[Value of the tumor colony assay in therapy planning in malignant ovarian tumors].

The aim of our study was to determine the clinical value of the human tumour colony assay for the treatment of patients suffering from advanced malignant ovarian tumours. Using this in vitro culture system the growth and chemosensitivity of clonogenic tumour cells could be studied. Cultures were obtained of only 52.6% of the 133 tumour samples; only 33 of 70 assays showed a sufficient growth of colonies. However, the significance of the stem cell assay for clinical use is represented by the prediction of drug resistance. In 17 trials the assay had a 67% true positive rate and a 100% true negative rate for predicting drug sensitivity and resistance, respectively. Apart from the methodical errors inherent in this method, the false positive prediction of drug sensitivity might be caused by the heterogeneity of the tumour.

Gonadotropin measurements with fast, non-radioactive methods.

Different analytical methods for the measurement of lutropin and follitropin were compared. Lutropin concentrations of urine samples were measured with the semiquantitative HI Gonavis test (Moshida, Japan), and quantitatively by a modified Ovustick procedure (Monoclonal Antibodies Inc., USA) and by DELFIA (LKB Wallac, Finland). Semiquantitative and quantitative procedures showed conformity in 37 of 40 menstrual cycles when lutropin analyses were performed daily. Values from the analysis of serum samples by dissociation enhanced lanthanide fluoroimmunoassay (DELFIA) and Radioimmunoassay (Serono, FRG) revealed a high degree of correlation (lutropin r = 0.970, follitropin r = 0.994). Immunologic analysis with enzyme- or europium-labelled tracer is a practicable alternative to haemagglutination or radioimmunoassay procedures.

Pregnancy-specific parameters in early pregnancies after in vitro fertilization: prediction of the course of pregnancy.

The plasma concentrations of the beta subunit of human chorionic gonadotropin (beta-hCG), pregnancy-specific glycoprotein, and progesterone were measured in 37 pregnancies after in vitro fertilization. Twenty-one pregnancies ended with delivery, 9 as clinical abortions, and 7 as biochemical abortions. To predict the pregnancy outcome as early as possible, we designed a prediction graph by calculating a "cutoff line" using discriminant analysis and prediction bands up to 95% probability. This prediction graph can help in the estimation of the probability of pregnancy outcome with satisfactory reliability by determination of beta-hCG only.

Detection of antibodies in pregnancy serum reacting with isolated placental basement membrane collagen.

Adsorption of human IgG from pregnancy sera to placental basement membrane collagen could be demonstrated by an in-vitro assay. Polystyrene tubes were coated with purified type IV collagen and incubated with serum samples. Bound human IgG was detected using 125I labelled antihuman IgG. Median adsorption levels were 4000 (interquartile range 2500-9500) cpm/ml serum in 25 uncomplicated pregnancies and 21 000 (interquartile range 14 500-44 500) cpm/ml serum in 14 pre-eclamptic women. Specifity of the adsorption reaction could be demonstrated using uncoated tubes and collagenase treated coated tubes, which all showed low binding. In competition experiments with dissolved collagen fractions (type I, III, IV) most effective displacement of binding was observed with type IV collagen. This indicates collagen selectively of the adsorption reaction of human IgG to basement membrane collagen. Our in-vitro studies support immunohistological findings of IgG binding to trophoblastic basement membranes and their ultrastructural abnormalities detected in pre-eclamptic placenta.

Laminin in pregnancy.

Laminin, a noncollagenous glycoprotein component of basement membranes, was measured in human serum by radioimmunoassay for the laminin P1 fragment. The concentration of immunoreactive laminin increased during pregnancy. Before delivery it was nearly twice the level found in nonpregnant women. After delivery levels dropped. Decreased serum concentrations were detected in placental insufficiency whereas increased concentrations were found in sera of preeclamptic women. Immunoreactive serum laminin seems to correlate with the amount or degradation of placental basement membranes in pregnancy.

Serum melatonin is not affected by glucocorticoid replacement in congenital adrenal hyperplasia.

Recently a hypothesis has been proposed suggesting a negative feedback in the regulation of cortisol (F) and melatonin (Mel). To study a possible influence of F on Mel regulation we examined 13 children with congenital adrenal hyperplasia (CAH) on two occasions: once 3 days after cessation of F substitution (group 1; n = 13) and once during treatment with dexamethasone (1 mg/m2/day) and fludrocortisone (0.1 mg/m2/day) (group 2; n = 11) 11 children matched by sex and age served as controls (group 3). While serum 17 OH-progesterone levels, an indicator for the activation of the hypothalamo-pituitary-adrenal axis in CAH, were significantly (P less than 0.001) elevated in untreated patients (group 1), serum Mel levels were not different among the 3 groups nor was the diurnal secretion pattern of Mel affected. Nocturnal serum Mel concentrations, however, correlated with the age of the subjects (r = 0.55, P less than 0.001 at 23.00 h), displaying high values in early childhood that declined with progressing age. The presented data do not support the view of a classical feedback mechanism in the regulation of Mel and F in humans. However, it confirms the description of a tremendous fall of nocturnal Mel concentrations during childhood.

