on a modified Hydra-Gym machine was investigated in 84 boys and 83 girls, 9-19 yr of age. Body weight, fat mass and fat free mass by underwater weighing, and thigh volume and cross-sectional area were also determined. No difference was observed in the absolute MWO10, MWO30, and MWO90 between girls and boys at 9 and 11 yr of age. However, significant differences appeared between genders from 13 yr of age onward, anaerobic performances of the knee extensor muscles of girls representing about 75% or even less of those of boys. The analysis of variance revealed that maximal work ouput during the three knee extension tests was significantly greater in males as well as in females from 9 to 18 yr, regardless how performance was related to morphological characteristics. Performance in absolute values or expressed per unit of body weight, fat free mass, and thigh cross-sectional area for the MWO10, MWO30, and MWO90 tests were almost always significantly lower in both genders when performances of the 9-yr-old group were compared with those of the 13-yr-old group or older groups. Improvement in maximal work output during the 10-s, 30-s, or 90-s knee extension tests with age occurred mainly between 9 and 15 yr in both genders. The results of the present study show that there are gender differences in predominantly anaerobic performances during growth and reveal that increase in muscle mass does not appear to be the only factor responsible for the age-related increment in the anaerobic working capacity of the knee extensor muscles.

Heredity and the path to overweight and obesity.

Overweight and obesity are not homogeneous phenotypes as individuals differ in terms of the regional distribution of the excess weight or fat. Overweight and obesity are also complex multifactorial phenotypes influenced by both genetic and nongenetic determinants. Heritability of fat mass or percent body fat derived from underwater weighing measurement reaches about 25% of the age and gender adjusted phenotypic variance. Based on twin and parent-child data, it has been reported that the heritability of resting metabolic rate, thermic response to food, and energy cost of submaximal exercise, adjusted for the proper concomitants, is as high as 40%. The level of habitual physical activity also exhibits a significant heritability level on the order of about 25%. Experimental overfeeding with identical twins demonstrates that there are inherited differences in body weight and body composition response. The most important factor identified thus far to account for the individual differences in response to long-term overfeeding is a nutrient partitioning characteristic, namely the proportion of fat vs lean tissue gained. The high gainers are those storing energy primarily in the form of fat, while the low gainers are storing relatively more in the form of lean tissue.

The response to long-term overfeeding in identical twins.

We undertook this study to determine whether there are differences in the responses of different persons to long-term overfeeding and to assess the possibility that genotypes are involved in such differences. After a two-week base-line period, 12 pairs of young adult male monozygotic twins were overfed by 4.2 MJ (1000 kcal) per day, 6 days a week, for a total of 84 days during a 100-day period. The total excess amount each man consumed was 353 MJ (84,000 kcal). During overfeeding, individual changes in body composition and topography of fat deposition varied considerably. The mean weight gain was 8.1 kg, but the range was 4.3 to 13.3 kg. The similarity within each pair in the response to overfeeding was significant (P less than 0.05) with respect to body weight, percentage of fat, fat mass, and estimated subcutaneous fat, with about three times more variance among pairs than within pairs (r approximately 0.5). After adjustment for the gains in fat mass, the within-pair similarity was particularly evident with respect to the changes in regional fat distribution and amount of abdominal visceral fat (P less than 0.01), with about six times as much variance among pairs as within pairs (r approximately 0.7). We conclude that the most likely explanation for the intrapair similarity in the adaptation to long-term overfeeding and for the variations in weight gain and fat distribution among the pairs of twins is that genetic factors are involved. These may govern the tendency to store energy as either fat or lean tissue and the various determinants of the resting expenditure of energy.

Correlates of plasma very-low-density lipoprotein concentration and composition in premenopausal women.

