[Efficacy and tolerance of trimethoprim/sulfamethoxazole administered intramuscularly in patients with urinary tract infections (author's transl)].

The antimicrobial efficacy, general and local tolerance of a new intramuscular application form of trimethoprim/sulfamethoxazole was evaluated in a French-German clinical study. Fifty-five urological patients with urinary tract infections were treated with two i. m. injections (3 ml) daily over three to five days followed by oral medication. The study revealed excellent antimicrobial efficacy of the drug. In general TMP/SMZ was tolerated well after i. m. administration. Elevation of creatinine phosphokinase was observed in only three patients treated i. m. for five days. The high viscosity of the solution renders i. m. injection technically difficult, leading to a high proportion of local complications. The indications for use of i. m. administered TMP/SMZ in the hospital are limited, however it should be suitable for private practitioners in that an effective antimicrobial treatment can be started which can be continued by oral or intravenous medication.

[Renal pneumatisation simulating suppuration : possible consequence of renal artery embolisation. Two cases (author's transl)].

The authors report two cases of renal pneumatisation immediately following renal artery embolisation performed for carcinoma of the kidney. The particularly painful and inflammatory (high fever, leucocytosis, increased sedimentation rate) led to abdominal X-rays which revealed signs of gas in the kidney. In the first case this consisted of rounded clear areas distributed in a linear fashion and indicative of intravascular pneumatisation. In case n degrees 2, intravascular pneumatisation was accompanied by gas in the calyces. In the absence of suppuration and of proven infectious cause, the authors failed to find any explanation for the presence of gas bubbles in the vessels and calyces of the embolised kidney.

Epidemiological study of 314 cases of cancer of the bladder in the administrative department of Bas-Rhin, France.

This study is composed of two epidemiological investigations. The first concerns the geographical distribution of cancer of the bladder in the department of Bas-Rhin. The geographical distribution of 314 cases of cancer of the bladder in eight districts of the Bas-Rhin has been studied. The resultant map shows considerable variations in morbidity. The urban predominance may be due to environmental factors. The second enquiry concerns the frequency of multiple cancers of different primary sites occurring in cases of cancer of the bladder. Nine such cases have been identified. There may be a common predisposing factor in such cases. All patients were heavy smokers.

[Grawitz's tumor and provoked hyperlipemia (author's transl)].

After reporting a case of a patient aged 69 years with an inoperable hypernephroma, in whom the presence of a polycythemia and a reduction in the provoked hyperlipemia curve was found to co-exist, the authors review the data in the published literature on the influence on the small intestine of cancers other than those in the gastro-intestinal system. Basing their comments on studies of hypernephromas, they stress that a reduction or delay in the hyperlipemia curve was found in 23 out of 28 patients. This abnormality can only be explained by hypotheses at the present time.

Purification and some properties of tartrate-sensitive acid phosphatase from rabbit kidney cortex.

Two forms of tartrate-sensitive acid phosphatases (EC 3.1.3.2) were purified from rabbit kidney cortex by a multiple-column-chromatography method. The basic form constituted 90% of the enzyme and migrated as a single band of protein on polyacrylamide-gel electrophoresis. The proteins contaminating the acidic form did not exceed 5% of the total protein. The specific activity towards p-nitrophenyl phosphate was 12 mumol/min per mg for the basic form and 0.7 mumol/min per mg for the acidic form. The basic form of the enzyme differs from the acidic form in its heat-stability, Km values, inhibition rates by tartrate and fluoride and substrate specificities. Relative to p-nitrophenyl phosphate hydrolysis rate, the acidic form hydrolysed a variety of physiological monophosphate esters, whereas the basic form hydrolysed only CMP and phosphoenolpyruvate. Bacterial neuraminidases had no effect on the activity and mobility of the acidic form on polyacrylamide-gel electrophoresis. Both forms have the same molecular weight (101000 +/- 4000) and are probably composed of two identical subunits. The question whether the two forms of the enzyme are different proteins or whether one is a modified form of the other is discussed.