Small sucrose density gradients: reproducible and automatic fractionation using a commercial fraction collector (Pharmacia).

A simple, inexpensive modification of a programmable commercial fraction collector (Frac 100; Pharmacia, Sweden) was described. This modification allowed reproducible automatic fractionation of small sucrose gradients by rinsing the hanging drop at the delivery head of the fraction collector into the sampling tube, using signals from the fraction collector to control this specialized function. The described principle may also be useful whenever fraction size is near to drop volume and high resolution and reproducibility are desired.

[In vitro test for the radioimmunological demonstration of the adsorption of maternal antibodies to placental basal membrane collagen].

Polystyrene tubes were activated by adsorption of placental basement collagen (Type IV collagen). Binding of maternal immunoglobulin from serum samples to immobilized collagen was detected with 125I-labeled Antihuman IgG. An almost complete loss of binding resulted by adding solubilized Type IV collagen to the serum or pretreating the coated tubes with highly purified collagenase as well as using uncoated tubes. A high adsorption rate of IgG to Type IV collagen (2029 +/- 388 cpm) was detected in patients with moderate to severe gestosis (n = 16) (index greater than or equal to 4). 925 +/- 192 cpm were measured in mild gestosis (n = 13). Only 315 +/- 24 cpm could be detected in samples of a healthy control group (n = 17). A correlation was found between the index of gestosis and adsorption of maternal IgG to Type IV collagen (r = 0,823). It is concluded that the concentration and adsorption of immunoglobulins binding to basement membrane collagen is related to the severity of the disease and may play an important role in the development of symptoms of gestosis.

[Modification of semen quality by acupuncture in subfertile males].

Our first attempt in treating subfertility by acupuncture was carried out on 28 males. The results obtained from the experiences in veterinary medicine encouraged us to attempt this type of therapy. Each patient received a total of 10 treatments for a period of three weeks. Several spermiograms were made after diagnosis subfertility. We checked spermiograms and hormone levels before and after acupuncture. Total count, concentration and motility were evaluated. In all cases we observed a statistically significant improvement of sperm quality. Having obtained these good results in our first studies, we now intend to continue acupuncture therapy in subfertile males for insemination or intercourse at the time of the ovulation of their female partners.

Obstetric analgesia and immunoreactive endorphin peptides in maternal plasma during labor.

We studied the effect of obstetric analgesia on maternal plasma levels of immunoreactive endorphin peptides (ir-EP) during labor and the postpartum period in three groups of parturients: group I (n = 22) had no analgesia, group II (n = 20) received pethidine intramuscularly, and group III (n = 10) had continuous epidural analgesia. Initial levels of ir-EP were similar in all three groups. Patients without any medication and patients on pethidine showed a significant rise in ir-EP in late labor and at delivery. Epidural analgesia was characterized by constant levels of ir-EP during labor and an insignificant rise at delivery.

Evaluation and correlation of hormone levels in normal and pathologic pregnancies.

Estriol determination for monitoring high-risk pregnancies, either in serum or urine samples, has been in use for many years. The disadvantages are the inaccuracy of a 24-hour urine sample and the delay of two days. The advantages are a possible high variation of serum hormone evaluations from a very short sampling period. In order to prove the reliability of serum hormone patterns in pathological pregnancies, we compared them to 24-hours urinary E3 patterns. Free E3 and HCS levels were determined in serum samples by Radioimmunoassays. Total 24-hour urinary E3 was determined by a procedure according to Frandsen. For the graphic presentation, hormone levels from each pregnancy were grouped weekly and the mean values +/- SD in the different weeks of gestation were computed. To estimate the tendency of hormone patterns we performed a Z-transformation of grouped single hormone evaluations. A comparison between the serum and urine E3 measurements showed similar hormone patterns. E3 hormone patterns measured either in urine or serum proved to be well comparable. The mean of the transformed values showed the tendency of the hormone patterns even more distinctly than the graphic presentation. The hormone parameters E3 and HCS for monitoring high-risk pregnancies should be rated according to the diseases. In cases of placental insufficiency, E3 seemed to be the more sensitive parameter, whereas in severe cases of EPH-gestosis HCS might be more reliable.

[The betamethasone test in the diagnosis of placental insufficiency].