Potential correlates of plasma very-low-density lipoprotein (VLDL) concentration and composition were studied in a sample of 75 premenopausal women. Fasting plasma free fatty acid (FFA) levels, as well as plasma glucose and insulin levels in the fasting state and during an oral glucose tolerance test, displayed significant positive correlations with plasma triglyceride (TG) and VLDL-TG levels (P less than .005). Plasma post-heparin lipoprotein lipase (LPL) activity, measured in a subsample of 31 women from the original sample, was negatively correlated with plasma TG, VLDL-cholesterol (CHOL), VLDL-TG, and VLDL-apolipoprotein (apo) B concentrations (.005 greater than P less than .05). Multivariate analyses showed that, after LPL was considered, the insulin area was the only other metabolic variable studied that was significantly correlated with VLDL-apo B concentration, whereas fasting FFA levels were significantly correlated with plasma TG and VLDL-TG levels. ANOVA revealed that plasma VLDL-CHOL, VLDL-TG, and VLDL-apo B levels were not associated with the glucose area, but were significantly associated with the insulin area (P less than .005). When the effect of insulin area was controlled for, the plasma FFA levels did not contribute significantly to the variance in VLDL-CHOL and VLDL-apo B, but showed an independent effect on VLDL-TG levels (P less than .05). Finally, stepwise multiple regression analyses indicated that once the variance explained by plasma LPL activity and by the insulin area was considered, no other metabolic variable could account for the variation in VLDL-CHOL and VLDL-apo B levels, whereas fasting FFA levels explained a further 5% of the VLDL-TG variance and one third of the variance observed in the VLDL-TG/apo B ratio.(ABSTRACT TRUNCATED AT 250 WORDS)

Macrophages can modify the nonpermissive nature of the adult mammalian central nervous system.

Although astrocytic gliosis has been linked to failure of axonal regeneration in the adult mammalian CNS, its role is not fully established. We used an in vitro assay to investigate the role of reactive astrocytes and macrophages in influencing axonal growth in the lesioned adult rat optic nerve. Soon after optic nerve transection, the nonpermissive nature of the optic nerve is altered to a permissive state near the lesion. This may account for injury-induced axonal sprouting and may contribute to the failure of these sprouts to elongate beyond the site of the lesion in vivo. We provide evidence that this lesion-induced change in the axonal growth-promoting properties of the CNS near the lesion may be produced by mononuclear phagocytes. In addition, several months after optic nerve transection, the degenerated nerves, which consist mainly of astrocytes and lack myelin, i.e., astrocytic "scar" tissue, are a good substrate for neurite growth. Taken together, these results suggest that in this in vitro system, substantial inhibitory effects are not associated with regions of astrocytic gliosis and that the nonpermissive nature of the CNS white matter can be modified by macrophages.

Using MZ twins in experimental research to test for the presence of a genotype-environment interaction effect.

Despite some evidence that genotype-environment interaction (G x E) effects may be involved in the variation observed in behavioral and biological traits, few attempts have been made to detect and quantify this component of genetic variation in humans. We propose that one way to achieve this goal is to challenge several genotypes in a similar manner, submitting both members of several MZ twin pairs to an ethically acceptable experimental treatment capable of inducing an adaptative response. In this situation, the G x E effect can be assessed with a two-way analysis of variance for repeated measures on one factor, the treatment effect. In this design, twins are considered nested within the pair, whereas the treatment effect is considered a fixed variable. The intrapair resemblance in the response to the treatment is quantified with an intraclass correlation coefficient computed with between-sibhips and within-sibhips means of squares. To illustrate this approach, changes induced by long-term endurance training were studied in 10 MZ twin pairs. Significant intrapair resemblance in the response of maximal oxygen uptake was observed, with about 7 to 8 times more variance between pairs than within pairs. This design with MZ twins may be helpful in the study of human variation for multifactorial phenotypes.

Effect of intensity of physical activity on body fatness and fat distribution.

To evaluate the effect of intensity of physical activity on body fatness and fat distribution, observations of 1366 women and 1257 men who participated in the 1981 Canada Fitness Survey were analyzed. Subjects were tested for energy expenditure of leisure-time activities and estimated maximal oxygen uptake (VO2max), body fatness was measured by subcutaneous skinfold thicknesses, and anthropometric measurements were made. Subjects of both sexes were categorized into four subgroups on the basis of their participation in leisure-time activities of various intensities. In general, subjects practicing vigorous activities on a regular basis had lower subcutaneous skinfold thicknesses and waist-to-hip ratios (WHRs) than those not performing these activities. These differences remained statistically significant after a covariance analysis was used to remove the effect of total energy expenditure of leisure-time activities on subcutaneous fat and fat distribution. Moreover, the WHR remained significantly lower in subjects performing high-intensity exercise after the effect of subcutaneous fat on fat distribution was adjusted for.