[Leydig cell tumours of the testis. Two cases presenting opposing sexual features : gynaecomastia, precocious virilisation, and revealed by study in vitro of aromatisation activity].

Two Leydig cell tumours presented with changes in sexual characteristics. The first patient was a child with precocious puberty whilst the second was that of an adult with recurrent gynaecomastia. Histochemical examination in vitro of tumour tissue from the second patient made it possible to explain the synthesis of oestrogens by a primary tissue destined to an essentially androgenic activity.

Ultrastructural changes of the intestinal wall after transileal cutaneous ureterostomy in men. A preliminary study. (With 1 color plate).

Pre- and postoperative specimens of ileal loops used in transileal ureteroplasty have been taken and ultrastructural modifications concerning the intestinal mucosa with special attention to the epithelial cells are described. The enterocytes undergo significant modifications because of the operation (i.e. collapse of striated border and entrancement of lysosomal inclusions). Contact with the urine brings about osmotic changes of the intestinal epithelium as well as irritation of the capillar endothelium. But most structural modifications observed histologically or by electron microscopy seem to be reversible. 6 months after the operation the histological structure of the ileum in contact with urine again becomes morphologically normal.

[Current considerations on male secretory sterility].

The problems of male sterility are considered in the light of fundamentals underlying the embryology and histophysiology of spermatogenesis and its hormonal control. Some forms of infertility are now understood but no treatment for them has yet been found. The mechanisms of other forms still elude firm definition: this is the case of, among others, some oligoasthenospermias in which, according to our experience, excessive estrogen production appears to play a physiopathologic role. In the prevailing uncertainty about therapeutic methods, the best short-term solutions in this field are in-vitro enhancement of the sperm's fecundating power and the improvement of artificial insemination techniques.

[A case of carcinoma of the stomach revealed by a testicular metastasis (author's transl)].

A testicular metastasis from a gastric carcinoma is exceptional among the rare secondary localisations of a visceral neoplasm in the testis. It should however be remembered for the testicular tumour sometimes precedes the clinical manifestations of the primary neoplasm. The diagnosis of the testicular metastasis from a gastrict carcinoma can only be made by histological examination and required the use of slides stained with mucicarmine. The method of spread to the testis seems mainly due to inversion of the lymph flow by neoplastic obstruction of the latero-aortic lymph nodes. In the cases reported in the literature, the presence of a testicular metastasis from a carcinoma of the stomach is a factor suggesting a poor prognosis.

Purification and some properties of arylsulphatases A and B from rabbit kidney cortex.

Arylsulphatases A and B (EC 3.1.6.1) of rabbit kidney cortex were purified 5250- and 7720-fold respectively by a multiple-column-chromatography method. The specific activity toward 4-nitrocatechol sulphate was 42mumol/min per mg for arylsulphatase A and 62 mumol/min per mg for arylsulphatase B. Each enzyme migrated as a single band on polyacrylamide-gel electrophoresis, and the enzyme activity corresponded to the band of protein on the gel. The rate of hydrolysis of ascorbic acid 2-sulphate by arylsulphatase A was three times that for cerebroside 3-sulphate. Arylsulphatase B hydrolysed UDP-N--acetylgalactosamine 4-sulphate and glucosamine 4,6-disulphate, but not galactosamine 6-sulphate.

[Experimental nephrotic syndrome in the rat. Biologic parameters and study of several hydrolases in different purified kidney fractions].

The induction of nephrotoxic nephritis in rats with rabbit antibodies preparation results in proteinuria, hypoproteinemia and hyperlipidemia with little glomerular lesions. A study of some hydrolases in cortex and medulla on one hand and glomerular and tubules on the other, showed changes in the activities of following enzymes. 1) A 20-30 % decrease in Na+, K+ dependent ATP-ase in whole kidney. 2) A 20 % decrease in beta-galactosidase activity in glomerular and medulla. 3) A 20 % increase of arylsulphatase A activity in tubules. These results are discussed in the light of the present knowledge of sulphatide metabolism in kidney.