A trial to evaluate a functional test of the fetoplacental unit by measuring the extent of suppression of maternal estriol serum levels by glucocorticoids is described. 21 women between the 30th and 40th week of gestation received 4 mg betamethason intravenous injection at 8.30 a.m. Blood samples were collected immediately before betamethason application and at 1.00 p.m., 4.00 p.m., 8 p.m. o'clock and at 8.30 a.m. on the next day. Estriol (E3), cortisol, dehydroepiandrosterone sulfate (DHEA-S) and human placental lactogen (HPL) were determined from serum samples by radioimmunologic methods. Patients were divided into 3 groups: severe placental insufficiency (group I, n = 6) described by growth retardation of the fetus of more than 2 weeks, normal gestations (group II, n = 8) and slight growth retardation (group III, n = 7). After temporary suppression in groups II and III, E3, DHEA-S and cortisol levels showed a significant increase the day after betamethason. In group I an increase of these parameters was observed 2 to 5 days after betamethason. These results show that a low intravenous dosage of betamethason and collection of blood samples for E3 assays in intervals of 12 hours can be used as a functional test of the fetoplacental unit.

Immunoreactive endorphin peptides in amniotic fluid during labour.

Immunoreactive endorphin peptides (irEP) were measured in term amniotic fluids at induction by amniotomy (n = 8), during early and late first stage of labour (n = 52), at vaginal delivery (n = 30) and at caesarean section (n = 10). Individual values ranged from 50 to 222 pg/ml, but mean values did not differ significantly, and serial determinations revealed that irEP in the amniotic fluid remained almost constant in each woman from the first stage of labour through to delivery. There was no correlation between pathological changes in the cardiotocogram or low umbilical artery pH and irEP levels in the amniotic fluid at vaginal delivery. We conclude that immunoreactive endorphin peptides in the amniotic fluid probably are of fetal or placental origin and do not reflect fetal wellbeing or distress.

Influence of laparoscopic follicular aspiration under general anaesthesia on corpus luteum progesterone secretion in normal and clomiphene-stimulated cycles.

In 32 patients with unstimulated normal cycles and 24 with cycles stimulated with clomiphene and human chorionic gonadotrophin (hCG) all visible follicles were punctured laparoscopically under general anaesthesia for the purpose of in vitro fertilization. In unstimulated cycles the time of surgery was between 24 and 32 h after the first luteinizing hormone (LH) increase in the urine; in the cycles stimulated with hCG (5000 i.u.) laparoscopy was between 35 and 37 h after injection. Blood samples for progesterone determination were taken about 7 days later. Progesterone levels were compared with those in a control group not subjected to surgery, in which the progesterone levels were determined 7 days after the LH increase. There was no statistically significant difference in the progesterone levels in the unstimulated subjects after laparoscopy compared with those in the control subjects but progesterone levels in the stimulated subjects were significantly higher (p less than 0.01). Durations of the luteal phases showed no significant differences thus laparoscopy under general anaesthesia does not impair luteal function.

[Estriol serum levels following betamethasone treatment for induction of fetal lung maturity (author's transl)].

23 pregnant women with 28 to 37 weeks of gestation, threatened by premature labor, received Betamethasone 8 mg at admission and 4 mg 24 hours later to prevent newborn-RDS. Four women experienced this treatment twice. Blood was sampled for determination of estriol before each administration of Bethametasone and 24 hours after. Group I, patients with no signs of placental insufficiency, showed a drop in estriol levels at the second sample, but these recovered at the third one. There was no recovery seen at the third sample with group III, representing patients with proven placental insufficiency. Patients demonstrating pathologic CTG or delivering babies with low Apgar scores (Group II) showed no consistent course with their estriol levels. The course of estriol after administration of Betamethasone may be used as a test to determine the quality of the feto placental unit.

[Steroid receptor levels in human breast carcinomas in relation to serum hormone levels in postmenopausal patients (author's transl)].

Serum levels of LH, FSH, HPRL, oestradiol, progesterone and testosterone were determined by radioimmunoassay in 45 postmenopausal patients in the morning before they were operated on for primary breast cancer. Cytosol oestradiol - (E2R) and progesterone receptor (PgR) levels were estimated by Scatchard plot analysis of saturation assays using the charcoal method. When serum hormone levels and receptor contents were correlated according to the distribution free test of Spearman, all correlations were found to be insignificant, with the exception of E2R/PgR (p less than 0.001).

Inhibition of LH-release by synthetic analogues of LH-RH in rat pituitary cell culture.

Conditions for testing the inhibitory activity of synthetic analogues of LH-RH in monolayer cultures of rat pituitary cells were optimized. Nine synthetic derivatives of LH-RH were tested to evaluate their inhibitory dose50(ID50) values. The peptide antagonists tested included linear, branched-chain, dimeric and tetrameric derivatives of LH-RH, Ac-D-Phe1, D-Cpa2, D-Trp3,6-LH-RH (analogue V) was found to be the most effective competitive inhibitor of the series with an ID50 value of 6 x 10(-9)M in the presence of 10(-9) M LH-RH. The kinetics of the LH-RH-blocking action of two inhibitors were also studied. Both the linear (analogue I) and the dimeric (analogue VI) peptide showed immediate inhibition of HL-release. Prolonged inhibitory activity of the same two peptides was observed when cells were incubated with LH-Rh subsequent to preincubation with the inhibitors.