Dietary and physical determinants of menarche.

Age at menarche shows a downward secular trend and differs according to socioeconomic conditions, presumably the result of dietary variations. We report two studies conducted in Quebec City in which the relation between diet and menarche was evaluated. In 1978-1980, baseline information for these studies was obtained on girls aged 9 through 15 and included body weight, height, fatfold thickness at six sites, percent of body fat (derived from underwater weighing), and a 3-day dietary record. In a cross-sectional study of 207 girls aged 11 through 15, we compared dietary patterns between premenarcheal and menarcheal girls using logistic regression. In a 6-year follow-up study of 109 girls who had not experienced menarche at the baseline data collection, the relation between time of occurrence of menarche and dietary patterns was assessed using proportional hazards models. The results confirm the association between body weight and age at menarche. Body fat, however, was not strongly related to the onset of menstruation. In both studies, a higher dietary energy intake was associated with an earlier age at menarche. Dietary composition, and dietary fat in particular, was not related to menarche.

Abdominal fat cell lipolysis, body fat distribution, and metabolic variables in premenopausal women.

It is well established that abdominal obesity is related to numerous metabolic abnormalities and that this correlation represents a significant risk factor for coronary heart disease and related mortality. In the present study the relationships among the regional distribution of body fat, selected metabolic variables, and abdominal adipose cell lipolysis were investigated in 30 premenopausal women, 34 +/- 8 yr (mean +/- SD) of age, with body mass indices ranging from 17-45 kg/m2. Basal as well as epinephrine- and isoproterenol-stimulated lipolyses were positively correlated with fasting plasma insulin and triglyceride levels (0.48 less than r less than 0.64; 0.05 greater than P less than 0.0005 and 0.46 less than r less than 0.60; 0.05 greater than P less than 0.005, respectively) and with the insulin area measured during an oral glucose tolerance test (0.49 less than r less than 0.67; 0.005 greater than P less than 0.0005). With the exception of epinephrine-stimulated lipolysis, these correlations remained significant when lipolysis was corrected for cell surface area. Basal and maximal epinephrine- and isoproterenol-induced lipolyses were also negatively related to plasma high density lipoprotein cholesterol (-0.52 less than r less than -0.36; 0.05 greater than P less than 0.005). However, these relationships were no longer significant after control for fat cell surface. The associations between abdominal lipolysis and fat distribution did not remain significant when data were adjusted for total adiposity. Taken together, these results support the notion that variations in abdominal adipocyte lipolysis 1) depend more on total body fatness than on fat distribution, and 2) may be involved in the metabolic complications associated with abdominal obesity, particularly those pertaining to plasma insulin and triglyceride metabolism.

Effect of dietary fat content on total and regional adiposity in men and women.

The relationship between dietary fat intake, adiposity and regional subcutaneous fat distribution was studied in 344 men and 335 women (aged 35-55 years). Three-day food records were used to estimate total energy intake (EI) and percent intake of all macronutrients. Subjects were categorized as having an EI that was low (lower quantile) or high (upper quantile) in fat. Results showed that both men and women with a higher percent of EI in the form of fat weighed significantly (P less than 0.05) more, and had higher subcutaneous adiposity indices than subjects with a low fat intake. There was also a significant difference for both sexes between low and high fat intake groups in trunk and extremity skinfolds and trunk/extremity skinfolds ratio adjusted for the sum of skinfolds. However, this effect was not present when correlations between percent fat intake and body composition or regional fat distribution variables adjusted for total fat intake per unit of body mass were computed. These results suggest that a greater EI in the form of fat influences total adiposity, as well as regional subcutaneous fat distribution, only when high fat consumers are compared to low fat consumers.
